He discusses the issue of sexual abuse by priests within the Catholic Church. He discusses books that have influenced his thinking, and out-of-print books that he has brought back into print. Characterising bi-atrial mechanisms of atrial fibrillation using high density non-contact mapping
A universal description of mechanisms of AF activation remains elusive. This thesis seeks to apply emerging techniques of global non-contact mapping to further understand mechanisms of AF activation and identify patient specific phenotypes. Methods were developed to allow quantification of complex patterns of activation and apply signal processing tools including dominant frequency and phase analysis to characterise AF properties. These methods were applied to analyse the impact of adenosine during AF, which is known to alter atrial electrophysiological properties. A significant shortening of atrial fibrillation cycle length, alongside an increase in dominant frequency was associated with a promotion of rotational patterns of activation throughout the left atrium. Extended and repeated recordings were analysed to identify both the optimal time period required for mapping and the spatial consistency of complex activation. Localised irregular activation patterns characterised by wavefront pivoting, partial rotation and splitting were highly spatially stable and coincided with areas of increasing conduction heterogeneity during short coupled atrial pacing.A technique using mean phase coherence was developed to allow analysis of interatrial propagation during simultaneous bi-atrial mapping of AF. Properties of atrial activation were identified to reveal predictors of AF termination with targeted ablation. Differences in the balance of interatrial activation could be observed between patients with changes in right atrial activation properties reflecting more arrhythmia complexity, refractory to acute termination with ablation.A novel method was developed to attempt to further characterise individual AF phenotypes based on whole chamber patterns of AF activation. The concept of recurrence was used to calculate both whole chamber and localised measures of organisation which was shown to correlate closely with both clinical AF phenotype and predict acute AF termination with ablation.Finally, clinical outcomes of a cohort of patients treated with a charge density guided ablation strategy are presented.In conclusion, novel insights into the bi-atrial properties of AF activation identified with charge density non-contact mapping are presented alongside a novel technique to quantify whole chamber organisation during AF, which aims to identify AF phenotypes and predicts acute AF termination with ablation.<br/
An evaluation of the relative success of internal and external electrical cardioversion of atrial arrhythmias in adults, including a comparison of single and dual coil lead devices
Introduction: Although there is a high prevalence of atrial arrythmias in patients with cardiovascular implantable electronic devices (CIED), few studies have compared the efficacy of cardioversion modalities for the restoration of sinus rhythm. The primary aim of this study is to compare the efficacy of internal and external first-time cardioversion for the treatment of atrial arrythmias in patients with an implantable cardioverter defibrillator (ICD), including the relative efficacy of dual and single coil leads.Methods: Demographic and clinical information was collected and analysed from consecutive electronic records of patients with a CIED who received atrial cardioversion at a single NHS trust between 2008 and 2023. Non-parametric statistical analyses were undertaken to enable the relative success of first-time cardioversion for different modalities to be determined.Results: 163 patients were identified, of which 71 received external and 92 received internal cardioversion. External cardioversion had the highest first-time efficacy (90.1%) with a statistically significant difference in first-time success between the external and combined internal cohorts (p=.002). Dual coil leads had superior first-time efficacy (80.4%) compared to single coil leads (56.1%), with a statistically significant difference between them (p=.022).Conclusions: First-time internal cardioversion success is superior for patients with dual coil leads compared to single coil leads. There is no statistical difference between first time success for dual coil internal and external cardioversions, making an initial attempt at internal cardioversion a potentially credible clinical option for patients with dual coil leads
Global Substrate Mapping and Targeted Ablation with Novel Gold-tip Catheter in De Novo Persistent AF
Results from catheter ablation for persistent AF are suboptimal, with no strategy other than pulmonary vein isolation showing clear benefit. Recently employed empirical strategies beyond pulmonary vein isolation involve widespread atrial ablation in all patients and do not take into account patient-specific differences in AF mechanisms or phenotype. Charge density mapping using the non-contact AcQMap system (Acutus Medical) allows visualisation of whole-chamber activation during AF and reveals localised patterns of complex activation thought to represent important mechanisms for AF maintenance that can be targeted with focal ablation. In this review, the authors outline the fundamentals of this technology, the initial data exploring the mechanistic role of activation patterns seen and the application to ablation of persistent AF
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Thermal hydraulic challenges of Gas Cooled Fast Reactors with passive safety features
Transient response of a Gas Cooled Fast Reactor (GFR) coupled to a recompression supercritical CO(2) (S-CO(2)) power conversion system (PCS) in a direct cycle to a Loss of Coolant Accident (LOCA) and a Loss of Generator Load Accident is analyzed using RELAP5-3D. A number of thermal hydraulic challenges for GFR design are pointed out as the designers strive to accommodate cooling of the high power density core of a fast reactor by a gas with its inherently low heat transfer capability, in particular under post-LOCA events when system pressure is lost and when reliance on passive decay heat removal (DHR) is emphasized. Although it is possible to design a S-CO(2) cooled GFR that can survive LOCA by cooling the core through natural circulating loops between the core and elevated emergency cooling heat exchangers, it is not an attractive approach because of various bypass paths that can, depending on break location, degrade core cooling. Moreover, natural circulation gas loops can operate in deteriorated heat transfer regimes with substantial reduction of heat transfer coefficient: as low as 30% of forced convection values, and data and correlations in these regimes carry large uncertainties. Therefore, reliable battery powered blowers for post-LOCA decay heat removal that provide flow in well defined regimes with low uncertainty, and can be easily overdesigned to accommodate bypass flows were selected. The results confirm that a GFR with such a DHR system and negative coolant void worth can withstand LOCA with and without scram as well as loss of electrical load without exceeding core temperature and turbomachinery overspeed limits. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
