    Per effetto delle dinamiche demografiche e della crisi delle risorse fiscali disponibili per i sistemi di welfare, accade che s’imponga sempre più alla nostra attenzione la condizione di non autosufficienza, sia quella che riguarda persone adulte con disabilità sia, soprattutto, quella legata all’età anziana. Il volume intende indagare, attraverso un metodo di ricerca originale, il tema del fronteggiamento sociale dei problemi di vita e della maniera in cui esso può essere facilitato dai professionisti dell’aiuto. L’indagine, di sicuro interesse per professionisti del settore e studiosi, mostra il lavoro effettivamente svolto dagli assistenti sociali nelle situazioni di non autosufficienza, con l’obiettivo di mettere in luce le caratteristiche distintive del loro apporto e di identificare le pratiche professionali che possono risultare maggiormente efficaci nell’innestare e sostenere i processi di assistenza

    Gerontological social work is a working area that engages many social workers, however there are few studies that highlight its characteristics. Often it is represented by students and professionals as not very interesting and connected exclusively to the provision of standard services. This paper presents the results of a research carried out in a District of Lombardy, aimed at describing the work of social workers who deal with dependent people and highlighting their strengths in dealing with the situations of people in difficulty. Semi-structured interviews were conducted with the 5 social workers working in the non-self-sufficiency area in the 4 District Municipalities. Social workers were asked to describe some situations that had a positive outcome. 32 situations were described and meaningful perspectives were pointed out, highlighting the complexity of Gerontological social work and the importance of an approach that focuses on care and relationships

    Un questa fase della ricerca ci si è proposti di procedere a una ricostruzione descrittiva dell’esperienza di alcune persone non-autosufficienti e dei loro caregivers, utenti dei servizi socio-assistenziali. Tali esperienze sono state analizzate al fine di identificare i “punti di svolta” e gli elementi più significativi in merito alla qualità della vita, in particolare per quanto attiene al rapporto con i Servizi e gli operatori. Da tale analisi, integrata con quanto emerso nella prima parte dello studio, sono state focalizzate alcune indicazioni prospettiche da tenere in considerazione per un eventuale sviluppo/rafforzamento del servizio sociale

    Good social work practices with disabled adults and elders who live at home: More than the delivery of services

    This paper presents a study about social work practices with disabled adults and elders who live at home. The basic idea behind the research project is that, alongside the indispensable functions linked to the provision of standard delivery, there are further interesting areas of action with which social workers contribute to maintaining or improve the quality of life of dependent people. The research was carried out in a district of Northern Italy. Semi-structured interviews were conducted with the five district social workers assigned to assist people with disability: each one was asked to describe some cases which s/he thought are examples of good practice. 32 case stories were collected, transcribed and processed through a thematic analysis. In a second step, 10 cases were chosen and 10 group interviews were conducted for each one of them, with the participation of the main people involved (for example: the service user, his/her caregiver, other family's members, health and social care professionals). So, various points of view about each case were also collected and analyzed. Results show that "good case" does not mean "simple case". In this context, social workers have to construct a tailored intervention plan, integrating family needs and resources, service provisions, formal and informal support from the local community. The most effective paths are those in which social workers can listen to the point of view of users and carers, acting with respect and sensitivity

    Background and aims. Elevation gradients provide ‘natural experiments’ for investigating plant climate change responses, advantageous for the study of protected species and life forms for which transplantation experiments are illegal or unfeasible, such as chasmophytes with perennial rhizomes pervading rock fissures. Elevational climatic differences impact mountain plant reproductive traits (pollen and seed quality, sexual vs. vegetative investment) and pollinator community composition; we investigated the reproductive ecology of a model chasmophyte, Campanula raineri Perp. (Campanulaceae), throughout its current elevational/climatic range to understand where sub-optimal conditions jeopardise survival. We hypothesised that: 1) reproductive fitness measures are positively correlated with elevation, indicative of the relationship between fitness and climate; 2) C. raineri, like other campanulas, is pollinated mainly by Hymenoptera; 3) potential pollinators shift with elevation. Methods. We measured pollen and seed quality, seed production, the relative investment in sexual vs. vegetative structures and vegetative (Grime’s CSR) strategies at different elevations. Potential pollinators were assessed by combining molecular and morphological identification. Key results. Whereas CSR strategies were not linked to elevation, pollen and seed quality were positively correlated, as was seed production per fruit (Hypothesis 1 is supported). The main pollinators of C. raineri were Apidae, Andrenidae, Halictidae (Hymenoptera) and Syrphidae (Diptera), probably complemented by a range of occasional pollinators and visitors (Hypothesis 2 partially supported). Potential pollinator communities showed a taxonomic shift towards Diptera with elevation (particularly Anthomyiidae and Muscidae) and away from Hymenoptera (Hypothesis 3 was supported). Conclusions. Pollinator availability is maintained at all elevations by taxa replacement. However, reduced pollen quality and seed production at lower elevations suggest an impact of climate change on reproduction (especially <1,200 m a.s.l., where seed germination was limited). Aside from guiding targeted conservation actions for C. raineri, our results highlight problems that may be common to mountain chasmophytes worldwide

    La ricerca che il libro presenta, nata dalla collaborazione tra il Centro di Ricerca Relational Social Work dell’Università Cattolica del Sacro Cuore e il sindacato Federazione Nazionale Pensionati CISL Emilia-Romagna, ha preso l’avvio nel primo periodo dell’emergenza sanitaria, quando si è imposta la necessità di interrogarsi sulle conseguenze sociali di quanto stava accadendo nelle residenze per anziani. Quali effetti aveva sugli ospiti l’interruzione dei contatti con l’esterno? E come hanno fatto gli operatori a non smarrire il senso relazionale del lavoro di cura?     Background: Mortality risk and outcome in critically ill patients can be predicted by scoring systems, such as APACHE and SAPS. The identification of prognostic biomarkers, simple to measure upon admission to an intensive care unit (ICU) is an open issue. The aim of this observational study was to assess the prognostic value of plasma mid-regional pro-adrenomedullin (MR-proADM) at ICU admission in non-selected patients in comparison to Acute Physiology and Chronic Health Evaluation II (APACHEII) and Simplified Acute Physiology Score II (SAPSII) scores. Methods: APACHEII and SAPSII scores were calculated after 24 h from ICU admission. Plasma MR-proADM levels were measured by TRACE-Kryptor on admission (T0) and after 24 h (T24). The primary endpoint was intra-hospital mortality; secondary endpoint was length of stay (LOS). Results: One hundred and twenty-six consecutive nonselected patients admitted to an ICU were enrolled. Plasma MR-proADM levels were correlated with LOS (r = 0.28; p = 0.0014 at T0; r = 0.26; p = 0.005 at T24). Multivariate analysis showed that T0 MR-proADM was a significant predictor of mortality (odds ratio [OR]: 1.27; 95% confidence interval [95%CI]: 1.03-1.55; p = 0.022). Receiver operating characteristic curves analysis revealed that MRproADM on ICU admission identified non-survivors with high accuracy, not inferior to the one of APACHEII and SAPSII scores (area under the curve [AUC]: 0.71; 95%CI: 0.62-0.78; p = 0.0002 for MR-proADM; AUC: 0.71; 95%CI: 0.62-0.79; p < 0.0001 for APACHEII; AUC: 0.8; 95%CI: 0.71-0.87; p < 0.0001 for SAPSII). Conclusions: Our findings point out a role of MR-proADM as a prognostic tool in non-selected patients in ICUs being a reliable predictor of mortality and LOS and support its use on admission to an ICU to help the management of critically ill patients
