    A semiparametric bivariate probit model for joint modeling of outcomes in STEMI patients

    In this work we analyse the relationship among in-hospital mortality and a treatment effectiveness outcome in patients affected by ST-Elevation myocardial infarction. The main idea is to carry out a joint modeling of the two outcomes applying a Semiparametric Bivariate Probit Model to data arising from a clinical registry called STEMI Archive. A realistic quantification of the relationship between outcomes can be problematic for several reasons. First, latent factors associated with hospitals organization can affect the treatment efficacy and/or interact with patient’s condition at admission time. Moreover, they can also directly influence the mortality outcome. Such factors can be hardly measurable. Thus, the use of classical estimation methods will clearly result in inconsistent or biased parameter estimates. Secondly, covariate-outcomes relationships can exhibit nonlinear patterns. Provided that proper statistical methods for model fitting in such framework are available, it is possible to employ a simultaneous estimation approach to account for unobservable confounders. Such a framework can also provide flexible covariate structures and model the whole conditional distribution of the response

    Il volume indaga l’ampio lavoro pittorico e plastico dell’artista comasco Mario Radice (Como, 1898-1987), tra le figure più complesse nel panorama del primo astrattismo italiano e progettista, teorico, critico d’arte, curatore di mostre e saggista, mirando soprattutto a ricostruire ciò che della sua arte è andato perso. La sua prolifica attività non si limita infatti alla produzione pittorica, ma si arricchisce di nuove esperienze progettuali, soprattutto attraverso la collaborazione con gli amici architetti, tra cui Giuseppe Terragni, Cesare Cattaneo e Ico Parisi. Il progetto mette in luce per la prima volta il lavoro sinergico tra Radice e i tre architetti comaschi, espressione delle tre arti scultura, architettura e pittura, documentando le loro opere con immagini fotografiche d’epoca, plastici, scritti, disegni preparatori e studi su carta – tra cui molti inediti – che approfondiscono il rapporto ideativo, progettuale e di collaborazione dei protagonisti

    Multi-criteria comparison among several mitigation strategies for dangerous near Earth objects

    In this paper a comparative assessment of the effectiveness of different deviation methods for Near Earth Objects is presented. Specifically, solar collector, nuclear interceptor, kinetic impactor, low-thrust propulsion, mass driver and gravity tug are modelled and compared. For each method, a mathematical model is developed in order to compute the achievable deviation. A multi-criteria optmization method is then used to construct the set of Pareto optimal solutions, minimizing the mass of the spacecraft at departure from the Earth and the warning time, i.e., the time from launch to the foreseen impact of the asteroid with the Earth, while at the same time maximizing the deviation. A dominance criterion is defined and used to compare all the Pareto sets for all the various mitigation strategies. Finally a Technology Readiness Level factor is associated to each strategy in order to estimate the required technological development

    Optimal low-thrust trajectories to asteroids through an algorithm based on differential dynamic programming

    In this paper an optimisation algorithm based on Differential Dynamic Programming is applied to the design of rendezvous and fly-by trajectories to near Earth objects. Differential dynamic programming is a successive approximation technique that computes a feedback control law in correspondence of a fixed number of decision times. In this way the high dimensional problem characteristic of low-thrust optimisation is reduced into a series of small dimensional problems. The proposed method exploits the stage-wise approach to incorporate an adaptive refinement of the discretisation mesh within the optimisation process. A particular interpolation technique was used to preserve the feedback nature of the control law, thus improving robustness against some approximation errors introduced during the adaptation process. The algorithm implements global variations of the control law, which ensure a further increase in robustness. The results presented show how the proposed approach is capable of fully exploiting the multi-body dynamics of the problem; in fact, in one of the study cases, a fly-by of the Earth is scheduled, which was not included in the first guess solution

    Screening for mutations of the APC gene in 66 Italian familial adenomatous polyposis patients: Evidence for phenotypic differences in cases with and without identified mutation

    Germline mutations in the APC gene are responsible for familial adenomatous polyposis (FAP), a dominantly inherited syndrome characterized by the development of hundreds to thousands of polyps in the colon and in the rectum of affected individuals and by variable extracolonic manifestations (gastric and duodenal polyps, osteomas, retinal lesions, and desmoid tumors). Through the combined use of single strand conformation polymorphism (SSCP) analysis and the protein truncation test (PTT), we have screened 66 Italian FAP patients and found 29 different APC mutations in a total of 34 cases. Of the identified mutations, 15 were nonsense, 12 were 1- to 5-bp deletions or insertions and two were complex rearrangements, all leading to the formation of premature stop codons. Only 10 mutations had been already previously described at the germline level, confirming the high heterogeneity of the APC mutational spectrum. The mean age of diagnosis in mutation positive cases and their affected relatives was significantly lower than in cases without identified mutation (30.6 vs 39.1 years, respectively; p = 0.003). In addition, among patients without a family history of polyposis, all mutation positive cases displayed at least one of the extracolonic manifestations usually associated with FAP, whereas in one-half of the cases without identified mutation, none of these phenotypes was observed. Although a fraction of apparently mutation-negative cases were likely to be due to limitations of the mutation screening strategy our results suggest, in agreement with previous reports, that allelic and/or genetic heterogeneity might be responsible for the phenotypic variability observed in FAP patients.
