Non-invasive differential diagnosis of noisy breathing infants and toddlers
Volgens internationaal onderzoek ervaart bijna 50% van de kinderen ten minste
één episode van piepende ademhaling in hun eerste levensjaar. Reutelen komt
echter ook veel voor in deze leeftijdsgroep, maar wordt vaak niet herkend als een
apart ziektebeeld, wat leidt tot misdiagnose en ondervertegenwoordiging in veel
onderzoeken. Piepen en reutelen zijn vormen van luidruchtige ademhaling met
verschillende akoestische kenmerken, oorzaken, natuurlijke geschiedenis en
reactie op behandeling. Momenteel zijn er geen objectieve diagnosetechnieken
voor baby’s met luidruchtige ademhaling en de overeenkomst tussen hoe
verschillende artsen ademhalingsgeluiden beoordelen is beperkt. Een vroege
differentiële diagnose (bij de eerste symptomen) op basis van objectieve en
niet-invasieve technieken zou een verkeerde diagnose en daaropvolgende onderof overbehandeling kunnen voorkomen.
Het hoofddoel van dit proefschrift was het onderzoeken van het klinische
potentieel van twee niet-invasieve methoden voor betere diagnose van subtypes
van luidruchtige ademen (piepen en reutelen) bij kinderen tussen 2 en 18
maanden oud: (i) analyse van uitgeademde vluchtige organische componenten
(VOC's), en (ii) akoestische analyse van opgenomen ademgeluiden met behulp
van machine learning algoritmen (DEEL I). De methode voor het verzamelen en
analyseren van uitgeademde lucht werd geoptimaliseerd (offline selected ion flow
tube mass spectrometry (SIFT-MS), een voor deze leeftijdsgroep aangepaste
adem sampler en de invloed van de ademhalingsroute (mond of neus) werd
onderzocht (DEEL II).
De Non-invasive Noisy breathing Infant (NiNbI) studie (DEEL I) telde niet
voldoende kinderen met een piepende ademhaling om onderscheid te kunnen

het ware werden uitgekozen. Ook toont dit aan dat reutelen een apart ziektebeeld
is, dat ten onrechte wordt/werd beschouwd als piepen en aldus onjuist wordt
behandeld.
Op basis van de uitgeademde lucht (hoofdstuk 1) kon een onderscheid worden
gemaakt tussen verschillende ernstgraden van reutelen (mild, matig, ernstig) en
herstelde kinderen, en tussen herstelde/milde en matige/ernstige reutelaars.
VOC's in uitgeademde lucht werden gemeten met behulp van
gaschromatografische massaspectrometrie (GC-MS) en offline SIFT-MS, die
vergelijkbare resultaten toonden bij het onderscheiden van de verschillende
patiëntengroepen. Toevoeging van door de ouders gemelde symptomen in de
afgelopen drie dagen aan het diagnostische classificatiemodel verbeterde de
classificatie, met name voor het onderscheid tussen licht, matig en ernstig
reutelen.
Akoestische analyse van ademgeluiden opgenomen met een contactloze
microfoon gebruikmakend van machine learning algoritmen was in staat de scores
voor de ernst van het reutelen, gegeven door een expertpanel van kinderartsen,
te voorspellen (hoofdstuk 2). Een dergelijke geautomatiseerde detectie van
reutelen zou idealiter gecombineerd worden met geautomatiseerde detectie van
piepen, om de differentiële diagnostiek te verbeteren in de klinische praktijk, waar
een lage overeenkomst tussen waarnemers over de evaluatie van
ademhalingsgeluiden een probleem kan zijn. Een grotere dataset van opgenomen
ademhalingsgeluiden (zowel piepen als reutelen) met meer gedetailleerde
beschrijvingen van een expertpanel van kinderartsen zou een betere training en
validatie van het machine learning algoritme mogelijk maken.
Offline SIFT-MS-analyse van uitgeademde monsters werd gerealiseerd door een
indirecte combinatie met thermische desorptie (TD). Deze methode toonde een
goede overeenkomst met de real-time analyse van de geselecteerde veelvoorkomende VOC’s in uitgeademde lucht, en bleek voldoende geschikt te zijn
(hoofdstuk 3). Verdere optimalisering van de methode zou de nauwkeurigheid
kunnen verhogen. Een commercieel verkrijgbare ademsampler werd aangepast
met een Teflon tussenstuk dat het gebruik van goed passende gezichtsmaskers
voor zuigelingen mogelijk maakt (hoofdstuk 4). Deze aangepaste ademsampler
leverde soortgelijke resultaten op als de originele voor de geselecteerde VOC's,
zowel bij dynamische als continue bemonstering. Met het gezichtsmasker konden
de zuigelingen zowel door hun mond als door hun neus ademen. Beide
ademhalingsroutes vertoonden relatief vergelijkbare patronen gemeten met realtime SIFT-MS en waren bij volwassenen sterk gecorreleerd (hoofdstuk 5).
De NiNbI-studie leverde pilot-data en eerste resultaten op over het klinische
potentieel van uitgeademde vluchtige biomerkers en akoestische analyse van
opgenomen ademgeluiden voor (i) het onderscheiden van reutelaars op basis van

different aetiologies, natural histories and different responses to treatment. At
present, objective diagnostic techniques for noisy breathing infants are lacking
and the agreement between observers about stethoscope evaluation of
respiratory sounds is limited. An early differential diagnosis (at first presentation
of symptoms) based on objective and non-invasive techniques could prevent
misdiagnosis and subsequent under- or overtreatment.
The general aim of this thesis was to investigate the clinical potential of two noninvasive methods for improved diagnosis of noisy breathing subtypes (wheezing
and rattling) in infants between 2 and 18 months old: (i) analysis of exhaled
volatile organic compounds (VOCs), and (ii) acoustic analysis of breath sounds
recorded with a non-contact microphone using machine learning algorithms
(PART I). The method for sampling and analysing exhaled breath was optimized,
i.e. offline selected ion flow tube mass spectrometry (SIFT-MS), an infant adjusted
breath sampler, and influence of breathing route (PART II).
The Non-invasive Noisy breathing Infant (NiNbI) study (PART I), did not include
sufficient wheezing infants to differentiate between wheezing and rattling.
Therefore, we adjusted the objective and focused on differentiating (i) rattling
infants based on severity, and (ii) rattling infants from infants who recovered from
noisy breathing. Rattling infants usually do not respond to the classical therapy
with inhaled airway dilators and/or inhaled steroids and are therefore referred to
the paediatrician when symptoms continue. This indicates that rattling infants are
singled out, so to speak. This could indicate that the rattling infants are a separate entity, who are/were incorrectly classified as wheezing and incorrectly treated as
such.
Differentiating between different rattling severities (mild, moderate, severe) and
recovered infants, and between recovered/mild and moderate/severe rattlers was
possible based on exhaled breath (chapter 1). VOCs in exhaled breath were
measured using offline gas chromatography mass spectrometry (GC-MS) and
SIFT-MS, which showed similar performance in discriminating the different patient
groups. Adding parent reported symptoms in the past three days to the diagnostic
classification model improved the performance, particularly for the discrimination
of mild, moderate and severe rattling.
Acoustic analysis of recorded breath sounds using machine learning algorithms
was able to predict rattle severity scores given by an expert panel of paediatricians
(chapter 2). Such computer-aided rattle detection would ideally be combined
with computer-aided wheeze detection to improve differential diagnostics in the
paediatric clinical practice, where low inter-observer agreement on the evaluation
of breath sounds can be an issue. A larger dataset of recorded breath sounds
(both wheeze and rattle) with more detailed descriptions from a paediatric expert
panel would allow better training and validation of the machine learning algorithm.
Offline SIFT-MS analysis of exhaled breath samples was achieved by an indirect
combination with thermal desorption (TD). This was to be a suitable offline method
for the selected common breath volatiles with good agreement to the real-time
method (chapter 3). Further optimization of the methodology could increase
accuracy of quantification. A commercially available breath sampler was adjusted
by adding a Teflon spacer allowing the use of well-fitting face masks for infants
(chapter 4). This adjusted breath sampler provided similar results to its original
both in dynamic and continuous sampling mode for the selected VOCs (acetone,
isoprene, acetaldehyde, pentane, propanal, and propanol). The face mask allowed
the infants to breathe through both their mouth and nose. Both breathing routes showed relatively similar patterns and were strongly correlated in adults using
real-time SIFT-MS analysis (chapter 5).
The NiNbI study provided pilot data and first results on the clinical potential of
exhaled volatile biomarkers and acoustic analysis of breath sounds for
differentiating (i) rattling infants based on severity, and (ii) rattling infants from
infants who recovered from noisy breathing. A clinical study with a larger sample
size of rattling and especially wheezing infants is needed in order to investigate
the clinical potential of both methods for the differentiation of noisy breathing

Real-time selected ion flow tube mass spectrometry to assess short- and long-term variability in oral and nasal breath
Breath-based non-invasive diagnostics have the potential to provide valuable information about a person's health status. However, they are not yet widely used in clinical practice due to multiple factors causing variability and the lack of standardized procedures. This study focuses on the comparison of oral and nasal breathing, and on the variability of volatile metabolites over the short and long term. Selected ion flow tube mass spectrometry (SIFT-MS) was used for online analysis of selected volatile metabolites in oral and nasal breath of 10 healthy individuals five times in one day (short-term) and six times spread over three weeks (long-term), resulting in nearly 100 breath samplings. Intra-class correlation coefficients (ICCs) were used to assess short- and long-term biological variability. Additionally, the composition of ambient air was analyzed at different samplings. The selected volatiles common in exhaled breath were propanol, 2,3-butanedione, acetaldehyde, acetone, ammonia, dimethyl sulfide, isoprene, pentane, and propanal. Additionally, environmental compounds benzene and styrene were analyzed as well. Volatile metabolite concentrations in ambient air were not correlated with those in exhaled breath and were significantly lower than in breath samples. All volatiles showed significant correlation between oral and nasal breath. Five were significantly higher in oral breath compared to nasal breath, while for acetone, propanal, dimethyl sulfide, and ammonia, concentrations were similar in both matrices. Variability depended on the volatile metabolite. Most physiologically relevant volatiles (acetone, isoprene, propanol, acetaldehyde) showed good to very good biological reproducibility (ICC > 0.61) mainly in oral breath and over a short-term period of one day. Both breathing routes showed relatively similar patterns; however, bigger differences were expected. Therefore, since sampling from the mouth is practically more easy, the latter might be preferred.The authors thank all the volunteers for participating in this study. Real‐time versus thermal desorption selected ion flow tube mass spectrometry for quantification of breath volatiles
Rationale Selected ion flow tube mass spectrometry (SIFT-MS) is versatile, rapidly provides result output and determines a wide range of volatiles, making it suitable for biomedical applications. When direct sampling into the SIFT-MS instrument is impractical, combining thermal desorption (TD) and SIFT-MS might offer a solution as it allows sample storage on sorbent tubes for later analysis. This work compares off-line TD SIFT-MS and real-time SIFT-MS for the quantification of selected breath volatiles. Methods Ten healthy non-smoking individuals provided 60 breath samples per method. For off-line analysis, breath was collected onto sorbent tubes via a breath sampler provided with filtered inspiratory air. After TD, samples were re-collected in Tedlar bags which were then connected to the SIFT-MS instrument. For real-time analysis, breath was sampled directly into the instrument. In both cases the analytical method included a total of 155 product ions, and 14 selected volatiles were quantified. The agreement between the methods was assessed using Pearson correlation coefficients and Bland-Altman plots. Results Overall, correlations between real-time and off-line analysis were moderate to very strong (r = 0.43-0.92) depending on the volatile of interest, except for 2,3-butanedione and styrene. The difference between real-time and off-line measured breath concentrations (average bias) ranged between -14.57 and 20.48 ppbv. For acetone and isoprene, it was 251.53 and 31.9 ppbv, respectively. Conclusions Real-time SIFT-MS and off-line TD SIFT-MS for quantification of selected breath volatiles did not show optimal agreement. Analyzing a multitude of analytes in breath via direct exhalation into a SIFT-MS instrument for real-time analysis is challenging. On the other hand, off-line analysis using a breath collection device also has its issues such as possible sample losses due to selective absorption depending on the sorbent used or during desorption and transfer to the instrument. Despite these drawbacks, both methods were moderately well correlated.The authors thank all the volunteers for participating in this study. Diagnostic characterization of respiratory allergies by means of a multiplex immunoassay
Allergic sensitization is commonly assessed in patients by performing the skin prick test (SPT) or determining specific immunoglobulin (IgE) levels in blood samples with the ImmunoCAP™ assay, which measures each allergen and sample separately. This paper explores the possibility to investigate respiratory allergies with a high throughput method, the Meso Scale Discovery (MSD) multiplex immunoassay, measuring IgE levels in low volumes of blood. The MSD multiplex immunoassay, developed and optimized with standards and allergens from Radim Diagnostics, was validated against the SPT and the ImmunoCAP assay. For 18 adults (15 respiratory allergy patients and three controls), blood collection and the SPT were performed within the same hour. Pearson correlations and Bland–Altman analysis showed high comparability of the MSD multiplex immunoassay with the SPT and the ImmunoCAP assay, except for house dust mite. The sensitivity of the MSD multiplexed assay was ≥78% for most allergens compared to the SPT and ImmunoCAP assay. Additionally, the specificity of the MSD multiplex immunoassay was ≥ 87% – the majority showing 100% specificity. Only the rye allergen had a low specificity when compared to the SPT, probably due to cross-reactivity. The reproducibility of the MSD multiplex immunoassay, assessed as intra- and interassay reproducibility and biological variability between different sampling moments, showed significantly high correlations (r = 0·943–1) for all tested subjects (apart from subject 13; r = 0·65–0·99). The MSD multiplex immunoassay is a reliable method to detect specific IgE levels against respiratory allergens in a multiplexed and high-throughput manner, using blood samples as small as from a finger prick.</p
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
