Nasal stimulation by water down-regulates cough in anesthetized rabbits
Contexte – La stimulation de la muqueuse trachéale peut provoquer une réponse défensive à type de toux. La finalité de cette réponse est la protection des voies aériennes via la clairance du mucus et des particules déposées au sein de l’arbre trachéo-bronchique. La stimulation de la muqueuse nasale est à l’origine de réponses défensives n’incluant toutefois pas la toux. La modulation de la toux suggère de possibles interactions centrales des afférences provenant de localisations anatomiques distinctes. Objectif – Déterminer si une stimulation trachéale mécanique discriminante est capable de provoquer une toux lors d’une apnée provoquée par l’instillation nasale d’eau distillée. Méthodes – Douze lapins anesthésiés et trachéotomisés ont été étudiés. Les stimulations trachéales mécaniques ont été réalisées dans 3 conditions : contrôle, après instillation nasale de sérum physiologique, et lors d’apnée suite à l’instillation d’eau distillée. Résultats – Les paramètres ventilatoires de références ne sont pas différents dans les 3 conditions. Un total de 171 stimulations trachéales a été réalisé. Lors de l’apnée, 81% des réponses sont des réflexes expiratoires et le pic de débit expiré est inférieur (p < 0.0001) à celui observé dans les conditions contrôle et sérum physiologique. L’incidence des réponses comprenant un réflexe de toux est plus faible (p < 0,0001) en cas d’instillation d’eau distillée que dans les 2 autres conditions.Conclusion – La stimulation nasale par l’eau distillée désensibilise les réflexes de défense respiratoire faisant suite à une stimulation trachéale mécaniqueContext - Cough may be triggered by irritation of afferents located in the airway mucosa. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Analysis of training and functional consequences of cardiovascular remodeling studied by MRI in amateur long-distance triathletes
Le triathlon est un sport d’endurance en pleine expansion, connu pour son importante sollicitation cardio-vasculaire. Ce travail porte sur l’impact que peut avoir la pratique du triathlon longue distance (half-ironman et plus) sur le système cardio-vasculaire. Cette rédaction propose une étude descriptive de l’entraînement des triathlètes puis une analyse des variations de la compliance aortique et du remodelage cardiaque mesurés grâce à l’IRM cardio-vasculaire au cours de la saison sportive.34 triathlètes (Tri) de sexe masculin pratiquant le triathlon longue distance et âgés de 19 à 39 ans ont été inclus, ainsi que 29 volontaires sains appariés sur le sexe et l’âge, et insuffisamment actifs (Ctrl) selon les recommandations de l’Organisation Mondiale de la Santé. Les triathlètes ont fourni leurs données d’entraînement via un agenda dédié. Tous les volontaires ont réalisé trois visites (V1, V2, V3) réparties au cours de la saison sportive. Chaque visite comprenait une tonométrie d’aplanation ainsi qu’une IRM cardio-vasculaire. Cet examen d’imagerie a permis entre autres de mesurer les sections aortiques et les volumes ventriculaires droits et gauches.Pour l’année 2018, les triathlètes ont déclaré s’entraîner en moyenne 10,5 ± 2,4 heures par semaine. Au cours des six mois d’entraînement qui ont précédé la course de référence, les paramètres hebdomadaires moyens d’entraînement sont les suivants : training load = 3244 ± 2225 ; monotony = 1,14 ± 0,44 ; strain = 4070 ± 4252 ; fitness = -3610 ± 3967. La compliance de l’aorte ascendante calculée avec la pression pulsée périphérique était significativement plus élevée dans le groupe Tri à V2 (3,85 ± 1,19.10-2 vs 3,25 ± 0,88.10-2 cm²/mmHg ; p = 0,032) et à V3 (3,85 ± 1,09.10-2 vs 3,25 ± 0,92.10-2 cm²/mmHg ; p = 0,027), sans variation longitudinale au cours de la saison sportive (p = 0,465). La majorité des volumes ventriculaires gauches et droits étaient significativement plus élevés dans le groupe Tri et pour les trois visites, mais sans variation longitudinale significative.Ces résultats sont en faveur de l’existence d’adaptations cardiaques stables dans le temps mais d’apparition ancienne. Elles sont liées à la pratique régulière et suffisamment soutenue d’un sport d’endurance comme le triathlon, et peuvent s’apparenter à la notion de cœur d’athlète. Des adaptations vasculaires sont également suggérées par ces résultats, qui pourraient contribuer au concept émergent d’artère d’athlète. Des études longitudinales complémentaires seraient utiles pour évaluer la dynamique de ce remodelage cardio-vasculaire à l’échelle d’une vie d’athlète ainsi que ses conséquences physiopathologiques éventuelles.Triathlon is a fast-growing endurance sport known for its high cardiovascular demand. This paper focuses on the impact that long distance triathlon (half-ironman and above) can have on the cardiovascular system. This writing proposes a descriptive study of the triathletes’ training and then an analysis of variations in aortic compliance and cardiac remodeling mesured by cardiovascular MRI during the race season.Thirty-four 19-39-year-old male long-distance triathletes (Tri) were included, as well as twenty-nine healthy sex- and age-matched volunteers who were insufficiently active (Ctrl) according to World Health Organization recommendations. Triathletes provided their training data through a dedicated diary. All volunteers completed three visits (V1, V2, V3) spread over the course of the race season. Each visit included an applanation tonometry and a cardiovascular MRI. This imaging modality allowed the measurement of aortic sections and right and left ventricular volumes.For the year 2018, triathletes reported training on average 10.5 ± 2.4 hours per week. During the six months prior to the reference race, the average weekly training parameters were: training load = 3244 ± 2225; monotony = 1.14 ± 0.44; strain = 4070 ± 4252; fitness = -3610 ± 3967. The compliance of the ascending aorta calculated with peripheral pulse pressure was significantly higher in the Tri group at V2 (3.85 ± 1.19.10-2 vs. 3.25 ± 0.88.10-2 cm²/mmHg; p = 0.032) and at V3 (3.85 ± 1.09.10-2 vs. 3.25 ± 0.92.10-2 cm²/mmHg; p = 0.027), with no longitudinal variation during the race season (p = 0.465). The majority of left and right ventricular volumes were significantly higher in the Tri group and for all three visits, but without significant longitudinal variation.These results are in favour of the existence of stable but long-standing cardiac adaptations. They are linked to regular and sufficiently sustained practice of an endurance sport such as triathlon and may be related to the notion of athlete's heart. Vascular adaptations are also suggested by these results, which may contribute to the emerging concept of the athlete's artery. Further longitudinal studies would be useful to assess the dynamics of this cardiovascular remodeling over an athlete's lifetime and its possible pathophysiological consequences
Lack of desensitization of the cough reflex in ovalbumin-sensitized rabbits during exercise
Introduction : La toux est un symptôme fréquent dans l’asthme, en particulier à l’effort mais peu des choses sont connus quant aux mécanismes impliqués. L’objectif de cette étude a été d’établir le rôle de l’exercice dans la modulation du réflexe de toux (RT) sur un modèle de lapin anesthésié en ventilation spontanée, présentant une inflammation éosinophilique des voies aériennes. Méthode : Nous avons étudié 10 lapins sensibilisés à l’ovalbumine (OVA) et 8 lapins contrôles. La réponse ventilatoire à la stimulation mécanique trachéale (ST) a été analysée pour chaque lapin en conditions de repos et à l’exercice pour quantifier l’incidence et la sensibilité de la toux. Le lavage bronchioloalaveolaire (LBA) et le comptage cellulaire a été réalisé pour vérifier la présence d’une inflammation à éosinophiles chez les lapins sensibilisés à l’OVA. Pour reproduire l’exercice, des contractions musculaires au niveau des pattes arrière ont été induites par stimulation électrique (CME). Résultats : Au total, 494 ST ont été réalisées, 261 en repos et 233 à l’exercice. Le taux d’éosinophiles dans le LBA a été retrouvé significativement plus élevé chez les lapins sensibilisés à l’OVA (vs contrôles, p=0.008). La CME a permis une augmentation similaire de l’ordre de 35% de la ventilation minute chez les lapins sensibilisés à l’OVA et chez les lapins contrôles par rapport au repos. La sensibilité du RT a été retrouvée significativement diminuée à l’exercice par rapport au repos pour les lapins contrôles (p=0.0313) contrairement aux lapins sensibilisés à l’OVA pour lesquels elle reste inchangée. Conclusion : Le phénomène de “down-regulation” du RT à l’exercice décrit chez les lapins contrôles n’a pas été observé chez les lapins sensibilisés à l’OVA. D’autres études sont nécessaires afin d’établir le rôle spécifique de l’inflammation bronchique sur la disparition du phénomène de “down-regulation” de la toux à l’exercice chez les patients asthmatiquesIntroduction: Cough is a major symptom of asthma frequently experienced during exercise but little is known about interactions between cough and exercise. The goal of our study was to clarify the potential modulation of the cough reflex (CR) by exercise in a spontaneously breathing anaesthetized animal model of airway eosinophilic inflammation. Materials & methods: Ten ovalbumin (OVA) sensitized rabbits and 8 controls were studied. The ventilatory response to direct (TS) performed both at rest and during exercise was determined to quantify the incidence and the sensitivity of the CR. Broncho-alveolar lavages (BAL) and cell counts were performed to assess the level of the airway inflammation following OVA-induced sensitization. Exercise was mimicked by electrically induced hind limb muscular contractions (EMC). Results: Among 494 TS were performed, 261 at rest and 233 at exercise. The OVA sensitized rabbits have a higher level of eosinophil (p=0.008) in BAL. EMC increased minute ventilation by 36% in OVA rabbits vs 35% in control rabbits, compared to rest values. The sensitivity of the CR decreased during exercise compared to baseline in control rabbits (p=0.0313) while it remained unchanged in OVA rabbits. Conclusion: The down-regulation of the CR during exercise in control rabbits was abolished in OVA rabbits. The precise role of airway inflammation in this lack of CR downregulation needs to be further investigated but it might contribute to the exercise-induced cough in asthmatic
Certificat médical de non contre-indication à la pratique sportive par le médecin généraliste: évaluation de fiches d'aide à la consultation dans une population de 73 enfants et 56 adultes
Dans une première partie est rappelée le cadre légal, ainsi que les recommandations cardiovasculaire, ostéo-articulaire et respiratoire régissant la rédaction du certificat médical de non contre-indication à la pratique sportive. L'étude a testé deux fiches d'aide à la consultation du Médecin Généraliste issues des recommandations pour l'enfant et l'adulte. Ces fiches ont été utilisées dans un cabinet de groupe de médecine générale auprès de 73 enfants et 56 adultes. L'acceptabilité par les patients a été très bonne. Les médecins évaluateurs ont été satisfaits de l'outil qui a été un peu modifié suivant les résultats. Chez l'enfant, une pathologie a été découverte chez 3 garçons, justifiant une simple adaptation de l'activité sportive. Chez l'adulte, des examens complémentaires ont été demandés dans 73% des cas, avec une corrélation positive pour l'âge et I'IMC. Une pathologie a été découverte chez 2 hommes, sans conduire à une contre-indication. Ces fiches d'aide à la consultation apparaissent utiles au Médecin Généraliste pour l'aider à rédiger le certificat médical de non contre-indication à la pratique sportive
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
