Ενδαγγειακή αντιμετώπιση αορτοβρογχικών και αορτοοισοφαγικών συριγγίων
Σκοπός – Υπόθεση: Τα αορτοβρογχικά και αορτοοισοφαγικά συρίγγια είναι σπάνιες παθολογίες αλλά μπορούν να καταστούν μοιραίες για τον ασθενή. Η ενδαγγειακή αντιμετώπιση των παθήσεων αυτών ξεκίνησε δειλά-δειλά την δεκαετία του ‘90 και έχει κλείσει περίπου είκοσι χρόνια εφαρμογής. Αποτελεί στις περισσότερες περιπτώσεις των παθήσεων αυτών τη μέθοδο εκλογής. Ανησυχίες εκφράζονται όσον αφορά τα απώτερα αποτελέσματα της μεθόδου. Σκοπός της παρούσης μελέτης είναι να παρουσιάσουμε τα μέχρι τώρα δημοσιευμένα αποτελέσματα στην ενδαγγειακή αντιμετώπιση των παθήσεων αυτών. Υλικό – Μέθοδος: Ερευνήθηκε η διεθνής βιβλιογραφία μέσω της βάσης δεδομένων PUBMED με λέξεις κλειδιά “aortobronchial fistula” & “aortobronchial fistulae” & “aortotracheal fistula” & “aortotracheal fistulae” & “aortopulmonary fistula” & “aortopulmonary fistula” & “aortoesophageal fistula” & “aortoesophageal fistulae”. Έγινε ανασκόπηση των ανωτέρω αποτελεσμάτων με κύριο κριτήριο την ύπαρξη ασθενών που αντιμετωπίστηκαν ενδαγγειακά για παρουσία αορτοβρογχικού ή αορτοοισοφαγικού συριγγίου. Για λόγους επεξεργασίας οι ανευρεθείσες μελέτες χωρίστηκαν σε δύο κατηγορίες, αυτές που παρουσίαζαν ενδαγγειακή αντιμετώπιση των αορτοβρογχικών συριγγίων και αυτές που παρουσίαζαν ενδαγγειακή αντιμετώπιση των αορτοοισοφαγικών συριγγίων. Αποτελέσματα: Ανευρέθηκαν 289 αποτελέσματα για αορτοβρογχικά συρίγγια και 482 για αορτοοισοφαγικά συρίγγια υπό μορφή περιλήψεων. Η πλειοψηφία των άρθρων ήταν παρουσίαση μεμονωμένων περιστατικών ή μικρών σειρών Καταγράφηκαν συνολικά 253 διαφορετικοί ασθενείς με αορτοβρογχικό συρίγγιο και 227 ασθενείς με αορτοοισοφαγικό συρίγγια που αντιμετωπίστηκαν ενδαγγειακά. Πέντε ήταν οι μελέτες με μεγάλο αριθμό ασθενών που ανευρέθηκαν σε όλη την αναζήτηση. Η κύρια παθολογία για την εμφάνιση αορτοβρογχικού συριγγίου ήταν η προηγηθείσα επέμβαση στην αορτή και η παρουσία αορτικού ανευρύσματος. Στις μελέτες των αορτοοισοφαγικών συριγγίων, η ενσφήνωση ξένου σώματος στον οισοφάγο και παθήσεις του οισοφάγου εμφάνιζαν μεγάλη ποσόστωση στην αιτιολογία αυτών. Ενώ στα αορτοβρογχικά συρίγγια τις περισσότερες φορές η ενδαγγειακή αντιμετώπιση ήταν και η οριστική, στα αορτοοισοφαγικά συρίγγια καταδείχθηκε ότι η προσθήκη είτε αποκοπής του οισοφάγου είτε εκτομής του βοηθά στην καλύτερη επιβίωση των ασθενών. Συμπεράσματα: Παρατηρείται διαφορά στα αποτελέσματα των ασθενών που υποβάλλονται σε ενδαγγειακή αντιμετώπιση αορτοβρογχικών και αορτοοισοφαγικών συριγγίων, όπως επίσης παρατηρείται διαφορά στην επιβίωση των ασθενών που υποβάλλονται σε ενδαγγειακή αποκατάσταση σε σχέση με τους ασθενείς που αντιμετωπίζονται εγχειρητικά. Τα αποτελέσματα δείχνουν καλύτερη βραχυπρόθεσμη επιβίωση της ομάδας με την ενδαγγειακή αντιμετώπιση συγκρινόμενη με την εγχειρητική ομάδα, με πολύ καλά πρώιμα αποτελέσματα της ενδαγγειακής αντιμετώπισης. Κατά συνέπεια προσδιορίζεται πλέον ως πιθανή μέθοδο εκλογής σε αυτούς του ασθενείς. Περαιτέρω αξιολόγηση της μεθόδου απαιτείται, με προοπτικές μελέτες, καταγραφή αποτελεσμάτων και προσδιορισμό των κατάλληλων πρωτοκόλλων παρακολούθησης.Objective: Aortobronchial and aortoesophageal fistula are rare conditions but may be lethal for the patients. Endovascular repair of the fistulas started in the 90s and stands in practice for almost twenty years. Most of the times represents the method of choice in these conditions. Concerns have been raised regarding late results of the method. The purpose of this review is to present published data of endovascular repair of aortobronchial and aortoesophageal fistulas. Materials and Methods: A search of PUBMED database was performed using as key-words “aortobronchial fistula” & “aortobronchial fistulae” & “aortotracheal fistula” & “aortotracheal fistulae” & “aortopulmonary fistula” & “aortopulmonary fistula” & “aortoesophageal fistula” & “aortoesophageal fistulae”. All papers that published any results regarding endovascular repair of the above fistula and were in English language were selected. For processing reasons the selected papers were assigned to two groups, one group with patients who underwent thoracic endovascular aortic repair (TEVAR) for aortobronchial fistula and a second group with patients who underwent TEVAR for aortoesophageal fistula. Results: Two hundred eighty nine (289) papers were initially identified for aortobronchial fistula and four hundred eighty two (482) papers were identified for aortoesophageal fistula. Most of the included papers were case repots or small case series. 253 patients with aortobronchial fistula and 227 patients with aortoesophageal fistula treated with TEVAR were pointed out. The main identified cause of aortobronchial fistula was previous aortic surgery and the presence of thoracic aneurysm. In the case of aortoesophageal fistula, ingestion of foreign body and esophageal disorders had a high percentage in the cause of these fistulas, along with previous aortic surgery and thoracic aneurysm. TEVAR may the definite therapy for aortobronchial fistulas, however in the cases with aortoesophageal fistula, esophageal transection or resection provides better outcome and survival rates. Conclusions: Results between patients who underwent TEVAR for aortobronchial fistula are different compared to those aortoesophageal fistula, as also difference outcome is observed between the TEVAR group and the group that underwent open surgery. Impressively good early outcomes of TEVAR for aortic fistulas with the bronchial tree or the esophagus, define it as the potential method of choice in these patients. Further evaluation of the method is needed, with prospective studies, outcome results and definition of appropriate follow-up protocols
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

Osteoblastoma of the rib: A rare benign tumor with an unusual location
AbstractINTRODUCTIONOsteoblastoma is a rare benign bone tumor commonly located at spine and long bones. However, rib involvement has been reported less frequently.PRESENTATION OF CASEIn this report, we describe a young adult male presenting with left posterolateral chest wall pain. Chest computed tomography showed a calcified tumor in the left third posterior segment of the rib. Radical mass resection was performed and histopathology confirmed a benign osteoblastoma. At last follow-up, 10 months postoperatively, the patient has eventually relieved of the unbearable chest pain. Imaging evaluation revealed no evidence of recurrent tumor.DISCUSSIONOsteoblastoma is an uncommon primary bone tumor accounting for only 1% of all bone tumors. Ribs are involved in less than 5% of patients. The disease has usually good prognosis with a tendency for local destruction and recurrence.CONCLUSIONRadical surgery remains the treatment of choice to prevent recurrences and to provide a definite diagnosis differentiating it from osteoblastoma-like osteosarcoma

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
