Vortex Vein Imaging: What Can It Tell Us?

This report also details the technical advancements to image the vortex veins, such as ultra-widefield indocyanine green angiography, which are critical to elucidate the importance of the vortices in various retino-choroidal disorders. Future applications of these advanced imaging systems to better understand the role of the vortex veins in health and disease are also discussed.Keywords: choroid, optical coherence tomography angiography, pachychoroid disease, ultra-widefield imaging, vortex vein

SD OCT features of dry arcuate longstanding retinal folds
PURPOSE. To report the spectral domain (SD) findings in a retinal fold secondary to vitrectomy surgery. METHODS. A 57-year-old man presented 1 month after vitrectomy surgery with gas injection (C(3)F(8) 6%) in his left eye for treatment of a macula-on retinal detachment and was noted to have a retinal fold in the superior aspect of the macula. Complete ocular examination, fundus photography, fluorescein angiography, autofluorescence, and SD optical coherence tomography (OCT) were performed. RESULTS. Spectral domain OCT revealed a slightly hyperreflective lesion in the outer retina, which occupied a discontinuity in the inner segment-outer segment junction and extended superiorly with apparent indentation of the overlying retinal layers. CONCLUSIONS. This case illustrates how SD OCT may be useful in describing the morphologic substrate of retinal fold after retinal surgery
Impact of Choriocapillaris Flow on Multifocal Electroretinography in Intermediate Age-Related Macular Degeneration Eyes
PURPOSE: To investigate the relationship between perfusion of the choriocapillaris (CC) and macular function in eyes with intermediate age-related macular degeneration.
METHODS:
In this prospective, observational, cross-sectional study, macular optical coherence tomography angiography images and multifocal electroretinograms were obtained in 20 eyes with intermediate age-related macular degeneration from 20 patients. The main outcome measures were (1) the percent nonperfused choriocapillaris area (PNPCA), which represents a measure of the total area of CC vascular dropout, and (2) the average size of the CC signal voids, which represent contiguous regions of CC dropout. Furthermore, amplitude and implicit time of multifocal electroretinograms N1 and P1 waves in the two central rings (R1 and R2) were included in the analysis.
RESULTS:
Of the 20 patients enrolled, only 17 eyes from 17 patients (13 women) were included in this analysis. Three patients were excluded because of poor scan quality. Mean ± SD age was 75.1 ± 7.9 years (range, 62-89 years). The best corrected visual acuity was 0.17 ± 0.13 logarithm of the minimum angle of resolution. In univariate analysis, both the PNPCA and average signal void size were found to have a significant direct relationship with N1 implicit time in the R2 ring (P = 0.006 and P = 0.035, respectively). Neither PNPCA nor the average signal void size was associated with P1 or N1 implicit times in R1.
CONCLUSIONS:
In intermediate age-related macular degeneration eyes, PNPCA and average signal void size are related to N1 multifocal electroretinogram implicit times, which suggests an association between CC perfusion and photoreceptor function
TYPE 3 NEOVASCULARIZATION Evolution, Association With Pigment Epithelial Detachment, and Treatment Response as Revealed by Spectral Domain Optical Coherence Tomography
Purpose: To demonstrate the evolution and treatment response of Type 3 neovascularization using spectral domain optical coherence tomography. Methods: We retrospectively analyzed 40 eyes treated with intravitreal anti-vascular endothelial growth factor therapy for Type 3 neovascularization over a variable follow-up period. Results: In 17 eyes, spectral domain optical coherence tomography captured the development of Type 3 neovascularization from punctate hyperreflective foci that preceded any outer retinal defect. The more mature Type 3 lesions were associated with outer retinal disruption and adjacent cystoid macular edema. In addition, 37 of 40 Type 3 lesions (93%) were associated with an underlying pigment epithelial detachment, of which 26 (70%) were drusenoid, 6 (16%) serous, and 5 (14%) mixed. Type 3 vessels appeared to leak fluid into the pigment epithelial detachment cavity, creating serous pigment epithelial detachments as large as 925 mm in maximal height. Treatment with anti-vascular endothelial growth factor agents led to prompt involution of the lesion and resorption of the intraretinal and subretinal pigment epithelium fluid after one or two injections (median = 1). Conclusion: In some eyes with age-related macular degeneration, the earliest sign of Type 3 neovascularization is punctate hyperreflective foci above the external limiting membrane. The mature Type 3 lesions and associated serous pigment epithelial detachments are highly responsive to anti-vascular endothelial growth factor therapy. OI Nagiel, Aaron/0000-0001-7275-698
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
