    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Effect of structured education programmes on home care nursing of chronic disease: an italian experience (2008-2012)

    Background The growing number of patients with chronic illness led to new models of carewhere patients and caregivers take a more active role. We developed a project focused on patient empowerment and family coaching. Theaim was toincrease appropriateness, efficiencyand quality ofhome care services. Methods The study was designed during 2007 by Alta Val d’Elsa‘s (one of the four areas of Siena Health Unit – Tuscany, Italy) primary care nurses using auditing techniques. An epidemiological analysis was performed to assess healthcare needs of the population. Structured training programmes for different pathologies were developed. Sentinel events were established for every disease. Each patient was assigned to a nurse case manager available by phone when on duty and responsible for training and verifying learning. We identified two outcome indicators:the reduction of percentage of low-complexity home care nursing interventions (HCNI), considered at high risk of inappropriateness, and the growth of high-complexity HCNI. The complexity of all possible interventions was established by Local Health Unit management. The project started in January 2008. A satisfaction survey (a one to ten scale, the higher the value, the better) was given to patients and caregivers. Results From 2008 through 2012, 20044 training activities were done for 3667 patients and caregivers. The overall number of interventions increased from 46 456 in 2007 to 52 767 in 2012. The percentage of low-complexity HCNI decreased from 57.5% in 2007 to 51.4% in 2012 while in the other three areasof the Local Health Unit decreased from 61.2% in 2007 to 59.6% in 2012. The number of high-complexity HCNI rose from 11 295 (24.3% of interventions) in 2007 to 17 662 in 2012 (33.5% of interventions); the mean percentage of high-complexity HCNI of the other three areas remained 21.3%. The level of satisfaction, evaluated only in 2008-09, was high: 9.35 for patients and 9.1 for caregivers. Conclusions Our findings suggest that structured educationprogrammes for patients and caregivers allow nurses to dedicate themselves to more complex and appropriate tasks and lead to an increase of number and quality of home care nursing interventions

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    Self-care management e family coaching: coinvolgere attivamente pazienti e familiari nell'assistenza domiciliare

    Background. L’elevata prevalenza di patologie croniche e la necessità di ottimizzare l’uso delle risorse stanno portando alla diffusione di modelli assistenziali di self-care management e family coaching, in cui i pazienti, adeguatamente supportati, assumono un ruolo attivo nella cura di sé stessi e i caregiver adottano misure in grado di prevenire un peggioramento delle loro condizioni. Obiettivi. Nella Zona-Distretto Alta Val d’Elsa dell’AUSL 7 di Siena, dal 2008 è stata avviata una sperimentazione di questi modelli con l’obiettivo di aumentare appropriatezza e qualità degli interventi domiciliari, ottimizzare l’utilizzo delle risorse, valorizzare le professionalità coinvolte nell’assistenza territoriale. Metodi. A partire da luglio 2007, nella fase di preparazione (6 mesi) basata sul learning audit, il team infermieristico dell’Alta Val d’Elsa ha: condotto un’analisi epidemiologica per identificare il bisogno assistenziale su cui intervenire con prestazioni ad alta e bassa complessità; redatto schede contenenti le prestazioni da effettuare per ogni attività di formazione; creato tabelle di planning-educativo, con tipologia e tempistica degli interventi; creato opuscoli informativi; definito gli eventi sentinella per la verifica della qualità dell’assistenza. La sperimentazione ha avuto inizio a gennaio 2008: l’inclusione nel nuovo servizio era valutata in modo partecipato con pazienti e/o caregiver. Ogni caso aveva un infermiere di riferimento. Le attività di formazione a domicilio erano programmate dopo l’analisi della cartella infermieristica e la valutazione con le scale di Braden, Conley, Barthel modificata e Mini Nutritional Assessment. Conseguentemente si stipulava un “contratto assistenziale”. Al termine del percorso era somministrato a pazienti e caregiver il questionario sul gradimento del servizio. Risultati. Da gennaio 2008 a dicembre 2012 sono stati inclusi nel servizio 3.667 pazienti e sono state effettuate 20.044 attività di addestramento. La percentuale di prestazioni a bassa complessità si è ridotta dal 57,5% del 2007 al 51,4% del 2012; la media delle altre 3 Zone della AUSL è passata dal 61,2% al 59,6%. La percentuale di prestazioni ad alta complessità è aumentata dal 24,3% del 2007 al 33,5% del 2012; la media delle altre 3 Zone è passata dal 21,2% al 21,3%. Pazienti e caregiver hanno riferito un gradimento del servizio rispettivamente pari a 9.35/10 e 9.10/10. Limiti. Il confronto tra le prestazioni delle quattro Zone è stato condotto senza aggiustamenti. I questionari di “gradimento del servizio” non sono stati validati. Conclusioni.L’utilizzo degli interventi di formazione ha permesso a pazienti e caregiver di assumere un ruolo attivo e consapevole nell’assistenza, liberando risorse del territorio, permettendo di intensificare l’assistenza a più alto indice di complessità ed aumentando la job satisfaction. L’organizzazione così ridefinita ha tratto benefici in termini di appropriatezza e di più efficiente risposta ai bisogni dei pazienti
