Эффективность возобновления терапии устекинумабом больных псориазом с метаболическими нарушениями. Клинический опыт
Psoriasis is a chronic immune-mediated inflammatory skin disease with possible damage to the musculoskeletal system, affecting 2-3 % of the adult population of the planet. To date, special attention in the treatment of psoriasis is paid to the comorbid background, since it is known that it can aggravate the course of psoriasis and affect the effectiveness of therapy. The purpose of psoriasis therapy is to achieve clean or almost clean skin, sustainable improvement of psoriatic arthritis, improving the quality of life, all this became possible with the advent of genetically engineered biological drugs. However, in the treatment of genetically engineered biological drugs there are “pain points”: primary inefficiency, “the effect of slipping”, the scheme of transition from one drug to another, the safety of the combined use of GIBP and system-based drugs, interruption of therapy.Objective. To study the therapeutic efficacy of ustekinumab in psoriasis patients with forced interruption of treatment, taking into account metabolic disorders.Materials and methods. 52 patients with a diagnosis of plaque psoriasis receiving ustekinumab for 3 years were under observation, which were divided into 2 groups, depending on body weight and metabolic disorders.Results. More than 80 % of patients taking the drug at a dosage of 45 mg, and at a dosage of 90 mg, to week 12 therapy reached PASI 75. By week 24, the proportion of patients in group I who reached PASI 75 was 96.3 %, in group II — 88.0 %.By week 48 PASI 75 reached 90.4 % patients: in group I — 100 % patients, in group II — 80.0 % patients.At 76 weeks, patients were interrupted ustekinumab treatment in connection with the purchase of the drug. The duration of treatment interruption ranged 36 weeks. By week 112, 86.5 % patients in both groups had a relapse of psoriasis, which was assessed by the loss of the therapeutic response of PASI 75.By week 124 (12 weeks after the resumption of therapy) 82.7 % patients reached PASI 75: in group I — 92.6 % patients, in group II — 72.0 % patients.After 16 weeks (week 128), all patients 100 % achieved PASI 75 IN group I. In group II to week 128 PASI 75 was observed in 80 % patients.Conclusion. With the resumption of treatment of patients with psoriasis with ustekinumab after a long interruption, there is a rapid and complete restoration of therapeutic effectiveness, however, in patients with psoriasis with metabolic disorders, there is a decrease in efficiency after the resumption of therapy in comparison with patients without abdominal obesity, which can determine the further strategy and tactics of management of such patients.Псориаз — хроническое иммуноопосредованное воспалительное заболевание кожи с возможным поражением опорно-двигательного аппарата, регистрирующееся у 2-3 % взрослого населения планеты. На сегодняшний день особое внимание при лечении псориаза уделяется коморбидному фону, поскольку известно, что он может утяжелять течение псориаза и влиять на эффективность терапии. Целью терапии псориаза является достижение чистой или почти чистой кожи, устойчивые улучшения псориатического артрита, улучшение качества жизни — все это стало возможным при появлении генно-инженерных биологических препаратов (ГИБП). Однако в терапии генно-инженерными биологическими препаратами существуют «болевые точки»: первичная неэффективность, «эффект ускользания», схемы перехода с одного препарата на другой, безопасность комбинированного применения ГИБП и системных базисных препаратов, прерывание терапии.Цель исследования. Изучение терапевтической эффективности устекинумаба у больных псориазом при вынужденном прерывании лечения, с учетом метаболических нарушений.Материалы и методы. Под наблюдением находились 52 пациента с диагнозом «бляшечный псориаз», получающих препарат устекинумаб в течение 3 лет, которые были разделены на 2 группы в зависимости от массы тела и метаболических нарушений.Результаты. Более 80 % пациентов, принимавших препарат как в дозировке 45 мг, так и в дозировке 90 мг, к 12 неделе терапии достигли PASI 75. К 24 неделе доля больных в I группе, достигших PASI 75, составила 96,3 %, во II группе — 88,0 %.К 48 неделе PASI 75 достигли 90,4 % пациентов: в I группе •— 100 % больных, во II группе — 80,0 % пациентов.На 76 неделе у пациентов было прервано лечение устекинумабом в связи с проблемами с закупкой препарата. Продолжительность прерывания лечения составила 36 недель. К 112 неделе у 86,5 % пациентов в обеих группах наблюдался рецидив псориаза, который оценивали по потере терапевтического ответа PASI 75.К 124 неделе (через 12 недель после возобновления терапии) 82,7 % пациентов достигли PASI 75: в I группе — 92,6 % больных, во II группе — 72,0 % пациентов.Через 16 недель (128 неделя) PASI 75 в I группе достигли все пациенты (100 %), во II группе PASI 75 отмечался у 80 % пациентов.Вывод. При возобновлении лечения больных псориазом препаратом устекинумаб после длительного прерывания отмечается быстрое и полное восстановление терапевтической эффективности, однако у больных псориазом с метаболическими нарушениями отмечается снижение эффективности после возобновления терапии в сравнении с больными без абдоминального ожирения, что может определить дальнейшую стратегию и тактику ведения таких пациентов
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
