Non-Fluoroscopic Transseptal Catheterization During Electrophysiology Procedures using a Remote Magnetic Navigation System.
Transseptal punctures are commonly performed, and left atrial (LA) access is frequently lost during lengthy, complex electrophysiology (EP) procedures. We describe a new technique for non-fuoroscopic re-crossing the fossa ovalis using a new multielectrode transseptal sheath (TS) and a new remote magnetic catheter navigation system (RMNS) (CGCI System, Magnetecs) that uses 8 rapid external electromagnets for real-time navigation of a magnet-tipped electrode catheter across the initial transseptal puncture site in 5 patients undergoing left-sided ablation procedures. The three-dimensional (3D) position of a 8.5 Fr steerable TS with 5-ring 5-15-15-5-mm spaced distal electrodes (Agilis ES©, St Jude Medical), and site of fossal ovalis crossing were "shadowed landmarks" on a 3D electroanatomic mapping (EAM) system (EnSite/NavXTM, St Jude Medical). The TS-magnetic ablation catheter assembly was pulled-back to the inferior vena cava. EAM landmarks were used with RMNS-guided "manual" and "automated" catheter navigation modalities, until septal crossing was obtained. Transseptal re-crossing was successfully performed in all patients in 6.2±8.1 sec using the "automated" RMNS-guided technique and in 30.4±28.4 sec using the "manual" RMNS-guided technique (p=0.01) without complications. This new RMNS was safely and effectively used to perform non-fuoroscopic transseptal catheterization
Distribución espacial de sphenophorus incurrens gyllenhal en la zona de producción de caña del ingenio emiliano zapata en morelos.
La zona de producción de caña de azúcar del ingenio Emiliano Zapata (iEZ) en Morelos comprende 11,080 ha, en los últimos tres años se ha visto afectada por el picudo barrenador Sphenophorus incurrens Gyllenhal, el cual perfora y forma galerías de 4-5 mm de diámetro en raíz y tallo, ocasionando una disminución en el rendimiento, por lo que se requiere conocer aspectos poblacionales de este insecto en la zona de producción del iEZ para mejorar su control, razón por la cual, este trabajo tuvo el objetivo de estudiar la distribución espacial de S. incurrens en dicha zona, así como el efecto de la temperatura en la poblaciones de larvas, pupas y adultos del insecto. La información para el análisis realizado en este trabajo fue proporcionada por el ingenio y corresponde a la zafra 2013-2014. El patrón de distribución (Ia) y el índice de agregación (V) obtenidos mediante el análisis espacial de índices de distancia, se integraron a un sistema de información geográfica para generar el mapa del patrón de distribución de S. incurrens; el efecto de la temperatura media en las poblaciones de larvas, pupas y adultos se analizó mediante técnicas de regresión. El patrón de distribución espacial de S. incurrens fue agregado y mediante el mapeo de V se identificaron los sitios de agregación en la zona cañera del iEZ. El coeficiente de determinación (R2) de los modelos de regresión generados en el análisis del efecto de la temperatura en las poblaciones de larvas, pupas y adultos mostraron relación con este factor.

Study protocol: differential effects of diet and physical activity based interventions in pregnancy on maternal and fetal outcomes--individual patient data (IPD) meta-analysis and health economic evaluation.

Interventions based on diet and physical activity minimise gestational weight gain with varied effect on clinical outcomes. The effect of interventions on varied groups of women based on body mass index, age, ethnicity, socioeconomic status, parity, and underlying medical conditions is not clear. Our individual patient data (IPD) meta-analysis of randomised trials will assess the differential effect of diet- and physical activity-based interventions on maternal weight gain and pregnancy outcomes in clinically relevant subgroups of women. METHODS/DESIGN: Randomised trials on diet and physical activity in pregnancy will be identified by searching the following databases: MEDLINE, EMBASE, BIOSIS, LILACS, Pascal, Science Citation Index, Cochrane Database of Systematic Reviews, Cochrane Central Register of Controlled Trials, Database of Abstracts of Reviews of Effects, and Health Technology Assessment Database. Primary researchers of the identified trials are invited to join the International Weight Management in Pregnancy Collaborative Network and share their individual patient data. We will reanalyse each study separately and confirm the findings with the original authors. Then, for each intervention type and outcome, we will perform as appropriate either a one-step or a two-step IPD meta-analysis to obtain summary estimates of effects and 95% confidence intervals, for all women combined and for each subgroup of interest. The primary outcomes are gestational weight gain and composite adverse maternal and fetal outcomes. The difference in effects between subgroups will be estimated and between-study heterogeneity suitably quantified and explored. The potential for publication bias and availability bias in the IPD obtained will be investigated. We will conduct a model-based economic evaluation to assess the cost effectiveness of the interventions to manage weight gain in pregnancy and undertake a value of information analysis to inform future research. Resilient photoswitchable metal–organic frameworks for sunlight-induced on-demand photochromism in the solid state
Organic photoswitchable molecules have struggled in solid-state form to fulfill their remarkable potential, in terms of photoswitching performance and long-term stability when compared to their inorganic counterparts. We report the concept of non-electron deficient host's surface with optimal porosity and hydrophobicity, as a priori strategy to design photoefficient organic solid-state photochromic materials with outstanding mechanical robustness. It is realized by the nanoconfinement of photochromes in a host matrix possessing optimal porosity and hydrophobicity. The resulting photochromic nanocomposites can be prepared in multigram scale employing a one-pot reaction under ambient conditions. When exposed to a light stimulus including natural sunlight, the photoswitchable nanocomposite powder changes color promptly and reversibly, in a matter of seconds (5 s and 30 s under UV irradiation and sunlight, respectively) along with excellent photo-fatigue resistance, which are on a par with inorganic photochromes. Exemplars of commercially viable prototypes that are optically clear, comprising smart windows, complex photochromic sculptures, and self-erasing rewritable devices, were engineered by direct blending with resilient polymers. Notably, the use of high-stiffness polymers (Young's modulus > 2 GPa) is no longer considered an insurmountable challenge. Finally, photochromic films with anticounterfeiting features could be manufactured through precision printing of nanocrystals by drop-on-demand inkjet printing technology
Zonificación de áreas inundables por máximas avenidas de la quebrada Sarin Mayo en la localidad de Namora – Cajamarca
La presente investigación tiene en objetivos determinar las áreas expuestas a inundarse por
máximas avenidas de la quebrada Sarin Mayo en la localidad de Namora – Cajamarca. El
trabajo se inició con la recabación y sistematización de los datos hidrológicos brindados por
el SENAMHI, así mismo la información cartográfica del entorno de la quebrada se logró a
través de las cartas nacionales y el levantamiento topográfico. Luego se determinaron los
parámetros de la cuenca con ayuda de ArcGis. A continuación, se realizó el modelamiento
hidrológico con apoyo del software Hec Hms, el modelamiento hidráulico con apoyo del
software Iber para cuatro periodos de retorno: 50, 70, 100 y 200 años, con lo que se
generaron los mapas de peligrosidad ante inundaciones. Los resultados del modelamiento
hidrológico fueron los caudales máximos iguales a 27,6; 32,1; 37,3 y 49,1 m3/s;
considerando al caudal de 32,1 m/s el causante de la inundación del 2013 por los vestigios y
testimonios de los pobladores. Del modelamiento hidráulico se obtuvieron los tirantes y
velocidades máximas, para el periodo de retorno de 200 años, el tirante fue de 3,28 metros y
la velocidad de 7,58 m/s. Asimismo, se estimó las áreas de inundación para las máximas
avenidas en inundación muy alta, 7 700 m3, en inundación media, 2 297 m2, en inundación alta, 5 441 m2. Se concluyó que las áreas expuestas a inundarse tienen un mayor nivel de peligrosidad en la margen derecha del cauce, e incluye algunas zonas de la ciudad
