    Traumatic hemipelvectomy: surgical and medical-legal aspects

    Background: Traffic accidents are major causes of disability and mortality due to the increasing number of vehicles, changes in lifestyle and high-risk behavior among the general population. Traumatic hemipelvectomy is a type of exceptional lesion representing 0.6% of all pelvic fractures, although lesions of the lower extremities are more usual than those of the upper extremities. Case presentation: The authors present an unusual case of traumatic left hemipelvectomy, and consider the medical-legal aspects resulting from an accident of a motorcycle driver, which involved a lateral collision between the subject’s motorcycle and a car.     Foreign bodies ingestion: what responsibility?

    The ingestion of foreign bodies is one of the most important and difficult emergencies for a physician to diagnose. Accidental ingestion is more common in children, in patients with dental implants, in individuals with mental disability and in drug users. Voluntary ingestion is found in patients who are psychologically unstable, in prisoners or those who attempt suicide. Foreign bodies may be divided into food as fish bones, chicken bones, food bolus, meat, etc. or real foreign bodies such as orthodontic implants, needles, pins, glass, coins, etc. The authors present a case of management, from the medicolegal point of view, of a female patient age 80, who complained, for some weeks of modest pain in the left iliac fossa, and afterwards the endoscopy showed a toothpick into the wall of the sigmoid colon. Assessed of the clinical status of the patient presented severe cardiac comorbidities so that before processing the patient to a second resolutive endoscopy, it was necessary to obtain the hemodynamic stability. However the management of cases of accidental ingestion of foreign bodies is particularly difficult. Medical errors can arise from the very first contact with the patient resulting in delays in appropriate treatment. The doctor to avoid compromising its position on medical liability, must use all the knowledge and diligence known by the art and science of medicine

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     USE OF PSYCHOTROPIC DRUGS IN MERCHANT SEAFARERS AND MEDICOLEGAL CONSIDERATIONS

    The work on merchant ships has always been associated with higher risk of fatal accidents and occupational diseases and, specifically, what characterizes the work at sea is the presence of a complex set of stressors that can affect the mental health of the people of sea. The authors present a study of the prevalence of psychopathological disorders and use of psychotropic drugs among Italian merchant ships long distance seafarers, analyzing, from a medicolegal point of view, their administration, sometimes necessary in the employment context, as well as the risk factors associated with the modality, the seat and the conditions in which this work takes place. We selected 600 workers who did never abuse alcohol or drugs and with apparent good physical health, interviewing them with the aid of a questionnaire. The analysis of the data collected showed that 78,3% of seafarers uses psychotropic drugs of which 53,3% is part of the deck section and 46,7% of the engine room. It was found that the psychotropic drugs most frequently consumed are anxiolytics (78,5%) and antidepressants (21,5%). Taking these drugs occurs without medical supervision, and in emergency situations while sailing the captain gives them, which although it has minimal rudiments of medicine, is not in a position to know the methods of consumption and especially the interruption manner of potentially dangerous drugs such as psychotropic drugs

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    The effect of 4-week endurance training on serum levels of irisin and betatrophin in streptozotocin- induced diabetic rats

    Betatrophin known as pancreatic β-cell proliferation marker is secreted as a result of the muscle irisin’s expression induced by exercise. The present study aimed to investigate the effect of endurance training on serum levels of irisin and betatrophin in diabetic rats. Twenty-four Wistar rats were randomly divided into three groups of (1) healthy control group (H-CG), (2) diabetic control group (D-CG), and diabetic group submitted to endurance training (D-ETG). The D-ETG performed endurance exercise (4 week/5 days) on the rodent treadmill. For data analysis we used one-way ANOVA, Scheffe test and Pearson correlation coefficient. Irisin (p =.04) and betatrophin (p =.005) levels were significantly decreased in the D-CG. Endurance exercise only increased serum levels of irisin significantly (p =.03). There was a significant correlation was shown between serum betatrophin and beta-cell function (p =.03). It appears that a specific exercise training can increase irisin hormone, with possible impact on betatrophin expression in diabetic individuals
