Real-world management, resource use, patient-reported outcomes and adherence in patients receiving direct oral anticoagulants for first stroke attributed to non-valvular atrial fibrillation in secondary care: A UK mixed-methods observational study.

This is an open access article distributed under the terms of the Creative Commons Attribution License, which permits unrestricted use, distribution, and reproduction in any medium, provided the original author and source are credited.This real-world study investigated the patient-related factors, characteristics, and outcomes of adult patients with non-valvular atrial fibrillation (NVAF) receiving a direct oral anticoagulant (DOAC) for secondary stroke prevention. This was a multi-centre, mixed-methods, non-interventional study conducted in 8 UK secondary care National Health Service centres. The study included adult patients who presented with first ischaemic stroke associated with NVAF without previous anticoagulants. Group 1 included all patients. Group 2 is comprised of prospectively enrolled patients who were initiated on apixaban (n = 49), edoxaban (n = 39) or rivaroxaban (n = 5) post-first stroke from Group 1. The primary objective (Group 1) was to describe patients' demographics, clinical characteristics, and medical history, stratified by the anticoagulant prescribed. The secondary objectives (Group 2) were to describe the patient management pathways, hospital resource use and clinical assessments associated with DOAC treatment, and the patient-reported satisfaction and experience of DOAC treatment. 234 patients were recruited from 8 centres (Group 1). Baseline CHA2DS2-VASc risk scores ranged from 2-7; 70% (157/224) had a score of ≥4. 86% (n = 202/234) of patients presented with stroke at accident and emergency. For Group 2, the median time from stroke to first DOAC dose was 6 (IQR, 2.0-10.2; n = 88) days; 50% patients had ≥ 1 outpatient visit recorded related to AF or DOACs. At 3 and 6 months, 73% (46/63) and 83% (43/52) had high (score of 8) Morisky Medication Adherence Scale score (MMAS-8), respectively. No patients reported being dissatisfied at 3 or 6 months post-DOAC initiation. The study findings demonstrate high levels of adherence, persistence, and treatment satisfaction in the 6 months post-initiation of DOAC after first stroke attributable to NVAF in patients. The presented results provide clinicians with valuable insights into the experience of post-stroke patients with NVAF receiving treatment with a DOAC for secondary prevention of stroke during the 6 months post-stroke

Vinpocetine alleviated alveolar epithelial cells injury in experimental pulmonary fibrosis by targeting PPAR-γ/NLRP3/NF-κB and TGF-β1/Smad2/3 pathways

To view a copy of this licence, visit http://creativecommons.org/licenses/by/4.0/.This study aimed to investigate the potential anti-fibrotic activity of vinpocetine in an experimental model of pulmonary fibrosis by bleomycin and in the MRC-5 cell line. Pulmonary fibrosis was induced in BALB/c mice by oropharyngeal aspiration of a single dose of bleomycin (5 mg/kg). The remaining induced animals received a daily dose of pirfenidone (as a standard anti-fibrotic drug) (300 mg/kg/PO) and vinpocetine (20 mg/kg/PO) on day 7 of the induction till the end of the experiment (day 21). The results of the experiment revealed that vinpocetine managed to alleviate the fibrotic endpoints by statistically improving (P ≤ 0.05) the weight index, histopathological score, reduced expression of fibrotic-related proteins in immune-stained lung sections, as well as fibrotic markers measured in serum samples. It also alleviated tissue levels of oxidative stress and inflammatory and pro-fibrotic mediators significantly elevated in bleomycin-only induced animals (P ≤ 0.05). Vinpocetine managed to express a remarkable attenuating effect in pulmonary fibrosis both in vivo and in vitro either directly by interfering with the classical TGF-β1/Smad2/3 signaling pathway or indirectly by upregulating the expression of Nrf2 enhancing the antioxidant system, activating PPAR-γ and downregulating the NLRP3/NF-κB pathway making it a candidate for further clinical investigation in cases of pulmonary fibrosis

This research is included in qualitative research using descriptive method. The data collection technique of this research is through documentation. This study uses the theory proposed by Rakhmat (2014), Abidin (2013), Keraf (2015), Morrisan (2014). The results of research on the stages of rhetoric in Ustaz Khalid Basalamah's lecture on Youtube found 40 data. (1) In the invention stage, there are 7 data including 5 evidence indicators and 2 statement indicators. (2) The disposition stage contains 15 data, including 7 preliminary data, 4 content data, and 4 closing data. (3) The Elocutio stage contains 7 data, including 3 asindenton data, 1 paradox data, and 3 hyperbole data. (4) The memory stage contains 6 data, including 3 information storage data and 3 experience data. (5) The Pronontitio stage contains 5 data, the data is included in the rhythm. It can be concluded that Ustaz Khalid Basalamah in the invention stage is more dominant in using evidence. In the disposition stage, the prelude is more dominant using greetings, content is more dominant using arguments, and closing is more dominant using expectations. In the elocutio stage, it is more dominant to use asidentone and hyperbole. In the memory stage, it is more dominant to use the storage of information and experiences. In the pronontitio stage, it is more dominant to use rhythm. Thus, Ustaz Khalid Basalamah has used the five stages of rhetoric

Financial Development, Economic Growth, and Poverty Reduction
The frequent failure of financial liberalisation efforts in developing countries, and the serious damage which recent financial crises have imposed on these economies, have led to renewed attempts to understand the relationships between financial sector development, economic growth and poverty reduction, and to provide a more robust intellectual foundation on which to design efficient and pro-poor financial sector policies for developing countries. The paper examines the contribution that financial sector development can make to poverty reduction in developing countries. The linkages between financial and economic growth, and between economic growth and poverty reduction, are considered, and some preliminary empirical evidence is presented on these linkages. The paper goes on to argue that financial market imperfections are a key constraint on pro-poor growth, and that public policy directed at the correction of these financial market failures is needed to ensure that financial development contributes effectively to growth and poverty reduction. The final part of the paper examines in some detail the role of financial regulation and supervision policy as a key area for public intervention directed at enhancing the financial sector’s contribution to poverty reduction.

Pediatric colonoscopy: King Khalid University Hospital Experience
Pediatric colonoscopy is now an established procedure to evaluate colonic disease. As there is no reports about pediatric colonoscopy in our community a retrospective study was carried in KKUH to analyze the pediatric colonoscopies. This is a retrospective study over 15 years period in KKUH to analyze the pediatric colonoscopy. Sixty-six colonoscopies were done in sixty-two patients. The age ranged from 6 month to 16 years; with 35 males and 27 females. Forty-seven of the patients were Saudi. Indication for colonoscopy were bleeding per rectum thirty-two, diarrhea ten, abdominal pain seven, ulcerative colitis in four, suspected polyps in five and other indication in seven patients. In 33 patients colonoscopy was done without premedication, 11 patient had sedation while 22 had general anesthesia. Colonoscopy revealed abnormal finding in 64%, normal findings 28.7% and due to poor preparation no endoscopic report in 7.3%. The commonest abnormal finding were ulcerative colitis in 19 patients and polyps in 17 patients. Pediatric colonoscopy is a safe, practical rewarding procedure especially in certain categories of patients mainly those suspected to have ulcerative colitis, rectal bleeding or diarrhea. A high index of suspicion of ulcerative colitis will lead to better management of patients
