    Impakt der Endoskopie in der selektierten Therapie des Hydrocephalus

    Die Anlage eines ventrikuloperitonealen Shunt-Systems ist eine der häufigsten Therapien in der Behandlung des pädiatrischen und adulten Hydrozephalus. Die korrekte Anlage eines Ventrikelkatheters ist ein fundamentaler Faktor, der eine klinische Besserung und die Shunt- Funktionalität kritisch beeinflusst [10, 34]. Unabhängig vom technologischen Fortschritt ist die Fehlerrate der Ventrikelkatheteranlage hoch, sie variiert zwischen 20 und 50 Prozent [1, 20, 28, 29, 33]. Die Okklusion des proximalen Katheterlumens gilt als die wichtigste Ursache für eine Shunt-Dysfunktion [1, 10, 29, 32] und die Position des Ventrikelkatheters beeinflusst die Shunt-Funktionalitäts-Rate [27]. Die klassische freihändige Standardtechnik für Shunting, die auf anatomischen Landmarken basiert, ist eine einfache Technik. In der Praxis zeigte sich aber, dass eine mit bis zu 45 Prozent hohe Fehllagerate des zentralen Katheters festzustellen war [1, 16, 21]. Derzeit stehen verschiedene Verfahren zur Verfügung, um die korrekte Katheteranlage zu optimieren. Neuronavigation, Ultraschall oder Smartphone-gestützte Assistenz wurden bereits in der Hydrozephalus-Therapie eingesetzt und gelten als vielversprechende Methoden zur Erhöhung der Genauigkeit bei der Ventrikelkatheteranlage [9, 14, 19, 21, 32]. Die kritische Nutzung eines Endoskops bei der Behandlung des Hydrozephalus wurde in den letzten Jahren zunehmend in Betracht gezogen. Das intraluminale ShuntScope wurde als wertvolle diagnostische Alternative in der Behandlung des Hydrocephalus eingesetzt [2–4]. Dennoch besteht kein grundlegender Konsens zur Rolle der Endoskopie bei der Anlage eines Ventrikelkatheters. Die Debatte bezüglich der optimalen intraoperativen diagnostischen Methoden wird weiterhin kontrovers geführt. Die vorliegenden Untersuchungsserien illustrieren mehrjährige klinische und neurochirurgische Erfahrungen in der ShuntScope-gestützten Behandlung bei verschiedenen Formen des Hydrocephalus. Die Ätiologie des Erwachsenen-Hydrozephalus weicht signifikant vom pädiatrischen Hydrozephalus ab. Die erhöhte Prädilektion für intraventrikuläre Blutungen und Infektionen sowie für eine erhöhte Shunt-Dysfunktion und eine erhöhte Rezidivrate hinsichtlich der Liquorzirkulationsstörung bei Kindern stellen eine Besonderheit bei der Behandlung der pädiatrischen Hydrozephalus-Fälle dar. Im Gegensatz zu Kindern wurde die Indikation zur intraoperativen Anwendung des ShuntScope bei erwachsenen Hydrozephalus-Patienten selten gestellt. Für die Objektivierung und die Bewertung der intraoperativ erhobenen Bildqualität wurde eine eigene Bildqualität-Skala vorgeschlagen und in Studien verwendet. In den Serien mit pädiatrischem Hydrozephalus wurde in 84,5 Prozent der Fälle ein zufriedenstellendes Bild erreicht, beim Erwachsenen-Hydrozephalus betrug der Wert 87,3 Prozent. In Serien mit pädiatrischem Hydrozephalus wurde kein statistisches Korrelat zwischen der erreichten ShuntScope-bedingten Bildqualität und der korrekten Lage des zentralen Katheters beobachtet (p-Wert 0,290). Dies war jedoch bei adultem Hydrozephalus der Fall (p-Wert < 0,001). Unter speziellen Umständen – bei posthämorrhagisch postinfektiösem Hydrozephalus – kann der Liquor cerebrospinalis eine erhöhte Proteinmenge enthalten, was zu einer verschwommenen Bildqualität führt. Interessanterweise wurde im Rahmen der vorliegenden Untersuchungen trotz solcher Hindernisse sowohl bei Kindern als auch in den Serien mit adulten Betroffenen keine statistische Korrelation zwischen der Bildqualität und dem Typ des Hydrozephalus beobachtet (p-Wert 0,190 für pädiatrische Hydrozephalen und 0,484 für die Erwachsenen-Hydrozephalen). Entgegen der Erwartung wurden trotz der eingeschränkten oder sogar mangelhaften Bildqualität in zwei Fällen die radiologisch verifizierte optimale Ventrikelkatheterposition Grad I und in sechs Fällen Grad II erreicht – ohne dass die Katheter revidiert werden mussten (in der Serie der adulten Hydrozephalen) – und in neun Fällen in der pädiatrischen Serie (sechs Fälle mit Grad I und drei Fälle mit Grad II). In einem Fall wurde der Katheter extraventrikulär unter schlechter Bildqualität implantiert, was am Folgetag revidiert werden musste. Die tiefgreifende intraoperative intraventrikuläre Orientierung bei Ventrikelasymmetrie, z. B. bei der tumorbedingten Obstruktion und der Schlitzventrikel beim Pseudotumor cerebri sind kritisch für die sichere und korrekte Ventrikelkatheteranlage. Die ShuntScope-Assistenz soll vor allem bei schweren und komplexen morphologischen Verhältnissen berücksichtigt werden. Die häufigste Indikation für die Applikation des ShuntScopes war sowohl im erwachsenen als auch im pädiatrischen Hydrozephalus eine tumor- oder zystenbedingte Liquorabflussstörung. Die optimale Anlage des Ventrikelkatheters (Grad I) wurde bei 79,37 Prozent der Verfahren der Erwachsenen- und bei 77 Prozent der Kinderserie erreicht. Basierend auf den dargelegten Ergebnissen kann resümiert werden, dass die ShuntScope-assistierte Technik einen bemerkenswert positiven Einfluss hat und zu einer korrekten Anlage des Ventrikelkatheters beiträgt. Die intraoperative ShuntScope-assistierte intraventrikuläre Exploration konnte in der Mehrheit der analysierten Fälle wichtige Informationen zur intraventrikulären Orientierung und zur endgültigen Positionierung des Katheters beisteuern. Die erfolgreiche Behandlung der Aquäduktstenose stellt durch ihre besonderen Eigenschaften und die vielfältige Ätiologie eine Herausforderung dar. Die endoskopische transaquäduktale Stentimplantation hat sich zu einer wertvollen Alternative für die Behandlung der Aquäduktstenose bei einem isolierten vierten Ventrikel, entwickelt [8, 12, 13, 17, 23, 26]. Die beste Behandlungsoption für andere Formen der Aquäduktstenose, z. B. die periaquäduktale tumorbedingte Aquäduktkompression bei pädiatrischen Patienten, ist nach wie vor umstritten. Mehrere Autoren empfehlen die endoskopische Drittventrikulozisternostomie bei tumorbedingter oder membranöser Aquäduktstenose als Erstlinientherapie vor Shunt oder Stent [13, 22, 26]. Ein einheitlicher Konsens über den besten endoskopischen Ansatz gibt es nicht, und die Debatte über das optimale Therapieparadigma bleibt ungelöst. In der vorliegenden Arbeit wurden unsere Erfahrungen mit dem endoskopischen transaquäduktalen Stentimplantation diskutiert. Die Liquorzirkulationsstörung wurde durch einen Tumor im Aquädukt (2 Fälle), im Thalamus/Mesencephalon (1 Fall) und im dritten Ventrikel (1 Fall), in der Lamina tecti (1 Fall), verursacht. Bei pädiatrischen Patienten sind intraventrikuläre Tumoren häufig eine Ursache für einen obstruktiven Hydrozephalus. Daher sind bei der neurochirurgischen Entscheidungsfindung die beiden wichtigsten therapeutischen Strategien, nämlich die Wiederherstellung der Liquorzirkulation und die Tumorbiopsie, von entscheidender Bedeutung. Die Lokalisation des Tumors, und der Grad der aquäduktalen Kompression sollten präoperativ sorgfältig analysiert werden. Die Planung der Trajektorie für die Tumorbiopsie und die transaquäduktale Stentimplantation werden durch einige der oben genannten Faktoren bestimmt. Tumoren, die sich im distalen Teil des dritten Ventrikels und im periaquäduktalen Bereich befinden, können durch den kontralateralen transventrikulären Zugang erfolgreich erreicht werden, was eine direkte Sicht auf den Tumor ermöglicht. Durch den gleichen Korridor kann der transaquäduktale Stent implantiert werden. Bei der durch den Tumor verzerrten intraventrikulären Anatomie, der Verengung des distalen Teils des dritten Ventrikels und des Aquädukteingangs kann die zusätzliche Verwendung eines intraluminalen ShuntScopes oder eines flexiblen Endoskops aufgrund der geringeren Invasivität im Vergleich zu einem starren Standardendoskop sehr vorteilhaft sein. Die Verwendung eines Gaab-Endoskops mit abgewinkelten Teleskopen (30°, 45°, 70°) ermöglicht eine maximale panoramische Exploration des Aquädukteingangs. Es ist auch wichtig zu betonen, dass die transaquäduktale Stentanlage nur dann durchgeführt werden sollte, wenn die anatomischen Orientierungspunkte klar identifiziert sind. Im Zweifelsfall sollte ein ventrikuloperitonealer Shunt oder eine Drittventrikulozisternostomie in Betracht gezogen werden. Die gleichzeitige Anwendung eines flexiblen Endoskops bietet einige zusätzliche Vorteile. Im Rahmen der vorliegenden Studie wurde der Einsatz eines flexiblen Endoskops in einem Fall in Betracht gezogen. Der Hauptgrund war, die intraaquäduktalen gliotischen Membranen vor der Stentanlage zu perforieren und den vierten Ventrikel zu explorieren. Die Verwendung der kombinierten endoskopischen Technik war vorteilhaft. Es trug dazu bei, die operative Effektivität zu optimieren, indem jeder der inhärenten Vorteile jedes Endoskops genutzt wurde, während gleichzeitig die Einschränkungen jeder endoskopischen Visualisierungstechnik ausgeglichen und überwunden wurden. Abgesehen von der verwendeten Technik sind intraoperative Komplikationen nicht immer vermeidbar. In der vorliegenden Serie lag eine leichte Fornixkontusion vor (1 Fall). Die erfolgreiche transaquäduktale Stentanlage wurde in allen vorgestellten Fällen erreicht. Es wurde keine technikbedingte Morbidität beobachtet. Basierend auf den Ergebnissen wurden die diskutierten Aspekte der vorgestellten Technik beleuchtet. Das soll Anregung sein, diese Technik bei gut ausgewählten Patienten zukünftig in Betracht zu ziehen. Zusammenfassend spielt die endoskopische Visualisierung in der selektierten Therapie des Hydrozephalus eine sehr wichtige Rolle. Die ShuntScope-gestützte Technik ist eine gute diagnostische Alternative, die wichtige Informationen für die endgültige Ventrikelkatheter- oder Stentanlage liefert. Damit kann eine hohe Rate an optimalen Katheteranlagen und deren langfristige Funktionalität erreicht werden

    Image quality and related outcomes of the ShuntScope for catheter implantation in pediatric hydrocephalus : experience of 65 procedures

    Purpose Ventricular catheter implantation in pediatric hydrocephalus can become a highly challenging task due to abnormal anatomical confguration or the need for trans-aqueductal stent placement. Transluminal endoscopy with the ShuntScope has been invented to increase the rate of successful catheter placements. This study aims to evaluate ShuntScope’s image qualities and related surgical outcomes in the pediatric population. Methods A retrospective analysis of all pediatric patients undergoing ventricular catheter placement using the ShuntScope from 01/2012 to 01/2022 in the author’s department was performed. Demographic, clinical, and radiological data were evaluated. The visualization quality of the intraoperative endoscopy was stratifed into the categories of excellent, medium, and poor and compared to the postoperative catheter tip placement. Follow-up evaluation included the surgical revision rate due to proximal catheter occlusion. Results A total of 65 ShuntScope-assisted surgeries have been performed on 51 children. The mean age was 5.1 years. The most common underlying pathology was a tumor- or cyst-related hydrocephalus in 51%. Achieved image quality was excellent in 41.5%, medium in 43%, and poor in 15.5%. Ideal catheter placement was achieved in 77%. There were no intraoperative complications and no technique-related morbidity associated with the ShuntScope. The revision rate due to proximal occlusion was 4.61% during a mean follow-up period of 39.7 years. No statistical correlation between image grade and accuracy of catheter position was observed (p-value was 0.290). Conclusion The ShuntScope can be considered a valuable addition to standard surgical tools in treating pediatric hydrocephalus. Even suboptimal visualization contributes to high rates of correct catheter placement and, thereby, to a favorable clinical outcome

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

This report discusses the clinical experiences with transaqueductal stenting for periaqueductal tumor-related aqueductal stenosis focusing on pediatric patients.&lt;/jats:p&gt; &lt;/jats:sec&gt;&lt;jats:sec&gt; &lt;jats:title&gt;Methods&lt;/jats:title&gt; &lt;jats:p&gt;A retrospective analysis of all patients undergoing endoscopic TAS from 01/1993 to 01/2022 in the author’s departments was performed. Demographic, clinical, radiological, and intraoperative endoscopic data were evaluated. All patients with AS-related occlusive hydrocephalus that was treated with TAS were analyzed and prospectively followed. Special attention has been given to providing insights into indications, surgical technique, and limitations.&lt;/jats:p&gt; &lt;/jats:sec&gt;&lt;jats:sec&gt; &lt;jats:title&gt;Results&lt;/jats:title&gt; &lt;jats:p&gt;Out of 28 endoscopic transaqueductal endoscopis stenting procedures, five procedures were performed on periaqueductal tumor-related obstructive hydrocephalus, two children and three adult patients. CSF pathway was obstructed by tumor located in the aqueduct in 2, by tumor in the thalamus/mesencephalon in 1, by a tumor within the third ventricle in 1, and by a tumor of the lamina tecti in 1. Simultaneously with transaqueductal stenting, 2 endoscopic third ventriculostomies (ETV), 3 tumor biopsies, and 1 tumor resection were performed. Postoperative complications included the following: CSF fistula (1 case), and asymptomatic fornix contusion (1 case). A working aqueductal stent was achieved in all cases based on clinical follow-up evaluation. Postoperatively, all patients showed improvement or resolution of their symptoms. The mean follow-up period was 25.2 months (range, 1–108 months). One patient died due to tumor progression during early follow-up. No stent migration was seen.&lt;/jats:p&gt; &lt;/jats:sec&gt;&lt;jats:sec&gt; &lt;jats:title&gt;Conclusion&lt;/jats:title&gt; &lt;jats:p&gt;Endoscopic third ventriculostomy remains the gold standard for treatment of CSF circulation obstructions with lesions in the posterior third ventricle and aqueduct. Transaqueductal stenting for periaqueductal tumor-related aqueductal compression is technically feasible. However, because of the potential high risks and subtle advantages compared with ETV transaqueductal stenting, it might be indicated in a small subset of well-selected patients if alternative treatment options are not at hand.&lt;/jats:p&gt; &lt;/jats:sec&gt

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
