Analisi di costo-efficacia dell’assistenza in Centro Diurno per pazienti con malattia di Alzheimer
1. Obiettivi dello studio -
L’obiettivo principale della ricerca è stato quello di valutare il rapporto costo-efficacia di un servizio di assistenza diurna che prevede interventi di riabilitazione cognitivo-comportamentale su un campione di pazienti malati di Alzheimer (AD). A tal fine è stata condotta un’analisi comparativa tra i costi (sia pubblici che privati, sia diretti che indiretti) e gli outcome (effetti sullo stato psicologico e sulla qualità della vita di pazienti con AD e dei loro caregiver) di due modelli alternativi di intervento per pazienti con AD residenti in Regione Veneto (Italia): a) assistenza diurna con interventi di riabilitazione cognitivo-comportamentale dei pazienti e supporto alle famiglie e ai caregiver (alternativa CD); b) servizio di assistenza domiciliare tradizionale (alternativa DOM). 2. Disegno e metodologia dello studio - E’ stato analizzato un campione di 53 pazienti (con AD lieve-moderato e moderato-severo) e rispettivi caregiver in un arco temporale compreso tra 18 e 24 mesi: a) 27 pazienti che utilizzavano un servizio di assistenza in Centro Diurno (CD); b) 26 pazienti che utilizzavano servizi di assistenza domiciliare (DOM). Il campione è stato stratificato per età, stato psicologico-cognitivo e capacità funzionali, secondo le caratteristiche del gruppo di pazienti seguito presso il Centro Diurno. I soggetti sono stati valutati clinicamente al tempo T0 (baseline), dopo 6 mesi (T1), dopo 12 mesi (T2) e dopo 18 mesi (T3). Per tutta la durata dello studio sono stati monitorati i costi diretti e indiretti, formali ed informali sostenuti, sia dalla famiglia che dagli enti pubblici (Regione e Azienda sanitaria locale) per l’assistenza al malato di AD. L’analisi dei costi delle due alternative CD e DOM viene effettuata attraverso un modello statistico multilivello. L’analisi di efficacia dell’intervento di riabilitazione cognitivo-comportamentale dei pazienti in CD viene effettuata con l’ausilio di un random effect model.
3. Risultati -
Sono stati rilevati i costi tangibili relativi ai pazienti dei due gruppi esaminati (trattati in CD e trattati a DOM) e ai loro caregiver principali, distinguendo tra costi diretti (inclusivi dei costi di assistenza informale) e costi indiretti, e tra costi privati e pubblici. Il costo diretto giornaliero per paziente dell’alternativa CD risulta più elevato (tra + 21 e + 37 €) di quello dell’opzione DOM a causa dei maggiori costi sanitari e di assistenza formale, a fronte dei maggiori costi di assistenza informale sostenuti dalle famiglie che caratterizzano l’opzione DOM. Considerando i costi indiretti (perdite di produttività e costi sanitari dei caregiver) e l’eterogeneità nelle caratteristiche demografiche e socio-economiche dei pazienti, il differenziale di costo tra CD e DOM si riduce e tende a diventare statisticamente non significativo. L’analisi svolta con random effect model sembra segnalare una maggiore capacità del CD, rispetto alla tradizionale modalità di assistenza a domicilio (DOM), di favorire il mantenimento delle capacità cognitive e, soprattutto, di controllare i disturbi comportamentali dei pazienti

Abstract
SiC/SiC ceramic matrix composites (SiCf/SiC CMCs) are regarded as the new materials for the hot-section components of aircraft gas turbine engines, since they have one-third of the density of metallic superalloys, a higher temperature capability, good mechanical strength, and excellent thermal shock resistance. However, high-temperature water-vapor-rich combustion gases can induce severe surface recession phenomena in SiC/SiC leading to component failure. For this reason, it is necessary to design protective coatings, i.e., environmental barrier coatings (EBCs), able to protect the SiC/SiC surface in combustion environments. In the present work, ytterbium monosilicate (Yb2SiO5), stable when exposed to water vapor at high temperatures, and ytterbium disilicate (Yb2Si2O7), characterized by a thermal expansion coefficient closer to that of the substrate, were selected for a multilayer EBC system. EBCs were processed using the atmospheric plasma spray (APS) technique. A set of deposition parameters were tested, varying the power of the torch, and the composition and microstructure of the deposited coatings were studied in terms of porosity, crack density, and post-deposition phase retention by performing SEM, EDS, and XRD analysis. The results allow for the definition of the influence of deposition parameters on the final properties of multilayer EBC coatings

L'ampia analisi proposta tocca tutte le opere dell'autrice evidenziandone i legami intertestuali, anche tra poesia e narrativa, e i progressivi sviluppi in un arco cronologico esteso, tra anni Sessanta e primi anni Duemila. Lo studio coglie anche l'importanza dei riferimenti alle arti visive, in particolare alla fotografia, che Giulia Niccolai ha praticato in prima persona negli anni della Neoavanguardia, e alla pittura americana.The essay places the narrator and poet Giulia Niccolai in the canonical twentieth century Italian literary discussing her poetics and stylistic figures. BDC-Decomposition for global influence analysis
In biochemical networks, the steady-state input-output influence is the sign of the output steady-state variation due to a persistent positive input perturbation; if the sign does not depend on the value of the strictly positive system parameters, the influence is structural. As recently shown for small perturbations, when the linearized system approximation is valid, steady-state input-output influences can be structurally assessed, for biochemical networks with m unknown parameters, by means of a vertex algorithm with complexity 2m. This letter shows that the structural input-output influence of a biochemical network is a global property, which does not require any small-perturbation assumption. It also shows that, using a new algorithm, the complexity can be reduced down to 2m-n , where n is the system order, thus drastically reducing the computation time. Finally, when the uncertain parameters belong to known intervals, non-conservative bounds are given for the steady-state ratio between output and input, allowing for sensitivity analysis.Accepted Author ManuscriptTeam Tamas Keviczk

Writer, journalist, theater and film critic, Marco Ramperti is one of the few that deals with dance in the Italian press of the fascist period. For the newspaper «La Stampa», between 1926 and 1928, he edits the column Luoghi di danza (dance venues), dedicated to ballroom and modern dances. The article investigates the performative nature of Ramperti’s writing, dominated by “staging” devices of the subject matter, as well as by an intrusive presence of the body, first of all that of the author. Un’analisi genetica della scrittura e dell’evoluzione poetica nei Quaderni in Ottavo di Franz Kafka
The eight Octavo Notebooks, written by Kafka in the crucial months between the end of 1916 and the beginning of 1918, represent for many reasons a precious and partially still unexplored source for Kafka studies. Not only do they include some of the greatest and most representative short prose works of the author (as The Great Wall of China, the Gracchus’ fragments, The Bucket Rider, A Report to an Academy), but they also bear witness to a highly creative and experimental writing phase, in which short stories, diary records, aphorisms and moral reflections come together as fragments and rough drafts. Far from considering the heterogeneity and fragmentary nature of these notebooks as an obstacle to a systematic interpretative analysis of the texts, this article will show how this extraordinary stylistic and thematic variety, as well as the private character of the drafts, provides useful elements for a textual-genetic approach and offers the perfect opportunity to reflect upon the relationship between ‘life’ and ‘writing’. The article intends to bring to light the peculiarity and the research potential of the Octavo Notebooks considered as a whole, which will be done through a comprehensive and chronological examination of the eight notebooks on the one hand, and a comparison with the other private documents of the author on the other. This kind of approach will enable to gain a careful insight into Kafka’s writing process and into his characteristic metaphorical system
