    National Institutes of Health classification for chronic graft-versus-host disease predicts outcome of allo-hematopoietic stem cell transplant after fludarabine-busulfan-antithymocyte globulin conditioning regimen

    Abstract In 2005, the National Institutes of Health (NIH) proposed standard criteria for diagnosis, organ scoring and global assessment of chronic graft-versus-host disease (cGvHD) severity. We retrospectively reclassified cGvHD with NIH criteria in a monocentric cohort of 130 consecutive adult patients with hematological malignancies presenting cGvHD after receiving allo-hematopoietic stem cell transplant (HSCT) with a fludarabine-busulfan-antithymocyte globulin (ATG) conditioning regimen, among 313 consecutive HSCT recipients. We compared NIH and Seattle classifications to correlate severity and outcome. The follow up range was effectively 2-120 months. Forty-four percent developed Seattle-defined cGvHD (22% limited, 78% extensive forms). Using NIH criteria, there were 23%, 40% and 37% mild, moderate and severe forms, respectively, and 58%, 32% and 8% classic cGvHD, late acute GvHD and overlap syndrome. Five-year overall survival was 55% (49-61), and cumulative incidences of non-relapse mortality (NRM) and relapse/progression at 2 years were 19% (14-23) and 19% (14-24). NIH mild and moderate forms were associated with better survival compared to severe cGvHD (hazard ratio [HR] = 3.28, 95% confidence interval [CI]: 1.38-7.82, p = 0.007), due to higher NRM among patients with severe cGvHD (HR = 3.04, 95% CI: 1.05-8.78, p = 0.04) but comparable relapse risk (p = NS).     Moving beyond Gunther Schuller’s Third Stream amalgamation of classical and jazz, this study explores whether the musical language of Olivier Messiaen can make a valid contribution to jazz piano performance. Initially, my project sought to answer such questions as: What elements of the musical language of Messiaen already exist in the jazz vocabulary? Am I able to extend this further? What are the timbral structures and pianistic effects within Messiaen’s musical language? What will be the most effective application of Messiaen’s musical language to jazz piano performance? Endeavouring to answer the final question led me to consider such aspects as whether the project should be limited to quoting Messiaen motifs, arranging Messiaen melodies, replacing jazz harmonic structures on standards with examples from Messiaen’s musical language or whether it would be better to approach the research conceptually. The work of Hubert Nuss provided encouraging reassurance that this was not an impossible task. In order to articulate this conception, the initial challenge was to decide how the classical and jazz worlds might meet in a ‘Messiaen’ technique. The approach adopted was similar to that used for undergraduate jazz study, namely, immersion in the piano scores and recordings of Messiaen’s music as well as by live performances. This was followed by the development and assessment of a contrived approach when specific techniques, such as tonal colourings or harmonic structures, were developed through prepared exercises and consciously included in my performance. It was then compared with an intuitive approach when no such precise parameters were established. This submission consists of CD recordings of two public recitals and an exegesis. It documents the development of this Messiaen technique and discusses its application in my performances.     Il dibattito pubblico sull’uso della guerra è particolarmente vivace all’inizio del 21° secolo. Diversi storici e scienziati politici affrontano questo delicato argomento nelle pubblicazioni sul ruolo internazionale degli Stati Uniti dopo l’11 settembre. Il saggio è una valutazione storiografica dell’argomento e si concentra in particolare sul confronto tra sostenitori di Bush e “liberals” negli ultimi due anni. Dopo una rassegna dell’approccio degli storici alla questione della guerra e della pace, l’autore approfondisce il più recente dibattito sollevato dalle politiche dei cosiddetti “vulcaniani”, ovvero i consiglieri militari del presidente Bush. Sebbene metta a confronto le posizioni assunte da falchi e colombe, neo-con e liberal, il saggio intende dimostrare come queste categorie non siano necessariamente applicabili oggi. Inoltre, mette in evidenza la rilevanza della politica interna nelle decisioni concernenti gli affari internazionali negli USA. Partendo dal “Soft power” di Joseph Nye per giungere allo scontro di Robert Kagan tra USA ed Europa, l’autore si concentra su ciò che i politologi Daalder e Lindsay hanno definito “la rivoluzione di Bush nella politica estera”. Nonostante l’apparente rottura con il tradizionale orientamento americano nelle questioni internazionali, l’approccio dei consiglieri per la sicurezza di Bush può essere ricollegato a diversi esempi passati nelle strategie di politica estera americana.The public debate in the United States on the use of war is very strong at the beginning of the 21st century. Several political scientists and historians are tackling this touchy issue in the recent literature on the international role of the Unites States after September 11. The article is a historiographic appraisal of the subject and concentrates especially on the discussion between the bushites and the liberals in the past two years. After an overview of the general approach historians had on the issue of war and peace, the author delves into the more recent debate raised by the policy of the so called volcanoes, Bush’s military advisors. Although comparing the attitudes of hawks and doves, of neo-cons and liberals, the essay shows how these categories do not necessarily apply to the present situation. Moreover it highlights the relevance of domestic politics in the decision concerning international affairs. From Joseph Nye’ soft power to Robert Kagan’s confrontation between the U.S. and Europe, the author concentrates on what political experts Daalder and Lindsay have called The Bush revolution in foreign policy. Although an apparent break with the traditional approach of the U.S. to international politics, Bush’s security advisors’ approach can be traced back to other examples of American foreign policy strategies

    L’autore ripercorre la sua esperienza di tirocinio curricolare svolta presso l'Istituto di Ricerche Internazionali Archivio Disarmo. Dalla partecipazione ad importanti eventi organizzati dall'associazione fino all'approfondimento della principale mansione realizzata. La stesura di tre pubblicazioni incentrate sul fenomeno del cosiddetto water grabbing e sull'impatto del climate change. Lo stress idrico di origine antropico correlato allo scioglimento dell’Antartide rivela l’estrema drammaticità della sfida ambientale, che si impone dinanzi alla comunità internazionale nel suo insieme. Infine, l'autore ha voluto divulgare la conoscenza riguardo al premio annuale organizzato esclusivamente da Archivio Disarmo: il premio giornalistico delle Colombe d'oro per la pace. The author retraces his experience of curricular training held at the Institute of International Research Archivio Disarmo. From the participation to important events organized by the association until the deepening of the main task realized. The writing of three publications focused on the phenomenon of so-called water grabbing and on the impact of climate change. The anthropogenic water stress associated with the dissolution of Antarctica reveals the extreme drama of the environmental challenge, which is imposed before the international community as a whole. Finally, the author wanted to disseminate knowledge about the annual award organized exclusively by Archivio Disarmo: the journalistic prize of the Colombe d'oro for peace

    [[abstract]]論文摘要 本論文意在藉《豐饒時節日課經》(Très Riches Heures)此一繪製作者與年代跨越法國十五世紀的祈禱書,探究作品背後變動的社會與文化現象。鑑於過去學者對〈十二月令圖〉單幅畫作的鑑定始終難有定論,且多半僅針對第一作者與贊助者,即蘭布兄弟和貝利公爵(1340-1416)的合作關係,而罕見作品完成者—傑昂•科倫布工作坊(c.1430-1493)與薩佛瓦公爵之深入研究。是故,本文將借用視覺文化的觀點,從贊助的角度切入,鋪陳作品從繪製到使用的可能脈絡,以求突破以往僅著眼於單一作者,純粹討論風格定位與傳承的侷限。文中第一章著重在月令系列圖像與風格的追溯和分析,除了彰顯貝利公爵如何基於個人需求,主導畫家的繪製內容外,更歸結出蘭布兄弟繪製的宮廷系列,也就是〈一月〉、〈四月〉、〈五月〉、〈八月〉,以及傑昂•科倫布工作坊繪製的勞務系列〈十一月〉。在引證前後兩工作坊的其他畫作後,分梳出該日課經中併置的「巴黎的國際歌德風」與「布魯日的地域風格」。第二章基於先前歸納的風格歧異性,以作者身份較明確的蘭布兄弟和傑昂•科倫布為對照,分別探討兩者地域背景、畫家與贊助者的關係,並推測工作坊可能的不同運作情形。第三章主要以貝利公爵為釋例,闡述西歐王室贊助者的實際需求與文化品味。第四章著眼於消費者的立場,配合十五世紀日課經閱讀文化之相關研究,強調《豐饒時節日課經》的使用脈絡與視覺性,歸論〈十二月令圖〉匯聚法國十五世紀前、後兩種視覺模式的時代意義。 出自蘭布兄弟之手的宮廷系列,表面上雖符合十二月令的圖像典型,但畫中再現的人事物,幾乎皆可從貝利公爵的財產清冊或傳記中尋得確實依據,可謂其個人生命與重要財產的圖像史料,豐富而多變。相較之下,傑昂科倫布工作坊繪製的部分,不僅少有贊助者日常活動的再現,更重複套用早年習自Jean Fouquet工作坊的藍色漸層薩佛瓦背景,儼然成為其風格的註冊商標。儘管該書前後兩位贊助者皆為法國皇室之後,但他們委託、贊助的機制卻漸呈商業化、現代化。貝利公爵挾其豐厚的政經實力,網羅所有手抄本專業人員於其門下,針對其個人品味與需求共同合作,訂製出專屬的私人祈禱書。他除了遵照西歐王室贊助傳統,提供藝師生活所需,並授與王室封號外,更經由相互贈禮,拉近彼此距離,建立主僕中帶有友情、箝制中帶有自由的微妙關係。相較之下,薩佛瓦公爵的委託方式則因除卻交遊、保持距離,而顯得效率較高,這與當時Bourges一地手抄本製作產業因大學興建和貿易頻繁,促使不同專業工作坊各自獨立、系統化合作的機制有關。

    Non-invasive ventilation indication for critically ill cancer patients admitted to the intensive care unit for acute respiratory failure (ARF) with associated cardiac dysfunction: Results from an observational study.

    BackgroundAcute respiratory failure (ARF) is a life-threatening complication in onco-hematology patients. Optimal ventilation strategy in immunocompromised patients has been highly controversial over the last decade. Data are lacking on patients presenting with ARF associating isolated cardiac dysfunction or in combination with another etiology. The aim of this study was to assess prognostic impact of initial ventilation strategy in onco-hematology patients presenting ARF with associated cardiac dysfunction.MethodsWe conducted an observational retrospective study in Institut Paoli-Calmettes, a cancer-referral center, assessing all critically ill cancer patients admitted to the ICU for a ARF with cardiac dysfunction.ResultsBetween 2010-2017, 127 patients were admitted. ICU and hospital mortality were 29% and 57%. Initial ventilation strategy was invasive mechanical ventilation (MV) in 21%. Others ventilation strategies were noninvasive ventilation (NIV) in 50%, associated with oxygen in 21% and high flow nasal oxygen (HFNO) in 29%, HFNO alone in 6% and standard oxygen in 23%. During ICU stay, 48% of patients required intubation. Multivariate analysis identified 3 independent factors associated with ICU mortality: SAPSII at admission (OR = 1.07/point, 95%CI = 1.03-1.11, pConclusionIn onco-hematology patients admitted for ARF with associated cardiac dysfunction, severity at ICU admission, invasive fungal infections and initial ventilation strategy were independently associated with ICU mortality. NIV was a protective factor on ICU mortality

    Venetoclax in Acute Myeloid Leukemia: Molecular Basis, Evidences for Preclinical and Clinical Efficacy and Strategies to Target Resistance

    Venetoclax is a BH3-mimetics agent specifically interacting with the antiapoptotic protein BCL-2, facilitating cytochrome c release from mitochondria, subsequent caspases activation, and cell death. Utilization of venetoclax has profoundly changed the landscape of treatment for the poor-prognosis category of AML patients unfit for intensive chemotherapy. In the phase III VIALE-A study, Venetoclax, in combination with the hypomethylating agent azacitidine, showed a 65% overall response rate and 14.7-month overall survival, in comparison with 22% and 8 months in the control arm. These results led to the widespread use of venetoclax in this indication. Other combination regimens, consisting of low-intensity, intensive, or targeted therapies are currently under evaluation. Despite promising results, preventing relapses or resistance to venetoclax is still an unmet clinical need. Numerous studies have been conducted to identify and overcome venetoclax resistance in preclinical models or in clinical trials, including the inhibition of other antiapoptotic proteins, the induction of proapoptotic BH3-only proteins, and/or the targeting of the mitochondrial metabolism and machinery
