Abstract
We present a case report of laparoscopic management of a spontaneous hemoperitoneum in the second trimester of pregnancy. The patient was a 40-year-old woman at 15 weeks of gestation. At laparoscopic surgery, the hemoperitoneum was evacuated, and the right-sided uterine vessels were closed with diathermocoagulation. Every pregnant woman with severe abdominal pain, vomiting, and imminent hypovolemic shock should be carefully evaluated. After ruling out the most prevalent causes of hemoperitoneum, idiopathic spontaneous hemoperitoneum should be considered. Rapid diagnosis and aggressive fluid replacement together with prompt surgical intervention may be the only chance for a favorable outcome for both mother and child in the presence of such a rare complication. Moreover, in early stages of pregnancy, the laparoscopic approach should be considered but only in the hands of experienced laparoscopic surgeon

The revision of Monetary Theory in Fanno's Analysis
The quantity theory of money equation, the neutral money convinction and the understanding about the bank as a credit intermediary are the focus of the monetary analysis from the beginning to the middle of the past century. Fanno is interested in these subjects and he develops the Wicksell’s model thanks to the co-ordination with the exchange equation and with some factors of the monetary and credit balance. So Fanno opens a new mainstream of economic analysis. He emphasizes the profit rate and his notion of time lags which magnify exogenous changes is crucial. Lo scopo di questo saggio è duplice. Nella prima parte vengono presentati gli elementi principali del dibattito teorico sul tema del rapporto tra lo Stato e il Mercato nel campo dell’istruzione (anche alla luce della recente letteratura sulla crescita economica che ha aggiunto elementi nuovi al dibattito tradizionale). Nella seconda parte vengono analizzati, utilizzando principalmente i dati disponibili sulla loro prima erogazione, le caratteristiche e l’esito della politica dei buoni scuola regionali per verificare la corrispondenza con le indicazioni della teoria economica. Dall’esperienza regionale italiana emerge il fatto che occorre integrare l’analisi di economia politica con l’analisi del piano normativo e del dibattito ideologico – come sempre accade quando ci si confronta con ambiti di scelta collettiva
La dinamica dello sviluppo economico nell'approccio di Amoroso
The formulation of a complete theory of economic dynamics was one of the tasks left to fu-ture generations by the first generation of marginalist economists. Among those who took up the challenge in Italy, Amoroso deserves pride of place: he was considered a pioneer, both in terms of his analytical techniques and for certain interpretations he offered during the golden days of dynamic theory in the 1950s and 1960s.
The orthodox doctrine of economic equilibrium hinges on the assumption that equilib-rium is reached instantly, and that once it has been reached it endures indefinitely, on condition that the underlying conditions remain constant over time. Amoroso endeavoured to arrive at a dynamic theory of equilibrium, based on the reciprocal actions and reactions in the industrial, trade and banking sectors. Forces tend to evolve, generating opposing trends, and the dynamics in an economy are thus the result of reciprocal actions and reactions in the three sectors. Amor-oso’s conception was grounded in corporative economics, and this induced him to emphasise state intervention. However he failed to include the latter as a corrective measure with which to counteract cyclical dysfunctions in the economy. The Great Depression and the Corporatist Shift of Italian Economists
The profound crisis which occurred in economic theory in the period between the two wars forced the neoclassical economists to look in different directions. One of them was the corporate economic theory – understood as a theory which asserted the fundamental role of the state in economic affairs. The protagonists of this attempt to integrate in Italy corporatism into economic science were L. Amoroso, G. La Volpe, E. Fossati, G. Del Vecchio, M. Fanno, G. Papi, and G. Masci, and therefore leading economists of the Italian school who adhered to both the Marshallian and Paretian traditions. The discussion in Italy was concerned more with economic policy problems than rigorous theoretical analysis. What interested the Italian economists was not the development of new theories, but the introduction of new economic institutions able to give concrete responses to the dramatic economic problems of the time. Abstract
OBJECTIVE: To describe how a hydronephrosis can lead to a difficult differential diagnosis between endometriosis and retroperitoneal fibrosis.
SETTING: Department of Obstetrics and Gynecology, Sacro Cuore Don Calabria General Hospital, Negrar, Verona, Italy.
PATIENT(S): The history of a 34-year-old woman revealed the appearance of hydroureteronephrosis on the right side at the 35th week of pregnancy. She had an magnetic resonance imaging scan and was diagnosed with a spread retroperitoneal fibrosis. After 2 months, the patient reported the occurrence of pelvic pain, dyspareunia and dysmenorrhea. She was treated with corticosteroids and tamoxifen with no results.
INTERVENTION(S): Laparoscopic surgery. A complete retroperitoneal extirpation was done of an endometriotic nodule of the right broad ligament, near the right ureter (without stenosis).
MAIN OUTCOME MEASURE(S): Reduction of pelvic pain.
RESULT(S): She noticed an important decrease of pain.
CONCLUSION(S): The cause of hydronephrosis could be a physiologic hydroureteronephrosis, which is the most common cause of dilatation of the urinary tract in pregnancy. The pain symptoms of the patients seemed to be linked to endometriosis and not to retroperitoneal fibrosis. Magnetic resonance imaging sometimes does not enable a correct diagnosis between these two pathologies. Fertile women with suspected fibrosis should undergo a diagnostic laparoscopy by an expert surgeon in retroperitoneal surgery
