    BACKGROUND AND SCOPE: Recent literature has demonstrated that inflammation contributes to all phases of atherosclerosis and brain damage caused by stroke. In acute phase of cerebrovascular diseases biochemical markers of inflammation, such as C-reactive protein (CRP), could represent an indicator of severity of stroke, but few studies have verified this hypothesis, especially in very old patients. The aim of this study was to evaluate the role of CRP on short- and long-term prognosis in 75-year old and over elderly patients with acute ischaemic stroke. MATERIALS AND METHODS: We retrospectively evaluated CRP values (nephelometric method), performed within 12 h from hospital admission, in 196 elderly patients (124 females and 72 males with mean age+/-SD 83.32+/-10.46 years), discharged with diagnosis of acute ischaemic stroke, 68 of them with atherothrombotic large vessel stroke, 38 with lacunar stroke and 90 with cardioembolic stroke. We studied the relationship between CRP values and short-term prognosis [30-day mortality, length of hospitalization (LOS) and physical disability measured by modified Rankin scale and long-term prognosis (12-month mortality and re-hospitalization)]. RESULTS: Mean values of CRP were significantly higher in patients with cardioembolic stroke compared with atherothrombotic large vessel and lacunar stroke, in patients who died in the first 30 days from the acute event compared with survivors. LOS and physical disability score rose with increasing values of CRP for all subtypes of stroke. Higher CRP values were associated with the 12-month re-hospitalization for cerebrovascular events, whereas it did not influence the 12-month cumulative re-hospitalization and 12-month mortality. CONCLUSIONS: Elevation of CRP values at hospital admission could represent a negative prognostic index in elderly patients with ischaemic stroke, in particular, for short-term prognosis

    Gli angiomi rubino sono neoformazioni benigne, legate ad un fenomeno di proliferazione cutanea vascolare. Nello sviluppo di tali lesioni sembra essere coinvolto HHV8 (Human Herpes Virus 8), capace di indurre neoangiogenesi nelle cellule endoteliali. Un ruolo importante nella suscettibilità a tale infezione, tuttavia, è anche svolto dalla competenza immunitaria del soggetto. La funzione di difesa è mediata dalle cellule natural killer (NK), attraverso un sistema di segnalazione cellulare, modulato da un particolare tipo di recettore, il KIR (killer-cell immunoglobulin-like receptor). Un determinato aplotipo recettoriale potrebbe alterare la risposta immunitaria promossa dalle cellule NK favorendo l’infezione di natura herpetica ed il conseguente sviluppo di angiomi rubino. Col presente studio osservazionale, effettuato su 60 soggetti, si è cercato di mettere in luce un possibile legame tra aplotipo KIR, HHV8 e lesioni angioproliferative

    Patch test reactions through the lens of dermoscopy: Further insights, particularly on weak allergic reactions

    Background: Distinguishing weak allergic reactions from irritant patch test reactions may be difficult. Objectives: To describe the dermoscopic features of allergic reactions (especially weak allergic ones) and irritant patch test reactions, and to assess the suitability of dermoscopy in supporting differential diagnosis. Methods: Eligible participants for this observational, cross-sectional study included consecutive adult outpatients patch tested during a 12-month period, who developed any skin reaction. Healthy volunteers were patch tested with sodium lauryl sulfate as irritant controls. At the 72-hour reading, patch test reactions were recorded both with a digital camera and a digital dermoscopic system. For each reaction, clinical and dermoscopic variables were separately assessed, scored, and then compared. Results: Erythema, vessels, and vesiculation were constant dermoscopic features of allergic reactions (n = 173). In 46 weak (+) allergic reactions, dermoscopy showed (a) erythema (100%), (b) dense polymorphic vessels (100%), and (c) whitish vesicles (78.3%). Scores for vesicles and dotted vessels were significantly higher in weak allergic than in irritant reactions. Vesicles were identified as the chief dermoscopic parameter for correctly distinguishing weak allergic from irritant reactions. Conclusions: Dermoscopy can improve accuracy in the differential diagnosis between weak allergic and irritant patch test reactions

    Eruptive Cherry Angiomas and Skin Melanoma: Further Insights into an Intriguing Association

    Background: Some risk factors for malignant melanoma (MM) are recognized. Objective: To compare the strength of association between MM and eruptive cherry angiomas (CAs) with that of other well-known associations. Methods: This cross-sectional study included all subjects referred to the Outpatient Dermatology-Oncology and Dermoscopy Units of the Universities of Ferrara and Bologna, Italy, over a 5-month period and submitted to total body skin examination. We recorded: age, sex, cutaneous and non-cutaneous malignancies, presence of CAs, arbitrarily considered as "eruptive"when >10, >40 common melanocytic naevi or >2 clinically atypical naevi. The strength of association between the possible risk factors and MM was calculated by odds ratio in both the whole population and age quartiles. Variables associated with MM were included in multiple logistic regression analysis. Results: 1,190 subjects were included; 615 had malignant skin tumours, 462 MM, 85 extracutaneous tumours. Five hundred and eighty-seven subjects had eruptive CAs, 485 subjects >40 melanocytic naevi and 368 more than 2 atypical melanocytic naevi. Eruptive CAs, especially in subjects younger than 70, and >2 atypical melanocytic naevi, mostly in subjects older than 50, were significantly associated with MM. The strength of these 2 associations was similar. The presence of >40 melanocytic naevi was not associated with MM. Conclusions: These findings confirmed an association between MM and eruptive CAs, which was as strong as the one between MM and >2 atypical melanocytic naevi. CAs seem an intriguing model of interaction between heterogeneous variables, like immunocompetence, stimuli inducing endothelial cell proliferation, and oncogenesis, which deserves further investigation

    Endometriosis is a condition characterized by the presence of endometrial tissue in ectopic sites. Cutaneous endometriosis represents less than 1% of all the cases of extra-genital endometriosis; the umbilicus is affected in up to 40% of the cases of cutaneous endometriosis.     Cosmological constraints on slow roll inflation: An update

    In light of the most recent cosmological observations, we provide new updated constraints on the slow roll inflation in different extended scenarios beyond the ΛCDM cosmological model. Along with the usual six parameters, we simultaneously vary different combinations of additional parameters, including the running of the scalar spectral index αs, its running of running βs, the tensor amplitude r and the spatial curvature ωk. From the Planck 2018 data, we find no evidence for a scalar running or a running of running, while analyzing the Atacama Cosmology Telescope data combined with WMAP 9-years observations data we find a preference for nonzero αs and βs at the level of 2.9σ and 2.7σ, respectively. Anyway, this preference is reduced when the tensor amplitude can vary in the model or βs is fixed to zero. The upper bound on r is only slightly affected by the additional parameters while the differences in the datasets can remarkably change the compatibility among the different inflationary models, sometimes leading to discordant conclusions
