    Developing a research community within an online healthcare feedback platform

    YesCare Opinion is an online feedback platform supporting patients to author stories about their care. It is not known whether authors would be willing to be involved in improving care through research. The aims of this study were to explore the views and preferences of Care Opinion authors about joining an online research community and to pilot new research community functionality. Five hundred and nine Care Opinion authors were invited to take part in an online survey in June 2019. Survey items included questions about participants' willingness to take part in research and their preferences for supporting processes. Data were analysed descriptively. Authors were invited to consent to join a research community and were asked to participate in three pilot studies. One hundred and sixty-three people consented to take part in the survey (32%). Participants indicated they would like to know the time commitment to the project (146, 90%), details about the organization carrying out the research (124, 76%) and safeguarding information (124, 76%). Over half indicated that they did not know how to get involved in healthcare research (87, 53%). Subsequently, 667 authors were invited to join the research community, 183 (27%) accepted, and three studies were matched to their expressed preferences for project attributes or organization type. Many people who leave online feedback about their experiences of healthcare are also willing to join a research community via that platform. They have strong preferences for supporting University and NHS research. Eligibility and acceptance rates to join pilot research studies varied. Further work is needed to grow the research community, increase its diversity, and create relevant and varied opportunities to support research.     Riverine plastic pollution from fisheries: Insights from the Ganges River system

The final version is available from Elsevier via the DOI in this recordAbandoned, lost or otherwise discarded fishing gear represents a substantial proportion of global marine plastic pollution and can cause significant environmental and socio-economic impacts. Yet little is known about its presence in, and implications for, freshwater ecosystems or its downstream contribution to plastic pollution in the ocean. This study documents fishing gear-related debris in one of the world's largest plastic pollution contributing river catchments, the Ganges. Riverbank surveys conducted along the length of the river, from the coast in Bangladesh to the Himalaya in India, show that derelict fishing gear density increases with proximity to the sea. Fishing nets were the main gear type by volume and all samples examined for polymer type were plastic. Illegal gear types and restricted net mesh sizes were also recorded. Socio-economic surveys of fisher communities explored the behavioural drivers of plastic waste input from one of the world's largest inland fisheries and revealed short gear lifespans and high turnover rates, lack of appropriate end-of-life gear disposal methods and ineffective fisheries regulations. A biodiversity threat assessment identified the air-breathing aquatic vertebrate species most at risk of entanglement in, and impacts from, derelict fishing gear; namely species of threatened freshwater turtle and otter, and the endangered Ganges river dolphin. This research demonstrates a need for targeted and practical interventions to limit the input of fisheries-related plastic pollution to this major river system and ultimately, the global ocean. The approach used in this study could be replicated to examine the inputs, socio-economic drivers and ecological impacts of this previously uncharacterised but important source of plastic pollution in other major rivers worldwide.European CommissionEngineering and Physical Sciences Research Council (EPSRC

    Long-chain omega-3 polyunsaturated fatty acids in relation to gut integrity, growth and cognitive development of rural African children

    Background and rationale: Weaning foods fed to infants in rural Gambia are often contaminated, resulting in infections which contribute to initiating a persistent inflammation of the gut. This enteropathy, which causes intestinal damage and malabsorption, is strongly associated with the high degree of growth faltering seen in Gambian infants. There is evidence that supplementary omega-3 long-chain polyunsaturated fatty acids (n-3 LCPs) might ameliorate this damage by reducing gastro-intestinal inflammation. Additionally, n-3 LCPs have been shown to benefit mental development and problem-solving ability in infants, but this has not yet been tested in an African population. Methods: A randomised, double-blind, controlled trial (500mg combined n-3 LCPs per day for six months) was conducted in a population of rural African infants aged 3 months - 9 months. The primary outcomes were infant anthropometric indicators and gut integrity (measured by urinary lactulose-mannitol ratios). Plasma fatty acid status (plasma fatty acid profiles), cognitive development (Willatts Test and an attention assessment at 12 months of age), intestinal mucosal inflammation (faecal calprotectin), and daily morbidities were the secondary outcome measures. Results: One-hundred and seventy-two Gambian infants completed the trial. Except for an increase in mid-upper-arm circumference z-scores in the intervention group (95% Cl: 0.06,0.56; p=0.017), no significant differences between treatment groups were detected for growth and lactulose-mannitol ratios at 9 months. At 12 months mid-upper-arm circumference remained greater in the intervention group, and significant increases in skinfold thicknesses were detected (pSO.022 for ali). Supplementation resulted in a significant increase in plasma n-3 LCP levels (p<O.001) and decrease in n-6 LCP:n-3 LCP ratios (p<O.OOl). Plasma n-6 fatty acid levels were not affected. No difference was detected for the other secondary outcomes. Conclusion: Fish oil supplementation proved safe and successfully increased plasma n-3 fatty acid status, but the results of this trial do not support the use of supplementary n-3 LCPs in young, breast-fed, rural Gambian infants for improving overall growth performance, intestinal integrity, and cognitive development, or reducing intestinal and systemic inflammatio
