    U radu je ispitivan utjecaj beta-ciklodekstrina (beta-CD) na vodotopljivost i oslobađanje celekoksiba iz inkluzijskih kompleksa. Kompleksi su priređeni metodom gnječenja i karakterizirani pomoću SEM, NMR, IR, DSC i difrakcijom rentgenskim zračenjem. Oslobađanje iz kompleksa bilo je značajno bolje nego iz fizičke smjese što ukazuje da je stvaranje inkluzijskog kompleksa povećalo topljivost teško topljivog celekoksiba.In this study, attempts were made to investigate the effects of betha-cyclodextrin (betha-CD) on the aqueous solubility and dissolution rate of celecoxib. Inclusion complexes were prepared by the kneading method and characterized by SEM, NMR, IR, DSC, and X-ray powder diffraction. Dissolution rate of the complexes was significantly greater than that of the corresponding physical mixtures and pure drug, indicating that the formation of inclusion complex increased the solubility of the poorly soluble drug celecoxib

    Two treatment strategies for management of Neurosymptomatic cerebrospinal fluid HIV escape in Pune, India

    Symptomatic cerebrospinal fluid (CSF) viral escape (sCVE) is reported in people with HIV, who are on ritonavir-boosted protease inhibitor (PI/r) containing antiretroviral therapy (ART). Management of sCVE includes performing genotypic HIV-1 resistance testing (GRT) on CSF and plasma HIV and changing ART accordingly. Neither GRT nor newer drugs (Dolutegravir and Darunavir/ritonavir) are routinely available in India. As a result, management of sCVE includes 2 modalities: a) ART intensification by adding drugs that reach therapeutic concentrations in CSF, like Zidovudine, to existing ART or b) Changing to a regimen containing newer boosted PI/r and integrase strand transfer inhibitor (INSTI) as per GRT or expert opinion. In this retrospective study, we report the outcomes of above 2 modalities in treatment of sCVE in Pune, India.Fifty-seven episodes of sCVE in 54 people with HIV taking PI/r-containing ART were identified. Clinical, demographic, laboratory and ART data were recorded. Forty-seven cases had follow-up data available after ART change including measurement of plasma and CSF viral load (VL).Of the 47 cases, 23 received zidovudine intensification (Group A, median VL: plasma- 290, CSF- 5200 copies/mL) and 24 received PI/INSTI intensification (Group B, median VL: plasma- 265, CSF-4750 copies/mL). CSF GRT was performed in 16 participants: 8 had triple class resistance. After ART change, complete resolution of neurologic symptoms occurred in most participants (Group A: 18, Group B: 17). In Group A, follow-up plasma and CSF VL were available for 21 participants, most of whom achieved virologic suppression (VL 200 copies/mL). In Group B, follow-up plasma and CSF VL were available for 23 participants, most of whom also achieved virologic suppression in plasma (21) and CSF (18). Four deaths were noted, 2 of which were in individuals who interrupted ART.This is a unique sCVE cohort that was managed with 1 of 2 approaches based on treatment history and the availability of GRT. At least 75% of participants responded to either approach with virologic suppression and improvement in symptoms

    The Value of Service in Library and Information Science

    This exploratory study examined the value of service, which has been implicitly present in the field for decades and is recognized as one of the pillars of the profession. However it seems that there is not a universal approach to it and it looks like it is taught inconsistently in LIS curriculum in the United States. This research sought to determine how the concept of service was viewed and treated in the Library and Information Science field, and based on the results, to make recommendations to information schools to improve librarians’ education and training. The results reflected an unclear definition of the value of service at all levels (students, professionals and professors) and also suggested that Library and Information Science schools do not have a standard way to teach about this value

    Evaluation of crestal bone levels using simvastatin as an adjuvant around immediate implants – A clinico radiographic study

    Objective: Simvastatin was used as an adjuvant medication in this clinico-radiographic investigation to assess the impact on crestal bone levels around immediate implantation. Materials and Methods: A randomized controlled trial with 50 patients who needed an implant placed right away was done. Simvastatin was used as an adjuvant in groups ((Group A), whereas group (Group B)) served as the control group for the participants. At baseline and during follow-up visits, clinical measures such as probing depth (PD) and bleeding on probing (BOP) were measured. At baseline and 12 weeks, radiographic measurements of crestal bone levels were taken. Results: At 12 weeks, Group A demonstrated a significantly lower PD and BOP than did Group B. Furthermore, at 12 weeks, Group A showed greater crestal bone preservation than did Group B. The results of the statistical analysis showed that the two groups were significantly different. Conclusion: The results of this study indicate that simvastatin use as an adjuvant medication after immediate implant insertion contributes to better clinical outcomes and greater crestal bone preservation. Simvastatin may be helpful in increasing bone regeneration, decreasing inflammation, and soft tissue healing. These findings demonstrate how simvastatin may be used as an additional therapy to enhance the effectiveness of rapid implant implantation operations

    High-Pressure Synthesis of betha and alpha-In2Se3-Like Structures in Ga2S3

    [EN] The synthesis of polymorphs of alpha '-Ga2S3 at room temperature on compression/decompression is studied from a joint experimental and theoretical point of view to reveal the nature of their crystalline structures. The results of Raman scattering and X-ray diffraction measurements on these polymorphs have been analyzed supported by theoretical ab initio simulations. On compression, alpha ' Ga2S3 undergoes a phase transition above 16 GPa to beta '-Ga2S3 with a tetradymite-like (R-3m) structure, isostructural with beta-In2Se3. On decompression, beta '-Ga2S3 undergoes a phase transition below 9.0 GPa to phi-Ga2S3, which is isostructural with alpha-In2Se3 (R3m). Raman signatures of symmetry breaking as well as clear structural differences between the pressure dependence of the unit-cell volume per formula unit, zero-pressure axial compressibilities, bulk modulus, and their first pressure derivative between beta '-Ga2S3 and phi-Ga2S3 have allowed us to determine the R3m nature of phi-Ga2S3. The observation of the R3m phase is also supported by theoretical total energy ab initio simulations. This result unveils a pressure-induced paraelectricferroelectric R-3m-to-R3m transition, like the theoretically predicted temperature-induced transition in several III-VI B2X3 compounds, which could find use in technological applications. Finally, phi-Ga2S3 undergoes a phase transition below 1.0 GPa to gamma-Ga2S3 with a disordered zincblende (F-43m) phase, isostructural with alpha-Ga2Se3 and remains metastable at room conditions.     Objetivo:analizar las principales características epidemiológicas de la mortalidad por infarto del miocardio en Costa Rica de 1970 al 2014. Métodos: los datos del número de muertos por IAM para todas las edades y de la población del país se obtuvieron consultando anuarios y bases de datos del Instituto Nacional de Estadística y del Centro de Población de la Universidad de Costa Rica (UCR). Con ellos se obtuvieron las tasas brutas y estandarizadas, "suavizadas" por promedios móviles y quinquenales, así como la relación porcentual con la mortalidad general. Posteriormente, se calcularon las tasas estandarizadas de mortalidad por IAM para el grupo de 35 a 74 años, por sexo y provincia. Resultados:la tendencia permite distinguir dos épocas definidas. Una de ascenso que ocurre desde 1970 hasta finales de la década de los años noventa. La otra en los primeros años del siglo XXI con inclinación hacia el descenso. Esto se observa con las tasas de promedios móviles y quinquenales, especialmente en el grupo de 35 a 74 años (hombres, r=0.99, r2=094, betha=-6.67, p=0.00, mujeres r=0.97, r2=0.94, betha= -6.67, p=0.01, ambos sexos r=0.97, r2=0.94, betha=-7.42, p=0.01). Conclusiones: pese a una leve tendencia a la disminución de la mortalidad por IAM en estos últimos quince años, el IAM continuará siendo un grave problema de salud en Costa Rica y para revertir la situación, tendrá el gobierno nacional que continuar esforzándose en disminuir la prevalencia de los principales factores de riesgo de las ECNT y en mejorar la atención del paciente que tiene la enfermedad. Objective:To analyze the mortality and some aspects of the descriptive epidemiology of acute myocardial infarction in Costa Rica from 1970 to 2014. Methods:The number of deaths due to AMI for all ages and the population of the country were obtained through the consultation of yearbooks and the databases of the National Institute of Statistics and the Population Center of the UCR. With them gross and standardized rates were extracted, "smoothed" by moving and five-year averages as well as the percentage relation with the general mortality. Subsequently, the standardized mortality rates for AMI were obtained for the group of 35 to 74 years, according to sex, provinces and socioeconomic regions. Results:The analysis of the trend allows to distinguish two very well-defined periods. One of ascent that happens from 1970 until the end of the decade of the nineties. The other appears with the first years of the 21st century with a clear tendency towards descent. This is observed in the case of rates with moving and five-year averages and very especially, with the group of 35 to 74 years (men, r = 0.99, r2= 0.94, betha = -6.67, p = 0.00, women r = 0.97, r2= 0.94, betha = -6.67, p = 0.01, both sexes r = 0.97, r2= 0.94, betha = -7.42, p = 0.01). Conclusions:Despite a slight tendency to decrease mortality from AMI in the last fifteen years, AMI will continue to be a serious health problem in Costa Rica and to reverse the situation, the national government will have to continue striving to reduce prevalence. of the main risk factors of NCDs and in improving the care of patients who have the disease.https://revistas.ucr.ac.cr/index.php/psm/article/view/39926/405311-2
