He is the author of three novels: Hatteras Light, Cape Fear Rising, Desert Kill; two books of nonfiction, Brilliant Passage...a schooning memoir and Creative Nonfiction - Researching and Crafting Stories of Real Life, and Writing a Book that Makes a Difference. His most recent book is Secret Soldiers, about the first and last battlefield deception outfit ever authorized by the U.S. Army. Gerard has written shows for public television and radio.     L'Alzheimer (EA) és la demència més prevalent i la manca de tractaments curatius fan necessari estudiar factors que podrien prevenir i/o endarrerir-ne la progressió com els trastorns del son; entre aquests, la síndrome de l'apnea hipoapnea del son (SAHS). Objectiu: Conèixer la prevalença de SAHS en pacients amb EA lleu-moderat. Avaluar si la presència de SAHS influeix en la seva evolució i valorar la utilitat de les escales de cribratge. Mètode: Estudi observacional de cohorts prospectiu. Subjectes amb diagnòstic nou d'EA i MMSE≥20 punts van ser sotmesos a: bateria de tests neuropsicològics (ADAS-Cog), analítica, punció lumbar, RMN i polisomnografia (PSG). A l'any es va repetir l'ADASCog i MMSE. Resultats: edat mitjana 75 anys, 58% dones, 59% hipertensos i 76% amb sobrepès-obesitat. Alta prevalença de SAHS (90.6%) segons PSG, de grau moderat-greu (68%). No diferències en l'evolució cognitiva (EC) a 12 mesos entre els grups amb SAHS i sense. Qüestionari STOP-Bang i Berlin no van ser eines de cribratge útils. Conclusions: Hi ha una alta prevalença de SAHS no diagnosticada en pacients amb EA lleu-moderat i aquesta no empitjora la seva EC.El Alzheimer (EA) es la demencia más prevalente y la falta de tratamientos curativos hacen necesario estudiar factores que podrían prevenir y/o atrasar su progresión como los trastornos del sueño; entre estos, el síndrome de la apnea hipoapnea del sueño (SAHS). Objetivo: Conocer la prevalencia de SAHS en pacientes con EA leve-moderado. Evaluar si la presencia de SAHS influye en su evolución y valorar la utilidad de las escalas de cribado. Método: Estudio observacional de cohortes prospectivo. Sujetos con diagnostico nuevo de EA y MMSE≥20 puntos fueron sometidos a: batería de test neuropsicológicos (ADAS-Cog), analítica, punción lumbar, RMN y polisomnografía (PSG). Al año se repitió el ADASCog y MMSE. Resultados: edad media 75 años, 58% mujeres, 59% hipertensos y 76% con sobrepeso-obesidad. Alta prevalencia de SAHS (90.6%) según PSG, de grado moderado-grave (68%). No diferencias en la evolución cognitiva (EC) a 12 meses entre los grupos con y sin SAHS. Cuestionario STOP-Bang y Berlin no fueron herramientas de cribaje útiles. Conclusiones: Existe una alta prevalencia de SAHS no diagnosticada en pacientes con EA leve-moderado y esta no empeora su EC.Alzheimer's (EA) is the most prevalent dementia and the lack of curative treatments makes it necessary to study factors that could prevent and/or delay its progression, such as sleep disorders; among these, sleep apnea hypopnea syndrome (SAHS). Objective: To know the prevalence of SAHS in patients with mild-moderate EA. To evaluate whether the presence of SAHS influences its evolution and to assess the usefulness of the screening scales. Method: Prospective observational cohort study. Subjects with a new diagnosis of EA and MMSE≥20 points were subjected to: neuropsychological test battery (ADAS-Cog), analysis, lumbar puncture, MRI and polysomnography (PSG). The ADASCog and MMSE was repeated one year later. Results: average age 75 years, 58% women, 59% hypertensive and 76% overweight-obese. High prevalence of SAHS (90.6%) according to PSG, moderate-severe grade (68%). No differences in cognitive evolution (EC) at 12 months between the groups with and without SAHS. STOP-Bang and Berlin questionnaire were not useful screening tools . Conclusions: There is a high prevalence of undiagnosed SAHS in patients with mild-moderate EA and this does not worsen his EC

    Alexander Gerard, a schottish philosopher, is nowadays almost forgotten, but at the time of Kant’s philosophical development he was a very popular author and his works were translated to german. Kant mentioned Gerard sometimes in his lectures on anthropology, particularly about genius and imagination. A near analysis and comparing of their conception of genius and imagination shows a significant influence of Gerard on Kant. Purpose of this paper is to analyse this influence in the particular case of imagination and active imagination.O filósofo escocês Alexander Gerard foi quase que inteiramente esquecido pela história da filosofia. Mas na época em que Kant estava desenvolvendo sua filosofia crítica, particularmente nos anos 1770, Gerard era um autor bastante popular, tendo seus principais escritos traduzidos inclusive para o alemão. Kant o menciona algumas vezes nas lições sobre antropologia, permitindo assim documentar sua leitura.     Reduced Blood RGS2 Expression in Mild Cognitive Impairment Patients

    © 2021 Milanesi, Cucos, Matias-Guiu, Piñol-Ripoll, Manda, Dobre and Cuadrado.Regulator of G protein signaling 2 (RGS2) is a gene involved in neuronal plasticity and synaptic signaling, whose expression in the brain is altered in neuropsychiatric and neurodegenerative disorders. Microarray data from large datasets suggested reduced RGS2 mRNA levels in the post-mortem brain tissue and blood of Alzheimer’s disease (AD) patients. The results were previously confirmed by quantitative real-time polymerase chain reaction (qRT-PCR) only ex vivo in lymphoblastoid cell lines derived from AD patients and controls. In this study, we compared RGS2 mRNA levels in peripheral blood samples from 69 mild cognitive impairment (MCI) patients to 50 age- and sex-matched non-cognitively impaired controls, out of which 25 patients were monitored at 1 year. We found that RGS2 was indeed downregulated in the peripheral blood of these patients (FR = −1.60, p < 0.001), and despite disease-specific therapy, RGS2 transcript levels continued to decrease at 1 year. The results suggest that RGS2 seems to be involved in AD pathology and progression and can be introduced in a panel of blood AD biomarkers

    Reconstructing embedded liberalism: John Gerard Ruggie and constructivist approaches to the study of the international trade regime

    In 1982, John Gerard Ruggie published a study of the postwar international trade and monetary regimes in which he introduced the concept of ‘embedded liberalism’. A large and growing number of international trade scholars are finding Ruggie’s concept of embedded liberalism an appealing one, and it now occupies a significant place on our conceptual horizon. In this article, the author returns to Ruggie’s original article to excavate lessons which are peculiarly relevant for current trade law scholarship. He argues that Ruggie’s account of embedded liberalism usefully serves to destabilize common assumptions about the objectives and normative underpinnings of the trade regime and thereby to expand our conceptions of what a liberal trade regime might plausibly look like. On the other hand, he explains why he does not share the enthusiasm of those who see in embedded liberalism an attractive normative vision to guide WTO reform. In addition, and most importantly, the author draws attention to the constructivist theoretical framework of Ruggie’s piece. He suggests that Ruggie’s article provides a useful introduction to the central elements of constructivist thinking about international institutions and shows how attention to constructivist insights has the potential to significantly enrich and expand our understanding of the trade regime and of trade law
