    Dysfonction myocardique du sepsis : de la physiopathologie au traitement

    SUMMARY: Myocardial dysfunction in sepsis: from pathophysiology to treatmentObjectives: To explore the pathophysiology and treatment of septic myocardial dysfunction (SMD) during human septic shock.Methods: We explored the following potential mechanisms during SMD: involvement of myocardial necrosis or inflammation explored by cardiac magnetic resonance imaging (MRI); the role of loading conditions explored by advanced echocardiography; the implication of relative adrenal insufficiency; the role of telomere shortening and that of sepsis mediators explored using a multi-analyte biological approach. We also conducted a survey of French-speaking intensivists on the management of SMD. Finally, we explored the efficacy and safety of dobutamine by advanced echocardiography in a multicenter observational study.Results: The survey of intensivists revealed a non-consensual definition of SMD, a preferential use of echocardiography as a tool for monitoring septic shock and dobutamine as the inotrope of choice to treat SMD, but with heterogeneous practices in its initiation or termination. Myocardial MRI of patients with SMD showed no inflammation or necrosis. We demonstrated an inverse relationship between afterload parameters and left ventricular contractility indices. The speckle tracking-derived strain was an early indicator of secondary SMD. The hierarchical classification of 20 sepsis mediators identified three biologically consistent clusters. The cluster linked to the innate inflammatory response (e.g., IL6, TNF-α, IL1-β, sST2) was associated with SMD and organ failure, while that linked to adaptive immunity (IL17, IFN) was associated with sepsis resolution and survival. We also demonstrated the association of relative adrenal insufficiency with SMD. Leucocyte telomere length was not associated with SMD.     Hémoptysie sur cancer pulmonaire dans une unité de soins intensifs

    Dans l hémoptysie grave secondaire au cancer pulmonaire, les données sur la prise en charge, l efficacité des traitements notamment l artériographie bronchique avec embolisation et l évolution sont rares. Nous avons conduit une étude rétrospective sur 102 patients admis pour une hémoptysie grave secondaire à un cancer pulmonaire hospitalisés entre mai 1995 et février 2008, dans une unité de soins intensifs. Ces patients, avec un âge médian de 63 ans (+-19) et un PS= 2, le caractère étendu du cancer (stade IIIB, IV ou cancer à petites cellules disséminées) et le recours à la ventilation mécanique. Les facteurs pronostics de mortalité à un an, sont l usage abusif d alcool, le performans status >= 2, un volume émis à l admission >= 300ml et un cancer en progression. Les patients, présentant une hémoptysie secondaire à un cancer pulmonaire dont l état général est satisfaisant, ont un pronostic justifiant une prise en charge multidisciplinaire en unité de soins intensifs compte tenu de l efficacité des thérapeutiques proposées et de la survie à un an. L artério-embolisation peut être proposé comme traitement de première intention.In patients admitted to the intensive care unit (ICU) for severe haemoptysis related with lung cancer, there are limited data on management, effectiveness of arterial embolization and patients'outcome. We conducted a retrospective study of 102 cases of severe haemoptysis due to bronchial carcinoma (non-small cell lung cancer 92%) admitted to he ICU between may 1995 and february 2008. Median age was 63 (53-71) years, performance status (PS) was 2, mechanical ventilation, and an extensive disease (stage IIIB, IV or extensive small cell cancer) .Also, alcohol abuseand a cumulated volume of haemoptysis at admission were associated with one year mortality in the multivariate model. In conclusion, our results suggests that a multidisciplinary approach should be adopted for the management of patients with hemoptysis as a complication of their lung cancer and arterial embolization should be proposed as first line therapy in such patients.ST QUENTIN EN YVELINES-BU (782972101) / SudocSudocFranceF

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
