    Helminth infestation causing anaemia in pregnancy attending primary care clinic: a case report

    Anaemia in pregnancy is among the commonest medical disorders encountered antenatally. However, the condition has sometimes been overlooked and not corrected accordingly given wrong or unidentified exact causes. Helminth infestation, even though rare generally nowadays, is still an important cause that needs to be ruled out in case of anaemia. We reported a case of an antenatal woman who presented with unresolved anaemia. However, she benefited from the helminth infestation screening and was able to be treated well. Her condition tremendously improved after the eradication of the infection. This case highlighted that anaemia should always include stool for ova and cyst despite no convincing history towards the infection. The symptoms could be mild or even not recognised

    Introduction: Antiphospholipid syndrome (APLS) is usually clinically suspected in antenatal women presented with recurrent miscarriages. Screening is usually conducted, especially for those with persistent, unexplained recurrent miscarriages in the first trimester. However, most patients might not seek medical attention earlier, causing a delay in screening. Nevertheless, it is a dilemma for patients with recurrent miscarriages, especially up to four miscarriages, to be presumed as APLS until proven otherwise. Case: We reported a 37-year-old Gravida 5 Para 0+4 at four weeks of amenorrhoea presented with one episode of per vaginal bleeding. APLS and other relevant causative screenings had been performed, and the results were normal. Surprisingly, her pregnancy is still viable and progressing well. Discussion: This case proved that recurrent miscarriages on more than three occasions are not always equivalent to antiphospholipid syndrome. The underlying cause can be idiopathic or other causes that have not been investigated yet, including the possibility of a congenital anomaly. Conclusion: Suspicion of APLS is warranted and should be verified with the gold standard screening tests. In any case, where the results were normal, further assessments such as uterine evaluation, infectious disease screening, and pathological examination should be considered. Pre-pregnancy screening should be a minimal requirement for all women of reproductive age for the evaluation and prevention of similar outcomes to those of other women

    Mild cognitive impairment and its associated factors amongst the older people attending government health clinics in Kuantan

    Introduction: Malaysia is undergoing a demographic transition towards an aging population, resulting in an anticipated rise in the number of older individuals at risk of developing dementia. Mild cognitive impairment (MCI) represents an intermediate clinical stage between normal cognition and dementia, characterized by cognitive decline that does not significantly impair daily functional activities. Early detection of MCI is critical, as early-stage interventions and modifications of risk factors can yield promising outcomes. Materials and Methods: This cross-sectional study evaluated the prevalence of MCI and its associated factors among 327 older adults attending healthcare clinics. Data were collected using the Elderly Cognitive Assessment Questionnaire, Geriatric Depression Scale, and Barthel Index. Binary logistic regression analysis was performed to identify significant factors of MCI. Results: The prevalence of MCI in the study population was 18.7%, with the majority demonstrating poor control of comorbid conditions. Significant factors of MCI included being a widower (OR 0.4; 95%CI: 0.18, 0.94), increasing age (OR 0.92; 95% CI: 0.88, 0.97), and having diabetes (OR 3.49; 95% CI: 1.81, 6.73). Conclusion: The findings highlight that a significant proportion of older adults are at risk of progressing to dementia but remain underdiagnosed during the early stages. Optimizing blood sugar control emerges as a crucial strategy to mitigate the progression of MCI to irreversible dementia. Implementation of active cognitive screening programs is essential for early identification and timely intervention

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
