    Metabolinen oireyhtymä ja parodontaali-infektio – tuloksia Pohjois-Suomen syntymäkohortti 1966 -tutkimuksesta

    AbstractThe aim of this cohort study was to investigate whether metabolic syndrome, obesity, and hyperglycaemia are associated with parameters of periodontal disease, deepened periodontal pockets and alveolar bone loss.This study was based on the follow-up studies at ages 31 and 46 of the Northern Finland Birth Cohort 1966 study, including clinical and radiological data on periodontal health at age 46, metabolic data at ages 31 and 46 and data on socioeconomic position and health behaviour factors. Article I included 1618 subjects. The study population of Article II consisted of never-smoking individuals without diabetes mellitus (n = 647). Article III was based on never-smoking individuals without diabetes mellitus (n = 725). The study population of Article IV included individuals without diabetes mellitus, and the analyses were done separately among the total population (n = 1318), among those who had never smoked (n = 725), and among current and former smokers (n = 556). Individuals with rheumatic and inflammatory intestinal and lung diseases were excluded from this study.The results of the study indicate that long-term (≥ 15 years) metabolic syndrome is associated with deepened periodontal pockets and alveolar bone loss. Long-term obesity, weight gain, and central obesity were associated with periodontal pocketing. It was also found that weight gain in males was associated with alveolar bone loss. Impairment in glucose control was found to be associated with periodontal pocketing and alveolar bone loss in those who do not suffer from diabetes mellitus. Artikkeli IV perustui ei-diabeetikoihin ja analyysit tehtiin erikseen koko populaatiossa (n = 1318), tupakoimattomien osajoukossa (n = 725) sekä entisillä ja nykyisillä tupakoitsijoilla (n = 556). Reuma-, tulehdukselliset suolisto- ja keuhkosairaudet poissuljettiin tutkimuksesta.Tutkimustulokset osoittivat pitkäaikaisen (≥ 15 vuotta) metabolisen oireyhtymän olevan yhteydessä syventyneisiin ientaskuihin ja alveoliluukatoon. Pitkäaikainen lihavuus, painon nousu ja keskivartalolihavuus olivat yhteydessä syventyneisiin ientaskuihin sekä vuotaviin syventyneisiin ientaskuihin. Lisäksi painon nousun havaittiin olevan yhteydessä alveoliluukatoon miehillä. Glukoositasapainon heikentymisen löydettiin olevan yhteydessä syventyneisiin ientaskuihin ja alveoliluukatoon.     Riski sairastua parodontiittiin on yksilöllinen. Taudille altistavat puutteellinen suuhygienia, tupakointi ja huonossa hoitotasapainossa oleva diabetes. Diagnosointi edellyttää hammaslääkärin kliinistä ja röntgenologista tutkimusta. Parodontiitin hoidossa on keskeistä hampaan pintojen ja syventyneiden ientaskujen mekaaninen puhdistus sekä tupakasta vieroittaminen. Hoitamaton tauti pahentaa ja ylläpitää elimistön subkliinistä systeemistä tulehdustilaa, joka vaikuttaa moniin yleissairauksiin.Peer reviewe

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
