    Maternal and child undernutrition: effective action at national level

    80% of the world's undernourished children live in just 20 countries. Intensified nutrition action in these countries can lead to achievement of the first Millennium Development Goal (MDG) and greatly increase the chances of achieving goals for child and maternal mortality (MDGs 4 and 5). Despite isolated successes in specific countries or for interventions--eg, iodised salt and vitamin A supplementation--most countries with high rates of undernutrition are failing to reach undernourished mothers and children with effective interventions supported by appropriate policies. This paper reports on an assessment of actions addressing undernutrition in the countries with the highest burden of undernutrition, drawing on systematic reviews and best-practice reports. Seven key challenges for addressing undernutrition at national level are defined and reported on: getting nutrition on the list of priorities, and keeping it there; doing the right things; not doing the wrong things; acting at scale; reaching those in need; data-based decisionmaking; and building strategic and operational capacity. Interventions with proven effectiveness that are selected by countries should be rapidly implemented at scale. The period from pregnancy to 24 months of age is a crucial window of opportunity for reducing undernutrition and its adverse effects. Programme efforts, as well as monitoring and assessment, should focus on this segment of the continuum of care. Nutrition resources should not be used to support actions unlikely to be effective in the context of country or local realities. Nutrition resources should not be used to support actions that have not been proven to have a direct effect on undernutrition, such as stand-alone growth monitoring or school feeding programmes. In addition to health and nutrition interventions, economic and social policies addressing poverty, trade, and agriculture that have been associated with rapid improvements in nutritional status should be implemented. There is a reservoir of important experience and expertise in individual countries about how to build commitment, develop and monitor nutrition programmes, move toward acting at scale, reform or phase-out ineffective programmes, and other challenges.     Food price volatility is one of the major challenges facing the global agricultural system today. This was most vividly illustrated during the global food crisis of 2007–9 when price spikes occurred for key staple food commodities—such as wheat, rice, maize, and soybeans. Given the variety of reactions by governments of countries experiencing similar food price shocks, the 2007–9 crisis offered an excellent natural experiment for generating knowledge on responses to price volatility in particular and on the political economy of agricultural policy-making more generally. This book contains the wealth of collaborative research by a global team of experts on food price policy—the research was undertaken on a sizeable group of low- and middle-income countries that were highly affected by the 2007–9 food crisis. The central aim of the study is to uncover which political economy factors—ranging from the constellation of different interest groups to the nature of political institutions—explain variations in policy responses across countries. The research output proves valuable for at least three target audiences. First, it can inform international organizations and donors about which types of policy interventions can mitigate price volatility and whether they are feasible given a country’s political economy context. Second, it can help national policy makers better understand the trade-offs of certain policy interventions. Third, it generates much-needed further knowledge about the agricultural policy-making process in developing countries, which remains incredibly scarce despite the importance of agriculture to these countries’ economies

No commercial use is permitted without permission.Migration plays an important role in development and as a strategy for poverty reduction. A recent World Bank investigation finds a significant positive relationship between international migration and poverty reduction at the country level (Adams and Page 2003). Burkina Faso, whose conditions for agriculture are far from favorable, has a long history of migratory movement, and migration within West Africa has long taken place in response to drought and low agricultural productivity. In recent decades, migration to destinations outside the African continent and in particular to Western Europe has become more important for migrants from Burkina Faso.Migration can be considered a livelihood diversification strategy because remittances resulting from migration constitute an income source that is uncorrelated with household income from agriculture. Migration affects the sending household in three ways. First, when a household member migrates, the household loses labor. Second, migration often results in remittances. Third, migration implies a reduction in household size for consumption. It is likely that both motives for and consequences of migration will differ by whether the destination is within Africa or outside Africa. Migration to destinations outside Africa is expensive in terms of transport costs but generates a comparatively high level of remittances for the household.This case study discusses the determinants and consequences of migration for households in four villages situated on the Central Plateau of Burkina Faso. Two forms of migration are distinguished: migration within Africa (continental migration) and migration to a destination outside Africa (intercontinental migration). A critical question is what happens to the welfare of rural households when they engage in either form of migration. When households lose labor, it may be harder for them to participate in and generate income from other activities such as agriculture. Remittances may partly compensate for these negative effects. No commercial use is permitted without permission.The Right to Food Campaign in India began in 2001. It was a time of absurd paradox. Even as the foodgrain stocks held by the government rose to 50 million metric tons, several parts of the country were reeling from a third consecutive year of drought. The threat of severe hunger loomed large, yet efforts to address this threat were insufficient. In April 2001 the People’s Union for Civil Liberties, Rajasthan, an active civil society group in the north Indian state of Rajasthan, submitted a writ petition to the Supreme Court of India. Briefly, the petition demanded that the country’s food stocks be used without delay to protect people from hunger and starvation. This petition led to a prolonged “public interest litigation”1 (PUCL vs. Union of India and Others, Writ Petition [Civil] 196 of 2001). Supreme Court hearings have been held since then at regular intervals, and significant “interim orders” have been issued by the court from time to time regarding the scope and implementation of eight food-related schemes of the Government of India. The litigation provided a springboard for the Right to Food Campaign.The Right to Food Campaign (RFC) asserts that “everyone has a fundamental right to be free from hunger and malnutrition and that the primary responsibility for guaranteeing these entitlements rests with the state” (Right to Food Campaign 2005). Further, if people’s basic needs are not a political priority, then state intervention itself depends on effective popular organization using democratic means. In broad terms, the RFC’s role is to ensure that hunger and malnutrition become a political priority and that resources reach the intended beneficiaries. The Right to Food Campaign is, however, not merely a pressure group that secures increased allocations to food schemes. It is a social movement with a much broader agenda, playing an important role in bringing down the barriers that people face in gaining access to the programs and resources to which they are entitled.In the Indian context, barriers of various kinds, including corruption, apathy, and many forms of social discrimination, make it difficult, and at times even impossible, for the intended beneficiaries to gain access to the programs expressly meant for them. Those at risk of hunger are necessarily poor, but also tend to be socially powerless and marginalized. The RFC recognizes that the realization of the right to be free from hunger and malnutrition depends critically on entitlements to livelihood security, such as the right to work, land reform, and social security.From the perspective of people within state institutions, social movements such as the Right to Food Campaign pose a confounding dilemma. On the one hand, the Right to Food Campaign is an ally in the government’s fight against hunger and malnutrition. The RFC achieves its goals through collective action—by taking on the state and its functionaries, often exposing the weak links, leakage, and corruption.
