Il diritto alla salute e le esigenze di bilancio. Il caso della Regione Marche.
Get PDFL'oggetto di questo lavoro, mediante l’illustrazione di alcune norme che maggiormente hanno inciso sulle scelte di politica sanitaria, è quello di rilevare se alla luce della riduzione delle risorse disponibili e dell’introduzione dei Lea, siano garantite le condizioni di uniformità, eguaglianza, stabilità nella tutela del diritto alla salute. Tracciando l’evoluzione normativa che ha condotto all’attuale fisionomia del diritto alla salute e della sua tutela nell’assetto giuridico del sistema sanitario italiano, si delinea il percorso che ha condotto ad una profonda revisione del sistema sanitario nazionale. Il meccanismo dei costi standard diviene il protagonista della legislazione sul federalismo fiscale, quale criterio di riferimento, in luogo del costo storico, per il riparto delle risorse finanziarie tra le Regioni; lo strumento pattizio, d’altro canto, la modalità con cui lo Stato e le Regioni si assumono reciproci impegni di contenuto sia rispetto alle risorse statali destinabili al finanziamento della spesa sanitaria sia al recupero e al contenimento di eventuali scostamenti. In questo contesto viene illustrato il caso della Regione Marche che ha ridisegnato il suo sistema sanitario regionale, da un lato prevedendo un accentramento delle funzioni di controllo e di scelte politiche fondamentali e dall’altro attribuendo, a livello decentrato, attività di programmazione e gestione. Il sistema sanitario regionale è frutto di ripetuti interventi legislativi che ne hanno, nel tempo, definito l’assetto, senza dimenticare che spesso le scelte politiche e normative della Regione sono state dettate dall’imperativo di rispettare i requisiti delle norme nazionali. In questo quadro si inserisce una
ricostruzione del quadro normativo, costituito delle norme e dalle delibere più importanti ed incisive che hanno condotto alla realizzazione dell’odierno sistema sanitario regionale marchigiano.The object of this paper is to verify if the conditions of uniformity, equality and stability in the protection of the right to health are guaranteed by illustrating some of the rules that have most affected the health policy choices, in the light of the reduction of available resources and the introduction of Lea. By drawing the evolution of the legislation that led to the current role of the right to health and its protection in the Italian healthcare system's legal framework, the path leading to a thorough review of the national health system is outlined. The standard cost mechanism becomes the leading aspect of the legislation on fiscal federalism as a reference criterion, instead of the historical cost, for the allocation of financial resources between regions; the pactional instrument, on the other hand, the way in which the State and the Regions assume reciprocal commitments on the contents, both in terms of state resources for financing health spending and in recovering and containing any deviations. In this context, the case of the Marche Region has redesigned its regional health system, on the one hand, by focusing on central control functions and policy choices, on the other hand, by decentralizing the planning and management activities. The regional health system is the result of repeated legislative measures that have defined it along the time, also considering that, often, the policy and regulatory choices of the Region have been dictated by the imperative of complying with the national laws. In this framework, a reconstruction of the regulatory framework, made up of the most important and incisive legal rules and declarations, led to the implementation of today's regional Marche health system
Going Beyond Counting First Authors in Author Co-citation Analysis
Get PDFThe present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
Get PDF“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
Get PDFWe provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts

use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
