    Changes in intervertebral disc morphology persist 5 mo after 21-day bed rest

    As part of the nutrition- countermeasures (NUC) study in Cologne, Germany in 2010, seven healthy male subjects underwent 21 days of head-down tilt bed rest and returned 153 days later to undergo a second bout of 21-day bed rest. As part of this model, we aimed to examine the recovery of the lumbar intervertebral discs and muscle cross-sectional area (CSA) after bed rest using magnetic resonance imaging and conduct a pilot study on the effects of bed rest in lumbar muscle activation, as measured by signal intensity changes in T2-weighted images after a standardized isometric spinal extension loading task. The changes in intervertebral disc volume, anterior and posterior disc height, and intervertebral length seen after bed rest did not return to prebed-rest values 153 days later. While recovery of muscle CSA occurred after bed rest, increases (P 0.016) in multifidus, psoas, and quadratus lumborum muscle CSA were seen 153 days after bed rest. A trend was seen for greater activation of the erector spinae and multifidus muscles in the standardized loading task after bed rest. Greater reductions of multifidus and psoas CSA muscle and greater increases in multifidus signal intensity with loading were associated with incidence of low back pain in the first 28 days after bed rest (P 0.044). The current study contributes to our understanding of the recovery of the lumbar spine after 21-day bed rest, and the main finding was that a decrease in spinal extensor muscle CSA recovers within 5 mo after bed rest but that changes in the intervertebral discs persist

    Resistive vibration exercise during bed-rest reduces motor control changes in the lumbo-pelvic musculature

    To understand the effects of a resistive vibration exercise (RVE) countermeasure on changes in lumbopelvic muscle motor control during prolonged bed-rest, 20 male subjects took part in the Berlin Bed-Rest Study (in 2003âÂÂ2005) and were randomised to a RVE group or an inactive control group. Surface electromyographic signals recorded from five superficial lumbo-pelvic muscles during a repetitive knee movement task. The task, which required stabilisation of the lumbo-pelvic region, was performed at multiple movement speeds and at multiple time points during and after bed-rest. After excluding effects that could be attributed to increases in subcutaneous fat changes and improvements in movement skill, we found that the RVE intervention ameliorated the generalised increases in activity ratios between movement speeds (p 6 0.012), reductions in lumbo-pelvic extensor and flexor co-contraction (p = 0.058) and increases in root-mean-square electromyographic amplitude (p = 0.001) of the lumbar erector spinae muscles. Effects of RVE on preventing increases in amplitude-modulation (p = 0.23) of the lumbar erector spinae muscles were not significant. Few significant changes in activation-timing were seen. The RVE intervention during bed-rest, with indirect loading of the spine during exercise, was capable of reducing some, but not all, motor control changes in the lumbo-pelvic musculature during and after bed-rest

    Prevention of bone loss during 56 days of strict bed rest by side-alternating resistive vibration exercise

    Bed rest is a recognized model for muscle atrophy and bone loss in space flight and in clinical medicine. We hypothesized that whole body vibration in combination with resistive exercise (RVE) would be an effective countermeasure. Twenty healthy male volunteers underwent horizontal bed rest for 56 days and were randomly assigned either to a group that performed RVE 11 times per week or to a group that underwent bed rest only (Ctrl). Bone mineral content (BMC) was assessed by peripheral quantitative computed tomography (pQCT) in the tibia and the radius and by dual x-ray absorptiometry (DXA) in the hip and lumbar spine at baseline and at regular intervals during bed rest and a 12-month follow-up. RVE appeared to protect muscle size and function, and it also prevented bone loss (p-values between b0.001 and 0.01). Bone losses were largest in the distal tibia epiphysis, where BMC declined from 421.8 mg/mm (SD 51.3) to 406.6 mg/mm (SD 52.7) in Ctrl, but only from 411.1 mg/mm (SD 56.6) to 409.6 mg/mm (SD 66.7) in RVE. Most of the BMC losses were recovered by 12-month follow-up. Analyses showed that the epiphyseal cortex, rather than spongiosa, depicted the most pronounced changes during bed rest and recovery. These results suggest that the combined countermeasure applied in this study is effective to prevent bone losses from the tibia. This underlines the importance of mechanical usage for the maintenance of the human skeleton

    Changes in lower extremity muscle function after 56 days of bed rest

    Preservation of muscle function, known to decline in microgravity and simulation (bed rest), is important for successful spaceflight missions. Hence, there is great interest in developing interventions to prevent musclefunction loss. In this study, 20 males underwent 56 days of bed rest. Ten volunteers were randomized to do resistive vibration exercise (RVE). The other 10 served as controls. RVE consisted of muscle contractions against resistance and concurrent whole-body vibration. Main outcome parameters were maximal isometric plantar-flexion force (IPFF), electromyography (EMG)/force ratio, as well as jumping power and height. Measurements were obtained before and after bed rest, including a morning and evening assessment on the first day of recovery from bed rest. IPFF (-17.1%), jumping peak power (-24.1%), and height (-28.5%) declined (P < 0.05) in the control group. There was a trend to EMG/force ratio decrease (-20%; P < 0.051). RVE preserved IPFF and mitigated the decline of countermovement jump performance (peak power -12.2%; height -14.2%). In both groups, IPFF was reduced between the two measurements of the first day of reambulation. This study indicates that bed rest and countermeasure exercises differentially affect the various functions of skeletal muscle. Moreover, the time course during recovery needs to be considered more thoroughly in future studies, as IPFF declined not only with bed rest but also within the first day of reambulation. RVE was effective in maintaining IPFF but only mitigated the decline in jumping performance. More research is needed to develop countermeasures that maintain muscle strength as well as other muscle functions including power

    Alkalmazásfejlesztés REST alapokon

    Dolgozatom témájaként egy olyan web-alkalmazás elkészítését tűztem ki, amely megfelelően alkalmazkodik a különböző képernyő felbontásokhoz, kellőképpen interaktív, eseményvezérelt, a legmodernebb technológiákat alkalmazza, és eleget tesz a REST architektúra megszorításainak. A TicketRadar fejlesztési folyamatainak bemutatása során nyilvánvalóvá válik, hogy a számos modern technológia segítségével mily módon lehet könnyen és gyorsan webalkalmazásokat fejleszteni. A felhasznált rendszerekről megállapítható, hogy minden egysége egy-egy jól megtervezett munka eredménye. Munkám során rengeteg új szemléletmódot ismertem meg – REST architektúra, MVC minta – és már magabiztosan használom a tárgyalt rendszereket. Symfony 2 és Angular JS keretrendszerek használatakor rájöttem, hogy mennyire egyszerűen megoldhatóak, olyan feladatok melyek korábbi webfejlesztési tapasztalataim során nehézséget okoztak. A REST architektúrának köszönhetően sikerült teljesen szétválasztanom a szerver- és kliens oldali feladatokat, valamint az MVC mintának köszönhetően a felhasználói felületek és a működés szétválasztását is sikerült megoldanom, így egy igazán flexibilis és jól bővíthető alkalmazást hoztam létre. Szakdolgozatomban részletesen, kódokkal szemléltetve is bemutattam, hogyan lehet Symfony 2 segítségével szerveroldali, AngularJS segítségével pedig kliensoldali alkalmazásokat létrehozni, valamint bemutattam e két rendszer integrálását és a további hasznos modul telepítését, használatát, mint pl. a Jade, amely leegyszerűsítette a HTML kódok írását, a Coffee Script, amely megkönnyítette a JavaScript fejlesztést, vagy a Bootstrap, amely segítette egy egyszerű, letisztult és responsive interfész kialakítását. Természetesen az elkészített alkalmazásomhoz még rengeteg fejlesztési ötletem van, mint pl. a regisztráció beiktatása, egy felhasználó kezelés kialakítása, valamint a RESTful kialakítás, de ezidáig is sikerült olyan szintre fejlesztenem, amely már éles környezetben is megállja a helyét. További hiányosság még az adatbázis mérete, ami idővel bővülni fog nem csak az általam feltöltött, hanem a látogatók által beküldött adatokkal is. Úgy gondolom, hogy sikerült egy olyan alkalmazást készítenem, amely nagyban segítheti a TicketRadar felhasználók országszintű tájékozódását a mindennapi rohanó élet során.BSc/BAMérnök Informatiku

    Anatomical sector analysis of load-bearing tibial bone structure during 90 days bed rest and 1- year recovery

    The aim of this study was to investigate whether the bone response to long bed rest–related immobility and during subsequent recovery differed at anatomically different sectors of tibial epiphysis and diaphysis. For this study, peripheral quantitative tomographic (pQCT) scans obtained from a previous 90-day Long Term Bed Rest intervention were preprocessed with a new method based on statistical approach and re-analysed sector-wise. The pQCT was performed on 25 young healthy males twice before the bed rest, after the bed rest and after 1-year follow-up. All men underwent a strict bed rest intervention, and in addition, seven of them received pamidronate treatment and nine did flywheel exercises as countermeasures against disuse-related bone loss. Clearly, 3–9% sector-specific losses in trabecular density were observed at the tibial epiphysis on average. Similarly, cortical density decreased in a sectorspecific way being the largest at the anterior sector of tibial diaphysis. During recovery, the bed rest–induced bone losses were practically restored and no consistent sector-specific modulation was observed in any subgroup. It is concluded that the sector-specific analysis of bone cross-sections has potential to reveal skeletal responses to various interventions that cannot be inferred from the average analysis of the whole bone cross-section. This approach is considered also useful for evaluating the bone responses from the biomechanical point of view
