    Hintergrund [:] Global Health (GH) als Studienfach steckt in Deutschland noch in den Kinderschuhen, nur wenige Universitäten bieten entsprechende Studiengänge an. Der Einbezug einer GH-Perspektive ermöglicht eine breitere Betrachtung von Gesundheit und Krankheit, der über einen konventionellen bio-medizinischen krankheitsbezogenen Ansatz hinausgeht. Sie umfasst der Gesundheit vor- und nachgelagerte Faktoren, einschließlich der zahlreichen sozialen, politischen und umweltbedingten Gesundheitsfaktoren, die zu Krankheit führen (Koplan, Bond et al. 2009). Viele Studierende und Angehörige der Gesundheitsberufe, die sich für globale Gesundheit interessieren, engagieren sich auch für unterversorgte einheimische Bevölkerungsgruppen (Haq 2000, Ramsey 2004). Versorgung unterversorgter Bevölkerungsgruppen bedeutet, Wissen, Kompetenzen und Fähigkeiten zu entwickeln, um Gesundheitsbarrieren abzubauen. In Deutschland sind Asylsuchende eine vulnerable Bevölkerungsgruppe, die ein höheres Infektionsrisiko und eine höhere Prävalenz psychischer Probleme aufweisen (Blackmore et al., 2020; Hintermeier et al., 2022; Jahn et al., 2022). Beim Aufbau einer Gesundheitsversorgung für Geflüchtete und Asylsuchende sollte eine Vielzahl von Faktoren berücksichtigt werden. Dazu gehören neben der Beachtung medizinischer und psychischer Diagnosen auch sprachliche und kulturelle Kompetenzen, Kenntnisse über die Herkunftsländer, ein Verständnis der gesellschaftspolitischen Dimensionen sowie kritisches Denken in Bezug auf die Faktoren des Gesundheitssystems. Die gleichen Fähigkeiten sind auch bei der Arbeit im Ausland in einem Umfeld mit begrenzten Ressourcen erforderlich. Akademische Einrichtungen spielen eine besondere Rolle bei der Ausbildung der nächsten Generation von Gesundheitsfachkräften und sind dafür verantwortlich, sie mit den Fähigkeiten auszustatten, die sie benötigen, um im Rahmen der sich entwickelnden Gesundheitsversorgung zu arbeiten (Frenk, Chen et al. 2010). In einer neuen Ära der medizinischen Ausbildung müssen Studierende die Dynamik von Gesundheitssystemen verstehen lernen, um eine effektive Versorgung zu gewährleisten, unabhängig davon, ob es sich um gefährdete Bevölkerungsgruppen im Inland oder in einem Umfeld mit begrenzten Ressourcen handelt. Zielsetzung [:] In dieser Dissertation wird das epidemiologische Krankheitsprofil einer in Deutschland unterversorgten Patientenpopulation - Geflüchtete und Asylsuchende - dargestellt. Durch die Kontextualisierung der breiteren Dimensionen ihrer medizinischen Bedürfnisse wird argumentiert, dass sie einen Mikrokosmos der globalen Gesundheitsprobleme in Deutschland darstellen. Die Versorgung von Asylbewerbern ist daher eine wichtige Facette der Global-Health-Ausbildung, die an deutschen Hochschulen derzeit unterentwickelt ist. Die Rolle, die Universitäten bei der Ausbildung der nächsten Generation von Gesundheitsfachkräften spielen können, wird anhand der Entwicklung einer Nord-Süd-Hochschulpartnerschaft dargestellt, die wesentliche Lücken in der Ausbildung von Gesundheitsfachkräften schließen soll. Als Beispiel für einen solchen Kapazitätsaufbau im Bereich der globalen Gesundheit wird eine medizinische Ausbildungsmaßnahme beschrieben, die sich mit Lücken in der chirurgischen Ausbildung befasst. Material und Methoden [:] Die Arbeit umfasst Inhalte aus einem Buchkapitel und drei veröffentlichten Artikeln. Kapitel 1 (Buchkapitel) bietet einen kontextuellen Rahmen, in dem beschrieben wird, wie die sozialen Determinanten von Gesundheit eine Gemeinsamkeit zwischen gefährdeten Bevölkerungsgruppen auf nationaler als auch internationaler Ebene darstellen. Diese Faktoren, einschließlich der Auswirkungen von Armut und politischer Instabilität auf die Gesundheit, beschreiben eine Gemeinsamkeit in den medizinischen Bedürfnissen gefährdeter Bevölkerungsgruppen. Kapitel 2 (Studie #1): Demografische Daten und medizinische Diagnosen wurden aus den Patientenakten extrahiert, die zwischen dem 15. September und dem 31. Dezember 2015 in der Flüchtlingsambulanz in Dresden, Deutschland, behandelt wurden. Die Daten wurden anonymiziert und mit Stata Version 14.0 analysiert. Kapitel 3 (Studie #2): Es wird eine deskriptive Analyse einer institutionellen Partnerschaft zwischen der Muhimbili University of Health and Allied Sciences (MUHAS) und der University of California San Francisco (UCSF) zur Förderung der medizinischen Ausbildung an der MUHAS vorgestellt. Kapitel 4 (Studie #3): Es wurde ein zweitägiger Schulungskurs für neun verschiedene Notfallverfahren und chirurgische Fertigkeiten entwickelt, der auf dem Lehrplan des Canadian Netzwerk für International Chirurgie basiert. Die Simulationsmodelle für die chirurgischen Fertigkeiten wurden mit lokal verfügbaren Materialien erstellt. Der Lehrplan wurde in einem Pilotversuch mit einer Gruppe von 60 fortgeschrittenen Medizinstudierenden getestet, die ihr chirurgisches Praktikum an der Muhimbili Universität absolviert hatten. Zur Bewertung der chirurgischen Fertigkeiten wurden zwei Instrumente eingesetzt: objektive strukturierte klinische Prüfungen und Befragungen zur Selbsteinschätzung der persönlichen Leistung vor und nach dem Training. Ergebnisse [:] Studie #1: Bei 2.700 Patienten wurden am häufigsten Atemwegserkrankungen diagnostiziert (17,4 %), gefolgt von verschiedenen Symptomen und nicht näher bezeichneten Beschwerden (14,1 %), Infektionen (10,8 %), Erkrankungen des Muskel-Skelett-Systems oder des Bindegewebes (9,3 %), Magen-Darm-Erkrankungen (6,8 %), Verletzungen (5,9 %) und psychischen oder Verhaltensstörungen (5,1 %). Die Asylsuchenden in dieser Studie wiesen keine hohe Belastung durch übertragbare Krankheiten auf, allerdings waren bei einigen von ihnen zusätzliche Untersuchungen und Behandlungen erforderlich, u. a. wegen Tuberkulose und Krätze. Atemwegserkrankungen waren bei neu angekommenen Flüchtlingen häufiger anzutreffen. Traumabedingte psychische Störungen machten die Hälfte der psychischen Diagnosen aus. Studie #2: Zu den Herausforderungen bei der Entwicklung der Hochschulpartnerschaft gehören die Notwendigkeit (1) neuer Finanzierungsmechanismen zur langfristigen Unterstützung institutioneller Partnerschaften und (2) institutioneller Veränderungen an der UCSF und der MUHAS zur Anerkennung und Unterstützung der für die Partnerschaft wichtigen Aktivitäten der Fakultät. Studie #3: An dieser Studie nahmen sechsunddreißig Studierende teil. Vor der Schulung war keine Studierenden in der Lage, einen chirurgischen Verband korrekt anzulegen, nur ein Student war in der Lage, eine Intubation bei Erwachsenen korrekt durchzuführen, und drei Studenten konnten sich korrekt chirurgisch waschen, einkleiden und Handschuhe anziehen. Die Leistungen verbesserten sich nach dem Training, wie die Ergebnisse der objektiven strukturierten klinischen Prüfung zeigten, die von 6/30 auf 15/30 stiegen. Die Studenten sahen einen großen Nutzen im Training praktischer Fertigkeiten. Die Kosten für das Training mit der Low-Tech-Simulation beliefen sich auf vier US-Dollar pro Student. Schlussfolgerung [:] Der Bereich der globalen Gesundheit gewinnt auf Bundesebene und in einigen akademischen Einrichtungen zunehmend an Anerkennung. Die Betreuung medizinisch gefährdeter Bevölkerungsgruppen ist ein Teilbereich von GH und erfordert besondere Fähigkeiten. Ausgewählte Bevölkerungsgruppen, die in Deutschland von gesundheitlicher Ungleichheit betroffen sind, stellen einen Mikrokosmos globaler Gesundheitsprobleme im Land dar und bieten Studierenden die Möglichkeit, Kenntnisse und Fähigkeiten für die Praxis globaler Gesundheit zu entwickeln. While the mention of global health conjures up images of exotic places and diseases, those may represent a very colonialist approach to the field. However, in its essence embracing a global health perspective allows an approach to health and disease in broader terms beyond a conventional bio-medical disease-based approach. It encompasses the upstream and downstream factors of health including the wide range of determinants of health – social, political and environmental - that lead to ill-health (Koplan et al., 2009). Many students and health professionals with an interest in global health are also committed to care for domestic underserved populations (Haq et al., 2000; Ramsey et al.,2004). Caring for underserved populations’ means developing the knowledge, competencies and skills that address barriers to health. One medically vulnerable population in Germany is asylum seekers who have a higher likelihood to develop infection and a higher prevalence of mental health problems (Blackmore et al., 2020; Hintermeier et al., 2022; Jahn et al., 2022). A variety of factors should be considered in building up a primary health care response for refugees and asylum seekers. Beyond attention to medical and mental health diagnoses, these include language and cultural competencies, knowledge of countries of origin, understanding of the socio-political dimensions as well as critical thinking related to health systems factors. These same skill sets are also required when working abroad in settings of limited resources. Academic institutions have a special role to play in training the next generation of health professionals and a responsibility to equip them with the skills to practice within evolving healthcare needs (Frenk et al., 2010). In a new era of medical education, students must be trained to understand the dynamics of primary health systems to deliver effective care, whether they be domestically vulnerable populations or in the context of resource limited settings. Objective[:] In this dissertation, the epidemiological profile of disease in a domestic underserved patient population – namely refugees and asylum seekers - is presented. By contextualizing the broader dimensions of their medical needs in primary care, the thesis makes a case that it represents a microcosm of global health issues in Germany. Care of asylum seekers is thus an important facet of Global Health training and one that is currently in development at some German academic institutions. The role universities can play in training the next generation of health professionals is presented through the development of a North-South university partnership to address essential gaps in the training of health professionals. A medical education intervention addressing gaps in surgical skills training is described as an example of such capacity building in global health. Material and Methods[:] Data is taken from one book chapter and three published articles. Chapter 1 (the book chapter) provides a contextual framework describing how social determinants of health represent a commonality between vulnerable populations both domestically and internationally. These factors, including the effects of poverty and political instability on health, describe a commonality in the medical needs of vulnerable populations. Chapter 2 (1st Study): Demographic and medical diagnosis data were extracted from the electronic medical records of patients seen at the ambulatory refugee clinic in Dresden, Germany between September 15 - December 31, 2015. Data were de-identified and analyzed using Stata version 14.0. Chapter 3 (2nd Study): A descriptive analysis of an institutional partnership between the Muhimbili University of Health and Allied Sciences (MUHAS) and the University of California San Francisco (UCSF) centered on promoting medical education at MUHAS is presented. Chapter 4 (3rd Study): A two-day training course in nine different emergency procedures and surgical skills based on the Canadian Network for International Surgery curriculum was developed. Simulation models for the surgical skills were created with locally available materials. The curriculum was pilot-tested with a cohort of 60 senior medical students who had completed their surgery rotation at Muhimbili University. Two measures were used to evaluate surgical skill performance: Objective Structured Clinical Examinations and surveys of self-perceived performance administered pre- and post-training. Results 1st Study: Two-thousand-seven-hundred and fifty-three individual patients were seen diagnosed most frequently with respiratory diseases (17.4%), followed by miscellaneous symptoms and otherwise not classified ailments (14.1%), infections (10.8%), musculoskeletal or connective tissue disorders (9.3%), gastrointestinal diseases (6.8%), injury (5.9%), and mental or behavioral disorders (5.1%). Asylum seekers in this study did not have a high burden of communicable diseases, however several warranted additional screening and treatment, including for tuberculosis and scabies. Respiratory illnesses were more common amongst newly arrived refugees. Trauma-related mental health disorders comprised half of mental health diagnoses. 2nd Study: The challenges facing the development of the university partnership include the need: (1) for new funding mechanisms to provide long-term support for institutional partnerships, and (2) for institutional change at UCSF and MUHAS to recognize and support faculty activities that are important to the partnership.3rd Study: Thirty-six students participated in the study. Prior to the training, no student was able to correctly perform a surgical hand tie, only one student was able to correctly perform adult intubation and three students were able to correctly scrub, gown and glove. Performance improved after training, demonstrated by Objective Structured Clinical Examination scores that rose from 6/30 to 15/30. Students perceived great benefit from practical skills training. The cost of the training using low-tech simulation was four United States dollars per student.Conclusion. The field of Global Health is gaining increasing recognition at the federal level and in some academic settings in Germany. Care of medically vulnerable populations is a tenant of global health requiring a specific skill set within a primary care approach. Select populations affected by health disparities in Germany represent a microcosm of global health issues in the domestic setting and offer an opportunity for students to learn and develop proficiencies for the practiceof global health. While the mention of global health conjures up images of exotic places and diseases, those may represent a very colonialist approach to the field. However, in its essence embracing a global health perspective allows an approach to health and disease in broader terms beyond a conventional bio-medical disease-based approach. It encompasses the upstream and downstream factors of health including the wide range of determinants of health – social, political and environmental - that lead to ill-health (Koplan et al., 2009). Many students and health professionals with an interest in global health are also committed to care for domestic underserved populations (Haq et al., 2000; Ramsey et al.,2004). Caring for underserved populations’ means developing the knowledge, competencies and skills that address barriers to health. One medically vulnerable population in Germany is asylum seekers who have a higher likelihood to develop infection and a higher prevalence of mental health problems (Blackmore et al., 2020; Hintermeier et al., 2022; Jahn et al., 2022). A variety of factors should be considered in building up a primary health care response for refugees and asylum seekers. Beyond attention to medical and mental health diagnoses, these include language and cultural competencies, knowledge of countries of origin, understanding of the socio-political dimensions as well as critical thinking related to health systems factors. These same skill sets are also required when working abroad in settings of limited resources. Academic institutions have a special role to play in training the next generation of health professionals and a responsibility to equip them with the skills to practice within evolving healthcare needs (Frenk et al., 2010). In a new era of medical education, students must be trained to understand the dynamics of primary health systems to deliver effective care, whether they be domestically vulnerable populations or in the context of resource limited settings. Objective[:] In this dissertation, the epidemiological profile of disease in a domestic underserved patient population – namely refugees and asylum seekers - is presented. By contextualizing the broader dimensions of their medical needs in primary care, the thesis makes a case that it represents a microcosm of global health issues in Germany. Chapter 2 (1st Study): Demographic and medical diagnosis data were extracted from the electronic medical records of patients seen at the ambulatory refugee clinic in Dresden, Germany between September 15 - December 31, 2015. Data were de-identified and analyzed using Stata version 14.0. Chapter 3 (2nd Study): A descriptive analysis of an institutional partnership between the Muhimbili University of Health and Allied Sciences (MUHAS) and the University of California San Francisco (UCSF) centered on promoting medical education at MUHAS is presented. Chapter 4 (3rd Study): A two-day training course in nine different emergency procedures and surgical skills based on the Canadian Network for International Surgery curriculum was developed. Simulation models for the surgical skills were created with locally available materials. The curriculum was pilot-tested with a cohort of 60 senior medical students who had completed their surgery rotation at Muhimbili University. Two measures were used to evaluate surgical skill performance: Objective Structured Clinical Examinations and surveys of self-perceived performance administered pre- and post-training. Results 1st Study: Two-thousand-seven-hundred and fifty-three individual patients were seen diagnosed most frequently with respiratory diseases (17.4%), followed by miscellaneous symptoms and otherwise not classified ailments (14.1%), infections (10.8%), musculoskeletal or connective tissue disorders (9.3%), gastrointestinal diseases (6.8%), injury (5.9%), and mental or behavioral disorders (5.1%). Asylum seekers in this study did not have a high burden of communicable diseases, however several warranted additional screening and treatment, including for tuberculosis and scabies. Respiratory illnesses were more common amongst newly arrived refugees. Trauma-related mental health disorders comprised half of mental health diagnoses. 2nd Study: The challenges facing the development of the university partnership include the need: (1) for new funding mechanisms to provide long-term support for institutional partnerships, and (2) for institutional change at UCSF and MUHAS to recognize and support faculty activities that are important to the partnership.3rd Study: Thirty-six students participated in the study. Prior to the training, no student was able to correctly perform a surgical hand tie, only one student was able to correctly perform adult intubation and three students were able to correctly scrub, gown and glove. Performance improved after training, demonstrated by Objective Structured Clinical Examination scores that rose from 6/30 to 15/30. Students perceived great benefit from practical skills training. The cost of the training using low-tech simulation was four United States dollars per student.Conclusion. The field of Global Health is gaining increasing recognition at the federal level and in some academic settings in Germany.     Hintergrund [:] Global Health (GH) als Studienfach steckt in Deutschland noch in den Kinderschuhen, nur wenige Universitäten bieten entsprechende Studiengänge an. Der Einbezug einer GH-Perspektive ermöglicht eine breitere Betrachtung von Gesundheit und Krankheit, der über einen konventionellen bio-medizinischen krankheitsbezogenen Ansatz hinausgeht. Sie umfasst der Gesundheit vor- und nachgelagerte Faktoren, einschließlich der zahlreichen sozialen, politischen und umweltbedingten Gesundheitsfaktoren, die zu Krankheit führen (Koplan, Bond et al. 2009). Die Asylsuchenden in dieser Studie wiesen keine hohe Belastung durch übertragbare Krankheiten auf, allerdings waren bei einigen von ihnen zusätzliche Untersuchungen und Behandlungen erforderlich, u. a. wegen Tuberkulose und Krätze. Atemwegserkrankungen waren bei neu angekommenen Flüchtlingen häufiger anzutreffen. Traumabedingte psychische Störungen machten die Hälfte der psychischen Diagnosen aus. Studie #2: Zu den Herausforderungen bei der Entwicklung der Hochschulpartnerschaft gehören die Notwendigkeit (1) neuer Finanzierungsmechanismen zur langfristigen Unterstützung institutioneller Partnerschaften und (2) institutioneller Veränderungen an der UCSF und der MUHAS zur Anerkennung und Unterstützung der für die Partnerschaft wichtigen Aktivitäten der Fakultät.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
