Angioma artero-venoso segmentario del dorso del naso nel carrefour fra i due circoli carotidei, esterno ed interno
Gli angiomi arterovenosi segmentari della regione maxillo-facciale sono di rarissimo riscontro.Un precedente studio sui tumori vascolari etmoido-mascellari, sia pure del 1976, ne metteva in evidenza infatti solo 14 osservazioni. Gli AA. presentano un caso di angioma arterovenoso del terzo medio-superiore dx del dorso del naso, che suscita particolare inte- resse perché situato nell’area di confluenza (inosculazione) delle aa.mascellare est.ed oftal- mica. Si trattava di un soggetto di sesso femminile, di 78 anni. La lesione, a cavocciolo, appariva di forma irregolarmente ovalare, colorito rosso-bluastro, asse maggiore di circa 25 mm, integrità della cute di rivestimento; risultava mobile sul piano osseo sottostante, presentava consistenza molle, non era dolente. Alla palpazione, inoltre, non si percepivano fremiti ma un carattere pul-sante, soprattutto in corrispondenza del polo superiore, prossimo alla regione del canto interno. Data la sede particolarmente critica, punto di confluenza tra i due circoli carotidei (a.oftalmica dalla c. interna e a. facciale dalla c. esterna) la formazione è stata accuratamente studiata in fase preoperatoria con Ecocolor Doppler e con angiografia digitale selettiva del circolo carotideo. Tale diagnostica si è dimostrata essenziale per l’approccio chirurgico, orientando soprattutto l’isolamento ed il timing di procedura sui vasi afferenti ed efferenti. Dopo l’asportazione della formazione angiomatosa è stato applicato uno splintaggio di contenzione per il controllo di eventuali fenomeni di sanguinamento postoperatorio.
Gli Autori sottolineano come tali casi, da esaminare, come nello specifico, in collaborazione con il chirurgo vascolare, si giovino espressamente di studi angiografici. Le dettagliate informazioni che la metodica fornisce, infatti, sono preziose, oltre che per la rappresentazione della reale distribuzione dei vasi coinvolti, anche per scongiurare sia complicanze emorragiche talora gravi che fenomeni tromboembolici dei vasi endocranici

Abstract
Risk assessment in health care facilities. The risk assessment is an important procedure in occupational medicine, also due to the international and domestic regulations and recommendations. In hospitals and other health care facilities too, the risk assessment could enable the identification of hazards, the characterization of risk and thus the programme of adequate preventive measures. In this paper some theoretical and methodological aspects of risk assessment practice and related documentation in health care facilities are focussed and the strict relation between preventive activities, quality system and accreditation standards for hospitals and other health services is finally discussed
Cerebrovascular reactivity and subcortical infarctions
OBJECTIVES:
To investigate the association between different kinds of ischemic lesions and cerebrovascular reactivity (CR) and to evaluate their relationships with the major risk factors for stroke.
SUBJECTS AND METHODS:
We evaluated CR using the breath-holding index technique during bilateral transcranial Doppler monitoring of flow velocity in the middle cerebral arteries of 41 consecutive patients attending our clinic for a recent, first-ever, ischemic stroke and in 15 control subjects. Based on the location of the lesion determined by computed tomography, the following 3 types of infarctions were identified: cortical (or territorial), single subcortical, and subcortical with multiple silent subcortical infarctions. Patients with a condition of severe carotid artery stenosis or occlusion, which in itself could account for altered CR, were excluded from this study. All physiological and pathologic conditions that could possibly cause an impairment in CR were recorded.
RESULTS:
The breath-holding index was significantly lower in the multiple subcortical infarctions group than in the control subjects (P < .001), single subcortical infarctions group (P < .01), and cortical infarctions group (P < .01). In all of the groups male sex (P < .05) and a history of hypertension (P < .05), regardless of whether hypertension was treated, correlated with low CR. The multiple regression analysis indicated that the only significant factor able to influence the breath-holding index was the type of lesion.
CONCLUSIONS:
Nonstenotic patients with first-ever stroke who had a recent symptomatic subcortical infarction associated with multiple silent infarctions seem to have an impaired cerebrovascular reserve capacity. The strong association of subcortical infarctions with multiple silent infarctions with low CR indicates the role of small vessel vasculopathy and hypoperfusion as possible pathogenetic mechanisms of subcortical infarctions with multiple silent infarctions
Electro-thermal characterization and modeling of a 4-wire microheater for lab-on-chip systems
The paper proposes a part of an extended research devoted to the application of Lab-on-Chip systems for the detection of viral infections. The focus is on accurate lumped element modeling, simulation and experimental characterization of the thermal behavior of an integrated glass micro-heater, useful for the design and simulation of dedicated electronic controlling systems. A lumped three-compartment model for the analysis of the thermal behavior of a heater has been proposed and discussed. The mathematical model was extensively simulated, and the associated parameters have been chosen in order to minimize the L2 norm of the error with respect to experimental data collected though an experiment campaign
