A novel diagnostic mCRP ALISA assay for the detection of chronic inflammation diseases
An elevated C-reactive protein (CRP) level in plasma has long been used in clinical
practice as a significant risk predictor for inflammatory diseases such as
cardiovascular diseases. CRP is an acute-phase protein that rapidly raises its
concentration from low to undetectable levels to a thousand-fold rise in healthy
individuals following acute inflammation (e.g., infection). CRP exerts its proinflammatory function through its monomeric isoform (mCRP) rather than the wellknown pentameric form (pCRP). Notably, in contrast to pCRP, mCRP is also a potent
activator of endothelial cells and neutrophils. Moreover, mCRP regulates the
complement pathway activation and LDL metabolism in a more efficiently than pCRP.
Based on all observations, it became clear that mCRP exhibits potent proinflammatory activities, whereas pCRP rather possesses anti-inflammatory
properties and/or may serve as a precursor of mCRP. Therefore, we hypothesized
that mCRP is a sensitive indicator for the detection of early inflammation in patients
suffering from chronic inflammation-associated diseases. So far, the current
standard method for CRP detection for clinical purposes has been mostly performed
using commercially available antibodies-based assays such as the highly sensitive
enzyme-linked immunosorbent assay (hsELISA). Some of these antibodies can bind
to both isoforms of CRP (pCRP and mCRP), but the lack of a reliable mCRP specific
assay has limited the validation of this biomarker in different inflammationassociated diseases. In this study, we aimed to develop a diagnostic tool that
specifically detects mCRP, using an aptamer-based approach and to develop an
Aptamer-Linked Immobilized Sorbent Assay (ALISA) as a robust quantification tool
to detect and quantify mCRP in human serum.
The first initial step to achieve the main objective was to characterize and to evaluate
the currently available antibodies against mCRP. In chapter 2, the specificity of the
two non-commercial antibodies 3H12 and 9C9 developed against a mutated,
recombinant mCRP by Prof. L. Potempa, Roosevelt University, Chicago and a
commercially available antibody CRP8 were evaluated. First, these antibodies were
analyzed for their binding affinity, specificity and stability using Surface Plasmon
Resonance. Based on these results, the antibody 3H12 has a higher specificity for
mCRP but turned out to be unstable upon immobilization and regeneration, thereby
gradually losing its binding affinity for mCRP. The CRP8 antibody has a higher affinity for mCRP but does also bind pCRP. As such, the available antibodies are no reliable
candidates to develop a mCRP assay that could specifically detect mCRP in the
presence of pCRP like in human serum.
In chapter 3, the in-house developed aptamers (AmC2, AmC3, AmC4) selected
against native human mCRP by an optimized SELEX procedure were evaluated as an
alternative to the available antibodies. Again, the binding affinity, specificity and
stability were analyzed using Surface Plasmon Resonance and Capillary
Electrophoresis. The kinetic parameters such as association and dissociation
constants were evaluated and compared with the characterized antibody 3H12. The
AmC2 aptamer showed the highest affinity and specificity and was fully stable upon
immobilization and regeneration. Therefore, the selected AmC2 aptamer was used
to design and to develop a robust and reliable diagnostic tool called an AptamerLinked Immobilized Sorbent Assay (ALISA).
In chapter 4, the applicability of the developed ALISA as a diagnostic tool was
explored. As a proof of principle, this novel test was first applied to determine mCRP
levels in the cell culture supernatant derived from inflammation-induced endothelial
cells, being an in vitro vascular endothelial model system for studying inflammation
responses in analogy to chronic inflammatory diseases. Next, as a proof of
application, this test was applied to serum samples of different independent patient
cohorts of inflammation-associated diseases including obese healthy controls
(n=28), Rheumatoid Arthritis (RA) patients (n=30) in comparison to healthy controls
(n=43), Major mood depressive disorder (n= 35) in comparison to healthy control
(n=45) and the chronic obstructive pulmonary disease (COPD) (n=40) with age,
gender-matched healthy controls (n=20). In all these patient cohorts, we observed
significant differences in serum mCRP concentration as compared to control
populations. Overall, this ALISA assay offers a novel, low-cost diagnostic platform
for the reliable detection of mCRP in human serum of patients with systemic
inflammation. Moreover, this assay provides an important tool to monitor these
diseases and may serve to guide therapeutic decisions. In the future, this tool can
also be applied to further explore the function of mCRP and its role in inflammationrelated disorders. In chapter 5, the role of mCRP in extracellular vesicles is explored. The main aim is
to address the question whether mCRP carrying EV can serve as a potential sentinel
marker for early inflammation. In this study, endothelial cells (HUVEC) were
cultivated either unstimulated or triggered for inflammation by TNF-α. EV were
isolated from the supernatant of both conditions using size exclusion
chromatography. Different tools such as immunofluorescence assays, western blot,
TEM and NTA analysis were performed to characterize and to confirm successful
isolation of EV from both conditions. The presence of mCRP on inflamed EV was
investigated using our developed mCRP ALISA, SPR and western blot analysis. We
demonstrated that inflamed EV do carry mCRP and may deliver this to untreated
HUVEC cells triggering a pro-inflammatory status in this recipient cell. Our current
study confirms that the circulating EV have a great potential as a sentinel biomarker
in early inflammation. This study also opens up the opportunity to develop a reliable,
reproducible and robust approach to detect circulating mCRP EV in a diagnostic
application.
In the last chapter 6, the results of the thesis are discussed in relation to the current
international knowledge on mCRP. In addition, future applications of this technology
are discussed.
To conclude, the major aim of this thesis to develop an optimized and robust
diagnostic tool to efficiently quantify mCRP in serum samples is achieved. The
developed ALISA assay was applied in several chronic inflammation-related diseases
and we showed a significantly elevated level of mCRP as compared to healthy
controls, clearly indicating the potential of mCRP as a low-grade systemic chronic
inflammatory biomarker. Therefore, the developed ALISA tool opens new
opportunities to explore some of the as-yet-unknown mCRP activation mechanisms

na een acute ontsteking (bijv. een infectie). CRP oefent zijn pro-inflammatoire
functie uit via zijn monomere isovorm (mCRP) in plaats van de alom gekende

detecteert, ook in een pCRP-achtergrond zoals in serum. Hiervoor wordt een op
aptameren gebaseerde aanpak ingezet omin een Aptamer-Linked Immobilized
Sorbent Assay (ALISA) te ontwikkelen die het mogelijk maakt om op een
betrouwbare manier mCRP in menselijk serum te detecteren en te kwantificeren.
De eerste stap om het hoofddoel te bereiken was het karakteriseren en evalueren
van de momenteel beschikbare antilichamen tegen mCRP. In hoofdstuk 2 werden
twee niet-commerciële antilichamen, ontwikkeld tegen een gemuteerd, recombinant
mCRP door Prof. L. Potempa van de Roosevelt University in Chicago en een
commercieel beschikbaar antilichaam CRP8 geëvalueerd. Eerst werden deze
antilichamen geanalyseerd op hun bindingsaffiniteit, specificiteit en stabiliteit met behulp van Surface Plasmon Resonance. Op basis van deze resultaten konden we
aantonen dat het antilichaam 3H12 een hoge specificiteit heeft voor mCRP, maar
het bleek onstabiel te zijn bij immobilisatie en meer nog bij regeneratie waardoor
het geleidelijk zijn bindingsaffiniteit voor mCRP verliest. Het commerciële CRP8
antilichaam heeft een hogere affiniteit voor mCRP, maar bindt echter ook in hoge
mate aan pCRP. Als zodanig zijn de beschikbare antilichamen (commerciëel of nietcommercieel) geen goede kandidaten om een mCRP-test te ontwikkelen die specifiek
mCRP kan detecteren in de aanwezigheid van pCRP zoals in menselijk serum.
In hoofdstuk 3 werden de in huis ontwikkelde aptameren (AmC2, AmC3, AmC4), die
door ons geselecteerd werden tegen natuurlijk menselijk mCRP via een
geoptimaliseerde SELEX procedure, geëvalueerd als alternatief voor de beschikbare
antilichamen. Opnieuw werd de bindingsaffiniteit, specificiteit en stabiliteit
geanalyseerd met behulp van “Surface Plasmon Resonance” en Capillaire
Electroforese. De kinetische parameters zoals de associatie- en
dissociatieconstanten werden geëvalueerd en vergeleken met het gekarakteriseerde
antilichaam 3H12. Het AmC2 aptamer vertoonde de hoogste affiniteit en specificiteit
en was volledig stabiel zowel bij immobilisatie en regeneratie. Daarom werd het
AmC2 aptamer geselecteerd om een robuust en betrouwbaar diagnostisch
hulpmiddel te ontwerpen en te ontwikkelen, meer specifiek een Aptamer-Linked
Immobilized Sorbent Assay (ALISA).
In hoofdstuk 4 is de toepasbaarheid van de ontwikkelde ALISA als diagnostisch
hulpmiddel onderzocht. Als “proof of principle” werd deze nieuwe test eerst
toegepast om de mCRP-niveaus te bepalen in het supernatant van endotheelcellen
die in celkweek werden gestimuleerd tot een inflammatoire conditie met behulp van
TNF-α. Dit celsysteem betreft tevens een in-vitromodelsysteem voor chronische
(cardio)vasculaire ontstekingsziekten. Vervolgens werd de test, als ”proof of
application”, toegepast op serum van verschillende onafhankelijke patiëntcohorten
met ontstekingsgerelateerde ziekten, waaronder obese gezonde controles (n=28),
reumatoïde artritis (RA) patiënten (n=30) in vergelijking met gezonde controles
(n=43), majeure depressie (n=35) in vergelijking met gezonde controles (n=45) en
chronische obstructieve longziekte (COPD) (n=40) met leeftijd en geslacht
gematchte gezonde controles (n=20). In al deze patiëntencohorten constateerden
we significante verschillen in de serum mCRP-concentraties in vergelijking met de controlepopulaties. In het algemeen biedt deze ALISA-test een nieuw en goedkoop
diagnostisch platform voor de betrouwbare detectie van mCRP in menselijk serum
van patiënten met systemische ontsteking ongeacht de pCRP concentratie in het
aanwezig in het serum. Bovendien biedt deze test een belangrijk instrument om
deze ziekten op te volgen en kan het mogelijk dienen als leidraad voor
therapeutische beslissingen. In de toekomst kan het ook worden toegepast voor de
studie van de functie van mCRP en zijn rol bij het ontstaan van
ontstekingsgerelateerde aandoeningen.
In hoofdstuk 5 werd de rol van mCRP in extracellulaire vesikels (EV) onderzocht. De
belangrijkste onderzoeksvraag hier is of mCRP bevattende EV kunnen dienen als
potentiële sentinel-markers voor een vroege ontsteking. In deze studie werden
endotheelcellen (HUVEC) gekweekt, hetzij zonder stimulans, hetzij getriggerd voor
ontsteking door TNF-α. EV werden uit het supernatant van beide condities geïsoleerd
met behulp van chromatografie. Verschillende analyses zoals immunofluorescentie
assays, western blot, TEM en NTA-analyses werden uitgevoerd om de succesvolle
isolatie van EV te bevestigen en deze EV te karakteriseren. De aanwezigheid van
mCRP op tEV afkomstig van cellen na stimulatie met TNF-α werd aangetoond door
middel van de door ons ontwikkelde mCRP ALISA, SPR en Western blot analyse. We
hebben verder ook aangetoond dat deze tEV mCRP kunnen afleveren aan
onbehandelde HUVEC-cellen en vervolgens een pro-inflammatoire status in deze
recipiënt cellen veroorzaken. Onze huidige studie bevestigt dat deze circulerende EV
een groot potentieel hebben als “schildwacht” bij een vroege ontsteking. Deze studie
opent ook de mogelijkheid om een betrouwbaar, reproduceerbaar en robuust
instrument te ontwikkelen om circulerende mCRP-EV te detecteren en te isoleren
voor verdere diagnostische toepassingen.
In het laatste hoofdstuk 6 worden de resultaten van het proefschrift besproken in
relatie tot de huidige internationale kennis over mCRP. Daarnaast worden
toekomstige toepassingen van deze technologie besproken.
Tot slot werd het belangrijkste doel van dit proefschrift, namelijk het ontwikkelen
van een geoptimaliseerde en robuuste diagnostische assay voor het efficiënt
kwantificeren van mCRP in serum bereikt. De ontwikkelde ALISA werd toegepast bij
verschillende chronische ontstekingsgerelateerde ziekten en we toonden een significant verhoogd niveau van mCRP in vergelijking met gezonde controles, wat
duidelijk wijst op het potentieel van mCRP als een hoogwaardige chronische
inflammatoire biomarker in deze ziekten. Daarom opent de ontwikkelde ALISA-assay
ook nieuwe mogelijkheden om de nog onbekende mCRP-activeringsmechanismen
en de rol ervan in het ontstaan van ontstekingsgerelateerde ziektes verder te

dissociatieconstanten werden geëvalueerd en vergeleken met het gekarakteriseerde
antilichaam 3H12. Het AmC2 aptamer vertoonde de hoogste affiniteit en specificiteit
en was volledig stabiel zowel bij immobilisatie en regeneratie. Daarom werd het
AmC2 aptamer geselecteerd om een robuust en betrouwbaar diagnostisch
hulpmiddel te ontwerpen en te ontwikkelen, meer specifiek een Aptamer-Linked
Immobilized Sorbent Assay (ALISA).
In hoofdstuk 4 is de toepasbaarheid van de ontwikkelde ALISA als diagnostisch
hulpmiddel onderzocht. Als “proof of principle” werd deze nieuwe test eerst
toegepast om de mCRP-niveaus te bepalen in het supernatant van endotheelcellen
die in celkweek werden gestimuleerd tot een inflammatoire conditie met behulp van
TNF-α. Dit celsysteem betreft tevens een in-vitromodelsysteem voor chronische
(cardio)vasculaire ontstekingsziekten. Vervolgens werd de test, als ”proof of
application”, toegepast op serum van verschillende onafhankelijke patiëntcohorten
met ontstekingsgerelateerde ziekten, waaronder obese gezonde controles (n=28),
reumatoïde artritis (RA) patiënten (n=30) in vergelijking met gezonde controles
(n=43), majeure depressie (n=35) in vergelijking met gezonde controles (n=45) en
chronische obstructieve longziekte (COPD) (n=40) met leeftijd en geslacht
gematchte gezonde controles (n=20). In al deze patiëntencohorten constateerden
we significante verschillen in de serum mCRP-concentraties in vergelijking met de controlepopulaties. In het algemeen biedt deze ALISA-test een nieuw en goedkoop
diagnostisch platform voor de betrouwbare detectie van mCRP in menselijk serum
van patiënten met systemische ontsteking ongeacht de pCRP concentratie in het
aanwezig in het serum. Bovendien biedt deze test een belangrijk instrument om
deze ziekten op te volgen en kan het mogelijk dienen als leidraad voor
therapeutische beslissingen. In de toekomst kan het ook worden toegepast voor de
studie van de functie van mCRP en zijn rol bij het ontstaan van
ontstekingsgerelateerde aandoeningen.
In hoofdstuk 5 werd de rol van mCRP in extracellulaire vesikels (EV) onderzocht. De
belangrijkste onderzoeksvraag hier is of mCRP bevattende EV kunnen dienen als
potentiële sentinel-markers voor een vroege ontsteking. In deze studie werden
endotheelcellen (HUVEC) gekweekt, hetzij zonder stimulans, hetzij getriggerd voor
ontsteking door TNF-α. EV werden uit het supernatant van beide condities geïsoleerd
met behulp van chromatografie. Verschillende analyses zoals immunofluorescentie
assays, western blot, TEM en NTA-analyses werden uitgevoerd om de succesvolle
isolatie van EV te bevestigen en deze EV te karakteriseren. De aanwezigheid van
mCRP op tEV afkomstig van cellen na stimulatie met TNF-α werd aangetoond door
middel van de door ons ontwikkelde mCRP ALISA, SPR en Western blot analyse. We
hebben verder ook aangetoond dat deze tEV mCRP kunnen afleveren aan
onbehandelde HUVEC-cellen en vervolgens een pro-inflammatoire status in deze
recipiënt cellen veroorzaken. Onze huidige studie bevestigt dat deze circulerende EV
een groot potentieel hebben als “schildwacht” bij een vroege ontsteking. Deze studie
opent ook de mogelijkheid om een betrouwbaar, reproduceerbaar en robuust
instrument te ontwikkelen om circulerende mCRP-EV te detecteren en te isoleren
voor verdere diagnostische toepassingen.
In het laatste hoofdstuk 6 worden de resultaten van het proefschrift besproken in
relatie tot de huidige internationale kennis over mCRP. Daarnaast worden
toekomstige toepassingen van deze technologie besproken.
Tot slot werd het belangrijkste doel van dit proefschrift, namelijk het ontwikkelen
van een geoptimaliseerde en robuuste diagnostische assay voor het efficiënt
kwantificeren van mCRP in serum bereikt. De ontwikkelde ALISA werd toegepast bij
verschillende chronische ontstekingsgerelateerde ziekten en we toonden een significant verhoogd niveau van mCRP in vergelijking met gezonde controles, wat
duidelijk wijst op het potentieel van mCRP als een hoogwaardige chronische
inflammatoire biomarker in deze ziekten. Daarom opent de ontwikkelde ALISA-assay
ook nieuwe mogelijkheden om de nog onbekende mCRP-activeringsmechanismen
en de rol ervan in het ontstaan van ontstekingsgerelateerde ziektes verder te
onderzoeken
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
