    Estacionalidad de los nacimientos en España y relación con factores sociodemográficos

    Antecedente: La estacionalidad de los nacimientos, entendida como un patrón que se repite anualmente según el cual en algunos meses nacen más niños que en otros, existe en todas las regiones en las que el fenómeno ha sido estudiado. Aunque han sido objeto de múltiples estudios, las causas y consecuencias de este fenómeno no están completamente identificados. Las implicaciones para la salud pública de este fenómeno abarcan tanto una adecuada planificación de los servicios de atención reproductiva según la época del año, como la consideración de la conocida relación entre el mes de nacimiento de un individuo con diferentes variables posteriores de salud, como la esperanza de vida o el desarrollo de determinadas enfermedades. Objetivo: Utilizar toda la información —tanto longitudinal como transversal— disponible en la estadística oficial española, junto con técnicas estadísticas apropiadas para grandes conjuntos de datos, para analizar la relación de la estacionalidad de los nacimientos en España con factores sociodemográficos diversos y dar respuesta a preguntas hasta ahora no planteadas: primero, cómo es la estacionalidad de los nacimientos en España desde que hay datos y cómo ha cambiado; segundo, qué factores sociodemográficos explican mejor esta estacionalidad; y tercero, si existe una transmisión intergeneracional de la estacionalidad, es decir, si hay una relación entre el mes de nacimiento de los padres y el de los hijos. Datos: Para llevar a cabo este objetivo, contamos con la información de la Estadística de Nacimientos, publicada por el Instituto Nacional de Estadística. La población considerada son todos los nacimientos ocurridos y registrados mensualmente por este organismo. Esto abarca dos periodos de tiempo: 1863-1870 y de 1900 en adelante. La disponibilidad de variables adicionales incluidas en los microdatos de esta estadística va aumentando conforme la estadística se va modernizando. Metodología: El abordaje metodológico de esta investigación es principalmente cuantitativo. Para el análisis de la estacionalidad desde 1863 hasta 2021 se utiliza el análisis espectral de Fourier, la metodología de series temporales de Box-Jenkins, así como el análisis cosinor. Para el análisis de la relación de la estacionalidad con diversos factores sociodemográficos se utilizan técnicas de imputación de datos faltantes y técnicas alternativas de minería de datos —análisis discriminante, redes neuronales, árboles de decisión y regresiones logísticas—. Para el análisis de la existencia una transmisión intergeneracional de la estacionalidad se utilizan técnicas estadísticas para variables categóricas, como son el análisis de correspondencias simples y múltiples. Resultados: En primer lugar, se ha examinado por primera vez la serie mensual completa de nacimientos en España desde 1863 hasta 1870 y de 1900 hasta 2021. Si bien durante el siglo XIX y XX los nacimientos tendían a acumularse más en la primavera, en el siglo XXI suceden más en los meses de otoño. Además de este lento cambio de patrón estacional, existen algunos eventos epidemiológicos y sociales que desestabilizan transitoriamente esta pauta, siendo los más fuertes la pandemia de Influenza Virus de 1918, el fin de la guerra civil española y la pandemia de Covid-19. En segundo lugar, se ha comprobado que los individuos con diferentes características sociodemográficas presentan distintos patrones de estacionalidad y se han ordenado por primera vez estos factores según su importancia obteniendo que las características sociodemográficas que más explican diferentes patrones estacionales en el periodo 2016-19 en España son aquellas relacionadas con la situación laboral materna: su actividad económica, su nivel de estudios y su cualificación laboral. Esto puede indicar que el cambio de patrón observado entre los siglos XIX, XX y XXI de primavera a otoño, puede estar relacionado con la paulatina incorporación de la mujer al mercado laboral. En tercer y último lugar, se ha respondido a la pregunta de si existe alguna relación entre el mes de nacimiento de los padres y el de los hijos. Los resultados muestran que sí: los individuos tienen más probabilidades de nacer en el mismo mes que sus padres, pero también que sus hermanos inmediatamente mayores. Además, se ha comprobado que los dos progenitores de un niño tienden también a haber nacido en el mismo mes, existiendo por tanto una tendencia de los miembros de una misma familia a nacer en un mismo mes. Conclusiones: La estacionalidad de los nacimientos existe en España desde que hay datos, habiendo cambiado su patrón en las últimas décadas. El mes de nacimiento, lejos de ser una variable aleatoria, es una variable en la que intervienen fenómenos epidemiológicos y sociales: desde pandemias y guerras, hasta la incorporación de la mujer al mercado laboral. El mes de nacimiento es además una cuestión familiar: las parejas se conforman con mayor probabilidad de personas que han nacido en la misma época del año y tienden a tener hijos en esas mismas épocas. Esto obliga a tener en cuenta las características familiares cuando se pretende explicar el efecto del mes de nacimiento en otras variables posteriores

    Childbirth is the first reason for hospitalization in Spain. After giving birth, it is not easy to get out unscathed from the hospital, in the sense of not having gone through a surgery (cesarean), a cut in the vagina (episiotomy), or other invasive procedures. The aim of this article is to question how scientific evidence is integrated in medical decisions about the delivery. For this, the article describes and analyzes the existing variability of perinatal care in Spain, which shows that the result of the delivery depends much more on factors such as the geography zone or whether the hospital is public or private, rather than the delivery process of women itself. It also analyzes the adequacy of interventions to the standards set by organizations such as the World Health Organization and the Ministry of Health.El parto es el motivo número uno de ingreso hospitalario en España. Después de parir, no es fácil salir ilesa del hospital, en el sentido de no haber sido sometida a una operación quirúrgica (cesárea), a un corte en la vagina (episiotomía), o a otras intervenciones invasivas. El objetivo de este artículo es cuestionar el modo en que la evidencia científica se integra en las decisiones médicas en torno al parto. Para ello, el artículo describe y analiza la variabilidad de la atención perinatal existente en España, que demuestra que el resultado de un parto depende mucho más de factores como la zona geográfica o de si el centro hospitalario es público o privado, que del propio proceso de parto de la mujer. Además analiza la adecuación de las intervenciones a los estándares fijados por organismos como la Organización Mundial de la Salud o el Ministerio de Sanidad

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    An Impact Evaluation of the Strategy for Normal Birth Care on Caesarean Section Rates and Perinatal Mortality in Spain

    The objectives of this research are to evaluate the impact of a health policy (the Strategy for Normal Birth Care, EAPN) on caesarean rates and perinatal mortality in Spanish public hospitals belonging to the National Health System (NHS) and to assess the related cost savings. Data from the Spanish Ministry of Health for the period 2002 −2011 and quantitative impact evaluation techniques (double difference method) are used to compare the effects of this policy in a treatment group composed of the NHS hos- pitals and a control group made up of private for-profit hospitals outside the scope of the EAPN. Both groups are compared some years before and after the health policy initiated in 2006 and approved in October 2007. The estimation results show that the EAPN had a significant effect in reducing caesarean rates of approximately 2 percentage points between 2007 and 2011, with increasing cost savings over the years ranging from 24 to 44 million euros depending on the year. Furthermore, EAPN reduced perinatal mortality levels by 0.08% in years 20 08 −20 09

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
