Orthopaedic geriatrics care - Is it effective?: A prospective population-based comparison of outcome in fractured neck of femur
Orthopaedic geriatric care has become widely accepted despite relatively little formal evaluation. In the East Dorset health authority all patients with fractured neck of femur initially share common orthopaedic facilities but only those from one geographical sector have subsequent access to an orthopaedic geriatric unit, patients from the other sector receiving standard care. We have carried out a prospective population-based comparison of the outcome of 155 consecutive incident cases of fractured neck of femur aged over 65 years. On admission to hospital the two populations were similar in respect of age, sex, fracture type, social status, medical and orthopaedic problems, mental status and dependency (Clifton assessment procedure for the elderly). Multiple regression analysis showed that the mean length of stay was 9.5 days shorter in patients from the sector with access to orthopaedic geriatric care (p < 0.05, 95% confidence interval 0.6 to 18.4 days). This reduction was not due to any difference between the two groups as regards dependency or the level of support provided on discharge. There was no difference in outcome at 6 months post-operatively in terms of mortality, functional outcome (pain and mobility), change in dependency or social status. We conclude that in the East Dorset health authority this combined approach has resulted in a significant reduction in the length of inpatient stay without any other apparent effect on patient outcome
Relationship between personality type and the ability to be a change agent
Plan BThis descriptive study investigated the relationship between personality types as indicated on the Myer’s Briggs Type Indicator and the ability to be a change agent as measured by the Change Agent Inventory. The Myer’s Briggs Type Indicator (MBTI) Form G is a questionnaire designed to make Carl Jung’s theory of psychological types understandable and useful in everyday life. The author, Katharine Cook Briggs (1875-1968) and her daughter, Isabel Briggs Myers (1897-1980) were disciplined observers of human personality differences. They studied and elaborated the ideas of Jung, and applied them to human interaction. After more than 50 years of research and development, the current MBTI is the most widely used instrument for understanding normal personality differences. The Change Agent Inventory (CAI) provides a comprehensive assessment of the criteria and characteristics necessary to successfully implement key business changes in an organization. It can be used to assess individuals charged with the responsibility to make sure a change happens; and to select primary change agents for major change projects. The CAI was developed by Implementation Management Associates, Inc. of Brighton, Colorado. After these two instruments were taken by members of the College Operating Team (COT), a focus group was conducted with these questions asked of each member: What is your attitude toward change? Do you like or dislike change? Why? How effective are you as a change agent? The participants in this study were nine managers at a two-year technical college. The WITC - New Richmond COT has a variety of personality preferences. All the COT members demonstrated a higher than average capacity for change. There was a tendency for more intuitive managers to score higher on the change agent instrument. There was also a tendency for the higher the score on the change agent instrument the lower the MBTI Type S score. However, there did appear to be a positive relationship between the focus group responses and CAI scores
Energy balance in healthy elderly women
There is a paucity of data on which to base estimates of the energy requirements of the elderly. In general, ageing appears to be associated with a reduction in energy requirement arising from a reduction in physical activity and loss of fat-free mass. The aim of the present study was to measure the total energy expenditure (TEE), basal metabolic rate (BMR), and energy expended on physical activity (calculated as TEE - BMR) in a group of healthy elderly women living in the community in Southampton. Mean rates of TEE (9.21 (sd 1.48) MJ/d) and energy expended on physical activity (412 (sd 1.19) MJ/d) were higher than those observed in some studies of younger adults in the UK, and higher than the factors used to estimate the average energy requirement for the elderly. The results suggest that an age-related reduction in energy requirement is not inevitable and support the hypothesis that the effects of ageing on physical activity, body composition, and hence energy requirements, are variable.</p
Does admission grip strength predict length of stay in hospitalised older patients?
Epidemiological studies have shown that grip strength measured in middle-aged and older people is a powerful predictor of functional decline, disability and mortality [1–3]. Findings from cross-sectional data also suggest that grip strength may be a useful single marker of frailty [4]. However, grip strength is rarely measured in a clinical setting either for objective assessment of muscle function in relation to physical function or to predict outcome. The few clinical studies involving measurement of grip strength have been confined to a surgical setting and showed that lower grip strength was associated with increased post-operative complications [5–10]. Two studies also showed a relationship between pre-operative grip strength and post-operative length of sta
The Human Resource Economics of Vernon Briggs
In the growing search for alternatives to the narrower versions of mainstream economic theory, the author celebrates the work of an earlier institutional economist, Vernon Briggs. Briggs's "human resource economics" draws from a variety of fields and emphasizes the importance of human capital to general prosperity. But Briggs brings a granular analysis to what, in much economic theory, can be a misleading abstraction. His economics is also unabashedly normative-an attempt to tell policymakers how to make the economy work.
