    Le volvulus gastrique est une torsion anormale de l’estomac autour de l’un au moins de ses axes occasionnant une gène à sa vidange mais aussi à sa vascularisation. Ainsi on distingue trois types anatomiques de volvulus gastrique : organo-axial, mésentérico-axial et mixte. C’est une pathologie rare mais certainement sous estimée car souvent méconnue. A travers l’étude de ce cas clinique, nous nous proposons de faire une mise au point avec revue de la bibliographie sur le volvulus gastrique révélé par une hernie diaphragmatique chez un homme de 60 ans. Les manifestations cliniques sont peu spécifiques, associant des douleurs épigastriques, vomissements noirâtres avec une impossibilité de poser une sonde nasogastrique. La radiographie standard du thorax permet de suspecter le diagnostic devant une anomalie de la position de l’estomac et la présence d’un niveau hydro-aérique en intra-thoracique. Le TOGD permet un diagnostic précis d’un volvulus mésentérico-axial de l’estomac confirmant son importance dans cette pathologie. La hernie diaphragmatique nous a paru être le facteur étiologique essentiel. Le traitement consiste en une dévolvulation de l’estomac et une réduction dans l’abdomen de l’estomac, la rate et le colon transverse ainsi qu’une fermeture de la brèche diaphragmatique avec fixation de la grosse tubérosité à la partie antérieure du diaphragme. Le traitement ne peut être que chirurgical que ce soit par laparotomie ou par laparoscopie. Il doit être précoce avant l’installation de la nécrose qui est grevé d’une mortalité atteignant les 50 %. Hormis cette situation, le pronostic reste favorable

    Métastases cutanées d’un carcinome vulvaire

    Les métastases cutanées (MC) sont rares. Elles représentent généralement un stade évolutif avancé de la maladie cancéreuse. BF, âgée de 64 ans, a été suivie pour un carcinome épidermoïde vulvaire traité par vulvectomie totale, curage ganglionnaire bilatéral, radiothérapie externe et chimiothérapie. 6 ans après le traitement, la patiente s'est présentée avec de multiples nodules rouges, fermes, au niveau de l'abdomen et des membres inférieurs. La biopsie des lésions cutanées a été en faveur d'un carcinome épidermoïde. L'origine des lésions cutanées a été attribuée à la néoplasie vulvaire. Le traitement a consisté en une chimiothérapie type CHOP. La patiente est décédée dans un tableau de métastases cérébrales 9 mois après le diagnostic. A la différence des métastases viscérales, lymphonodales ou osseuses, Les MC sont peu fréquentes (0,7 et 9% de toutes les métastases) et leur identification présage d'un mauvais pronostic. Elles peuvent être précoces et parfois révélatrices, concomitantes ou tardives. Elles se présentent le plus souvent comme des nodules cutanés, parfois comme des lésions inflammatoires, cicatricielles ou bulleuses et rarement comme un granulome annulaire ou un ulcère. Les MC peuvent soit reproduire l'aspect histologique de la tumeur primitive, soit former d'autres contingents, parfois plus indifférenciés. Chez la femme, le cancer de sein représente l'étiologie la plus fréquente, suivi du colon, du mélanome, des ovaires et des poumons. Quant au cancer vulvaire, il est rarement métastatique. Les MC sont de mauvais pronostic, près de la moitié des malades décèdent dans les 6 mois suivant le diagnostic.Pan African Medical Journal 2015; 2

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Rhomboid glossitis (RG), first described by Brocq in 1914, is an uncommon benign abnormality of the tongue, most frequently affecting men. His etiology is unknown, although it has been proposed that it may be derived from chronic candidiasis, or that it may be of embryological, inflammatory, or even immunological origin. It appears as a rounded or rhomboid painless plaque with well-defined margin, intense reddish or pinkish in colour due to atrophy or depapillation, and firm to palpation. The diagnosis of rhomboid glossitis is largely based on clinical examination, though histopathological studies of biopsies may be required if clinical appearance does not allow other possible diagnoses to be ruled out. No treatment is required for asymptomatic cases. We report a case of a female patient aged 24 years, consulted for a painless plaque in dorsum of the tongue. Clinical examination indicated an erythematous plaque, located anterior to the lingual "V" in left paramedial position, elongated and slightly fissured, about 2 x 0.5 cm in size. Candida culture tested positive. The lesion was thus diagnosed as RG with presence of C. albicans. Treatment was with fluconazole for 2 weeks at 50 mg per day. A complete disappearance of the lesion was noted. After 12 months follow-up, no recurrence was reported.Pan African Medical Journal 2016; 2

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
