    A pre-post test evaluation of the impact of the PELICAN MDT-TME Development Programme on the working lives of colorectal cancer team members

    Background: the PELICAN Multidisciplinary Team Total Mesorectal Excision (MDT-TME) Development Programme aimed to improve clinical outcomes for rectal cancer by educating colorectal cancer teams in precision surgery and related aspects of multidisciplinary care. The Programme reached almost all colorectal cancer teams across England. We took the opportunity to assess the impact of participating in this novel team-based Development Programme on the working lives of colorectal cancer team members.Methods: the impact of participating in the programme on team members' self-reported job stress, job satisfaction and team performance was assessed in a pre-post course study. 333/568 (59%) team members, from the 75 multidisciplinary teams who attended the final year of the Programme, completed questionnaires pre-course, and 6-8 weeks post-course.Results: across all team members, the main sources of job satisfaction related to working in multidisciplinary teams; whilst feeling overloaded was the main source of job stress. Surgeons and clinical nurse specialists reported higher levels of job satisfaction than team members who do not provide direct patient care, whilst MDT coordinators reported the lowest levels of job satisfaction and job stress. Both job stress and satisfaction decreased after participating in the Programme for all team members. There was a small improvement in team performance.Conclusions: participation in the Development Programme had a mixed impact on the working lives of team members in the immediate aftermath of attending. The decrease in team members' job stress may reflect the improved knowledge and skills conferred by the Programme. The decrease in job satisfaction may be the consequence of being unable to apply these skills immediately in clinical practice because of a lack of required infrastructure and/or equipment. In addition, whilst the Programme raised awareness of the challenges of teamworking, a greater focus on tackling these issues may have improved working lives furthe

Ten Andean natives with a Hct >65% and 10 natives with a Hct 65% showed an increased incidence of CMS compared with subjects with Hct <60%. Spontaneous baroreflex sensitivity was significantly lower in subjects with high Hct compared with the control group. The effects of supplemental oxygen or modification of the breathing pattern on autonomic function were as follows: 1) heart rate decreased significantly after both maneuvers in both groups, and 2) spontaneous baroreflex sensitivity increased significantly in subjects with high Hct and did not differ from subjects with low Hct. Temporary slow-frequency breathing may provide a beneficial effect on the autonomic cardiovascular function in high-altitude natives with CMS

Abstract PURPOSE: To evaluate the role of topical mitomycin C in corneal wound healing (CWH) after photorefractive keratectomy (PRK) in highly myopic eyes. DESIGN: Prospective, double-masked, randomized clinical trial. PARTICIPANTS: Seventy-two eyes of 36 patients affected by high (>7 diopters) myopia. METHODS: In each patient, one eye was randomly assigned to PRK with intraoperative topical 0.02% mitomycin C application, and the fellow eye was treated with a placebo. Postoperatively, mitomycin C-treated eyes received artificial tears (3 times daily, tapered in 3 months), whereas the fellow eye was treated with fluorometholone sodium 2% and artificial tears (3 times daily, tapered in 3 months). MAIN OUTCOME MEASURES: Uncorrected visual acuity (UCVA) and best-corrected visual acuity (BCVA), contrast sensitivity, manifest refraction, and biomicroscopy. Contrast sensitivity was determined using the Pelli-Robson chart. Corneal confocal microscopy documented CWH. RESULTS: Mean follow-up was 18 months (range, 12-36). No side effects or toxic effects were documented. At 12-month follow-up examination, UCVAs (logarithm of the minimum angle of resolution) were 0.4+/-0.48 and 0.5+/-0.53 (P = .03) in mitomycin C-treated eyes and corticosteroid-treated eyes, respectively. At 1 year, corneal haze developed in 20% of corticosteroid-treated eyes, versus 0% of mitomycin C-treated eyes. At 12, 24, and 36 months, corneal confocal microscopy showed activated keratocytes and extracellular matrix significantly more evident in untreated eyes (Ps = 0.004, 0.024, and 0.046, respectively). CONCLUSION: Topical intraoperative application of 0.02% mitomycin C can reduce haze formation in highly myopic eyes undergoing PRK. Comment in Ophthalmology. 2006 Feb;113(2):357; author reply 357-8

    El crecimiento de la construcción de infraestructura demanda cada año una mayor cantidad de recursos naturales, es así que los agregados requeridos para la producción de concreto suman una voluminosa cantidad, por lo que existe la necesidad de reutilizar los desechos sólidos generados de las demoliciones de construcciones antiguas, es por ello que el agregado reciclado grueso son vistas como una opción de remplazo del agregado grueso natural La presente investigación tuvo como objetivo determinar el efecto del comportamiento mecánico del concreto f´c=280kg/cm2 , con la sustitución del agregado reciclado grueso al 10%, 20% y 50% y comparar sus resultados con concreto patrón, para ello la investigación fue de tipo aplicada con enfoque cuantitativo y el diseño de investigación fue experimental. La población y muestra del presente proyecto fueron 36 probetas de concreto, para ello se planteó la hipótesis que habría un efecto favorable en la sustitución de agregado reciclado grueso en el comportamiento mecánico de un concreto f´c=280 kg/cm2 , por lo que se determinó que la resistencia a la compresión a los 28 días de edad del concreto patrón fue de 427.98 kg/cm2 , y reduce a 426.96 kg/cm2 , 370.85 kg/cm2 y a 275.34 kg/cm2 por la sustitución de 10%, 20% y 50% de agredo reciclado grueso respectivamente, de lo que se concluyó que la el efecto del agregado reciclado grueso es perjudicial en la resistencia a la compresión del concreto, sin embargo, la sustitución de 10% no reduce la resistencia en gran magnitud, siendo la sustitución más viable a su uso

We obtain a quasi particle 4f-energy in the weak correlation regime and we discuss the bimodal structure of the f-f propagator in this regime comparing with the Hubbard-type structure in the strong correlation regime
