    Examining the diagnostic pathway for lung cancer patients in Wales using discrete event simulation

    Background: UK’s National Health Service (NHS) has one of the poorest lung cancer survival rates in Europe. To improve patient outcomes, a single cancer pathway was introduced in the NHS. In this study, a Discrete Event Simulation was developed to understand bottlenecks during lung cancer treatment.Methods: this study focused on the lung cancer diagnostic pathways at two Welsh hospitals. Discrete Event Simulation is a computer-based method that has been effectively used in demand and capacity planning. In this study, simulation models were developed for the current and proposed single cancer pathways. The validated models were used to provide Key Performance Indicators. Several “what-if” scenarios were considered for the current and proposed pathways.Results: under the current diagnostic pathway, the mean time to treatment for a surgery patient was 68 days at the Royal Glamorgan Hospital and 79 days at Prince Charles Hospital. For chemotherapy patients, the mean time to treatment was 52 days at the Royal Glamorgan Hospital and 57 days at Prince Charles Hospital. For radiotherapy patients, the mean time to treatment was 44 days at Royal Glamorgan Hospital and 54 days at Prince Charles Hospital. Ensuring that the patient attends their first outpatient appointment within 7 days and streamlining the diagnostic tests would have the potential to remove approximately 20 days from the current lung cancer pathway resulting in a 20-25% increase of patients receiving treatment within 62 days. Ensuring that patients begin their treatment within 21 days of diagnosis sees almost all patients comply with the 62-day target.Conclusions: discrete Event Simulation coupled with a detailed statistical analysis provides a useful decision support tool which can be used to examine the current and proposed lung cancer pathways in terms of time spent on the pathway

    Division of labour and sharing of knowledge for synchronous collaborative information retrieval

    Synchronous collaborative information retrieval (SCIR) is concerned with supporting two or more users who search together at the same time in order to satisfy a shared information need. SCIR systems represent a paradigmatic shift in the way we view information retrieval, moving from an individual to a group process and as such the development of novel IR techniques is needed to support this. In this article we present what we believe are two key concepts for the development of effective SCIR namely division of labour (DoL) and sharing of knowledge (SoK). Together these concepts enable coordinated SCIR such that redundancy across group members is reduced whilst enabling each group member to benefit from the discoveries of their collaborators. In this article we outline techniques from state-of-the-art SCIR systems which support these two concepts, primarily through the provision of awareness widgets. We then outline some of our own work into system-mediated techniques for division of labour and sharing of knowledge in SCIR. Finally we conclude with a discussion on some possible future trends for these two coordination techniques

The Single Regeneration Budget (SRB) Challenge Fund aims not only to achieve its substantive aim of local economic regeneration, but also to change the way local partners operate. Strategy is crucial to both of these objectives. This paper reviews the differing approaches taken to strategy, characterized by consultation, participation and generation of policy and project options. Based on interviews with 16 SRB partnerships and two government offices, it demonstrates that in the first round of SRB, partnerships adopted a range of strategy development approaches - a dynamic process which is not easily captured by the administrative structures used to implement the Challenge Fund.     The human body as a terrorist weapon: hunger strikes and suicide bombers

    This article argues that a major factor in terrorist acts is an appeal to the actor's own community at an emotional and symbolic level, through acts of sacrifice, particularly self-sacrifice. Although other aims also exist, a prime concern is to recall the actor's home audience to the struggle, because the actor regards himself as acting on their behalf. This utilizes the imagery and symbolism of traditional religion, implying a strong communal and non-material impetus to terrorist acts, rather than rational material calculation, that modern Western man finds difficult to comprehend. It also recalls much classical social theory, which emphasized the central role of religion in community. Self-sacrifice tells an emotional story to the actor's community that is comprehensible to them and will have an emotional appeal to maintaining the community. For the Northern Ireland hunger strikes (possibly analogous to suicide bombers) this is reflected in their appeal solely to a Catholic/nationalist community that equates strongly with ideas of a pre-modern society under threat from a modernizing society. All the hunger strikers were very normal for their community, but left non-Catholics completely unmoved. Consequently there is a need to understand the communal dynamics behind terrorism if one is to effectively counter the threat and that different societies may have different values regarding the individual, community, and life itself. Individual motivations do not provide an adequate explanation for much terrorism and it is a failure to grasp this that severely hinders much counterterrorism

    Quartz Archaeology Project

    The Quartz Archaeology Project is creating open-access datasets for quartz occurrences in a variety of archaeological and natural contexts to improve our understanding of the significance of quartz in the deep and recent past. This project will investigate the physical characteristics of quartz pieces at natural outcrops, archaeological sites, and locations of more recent industrial activities. It will also involve the creation of experimental assemblages of quartz fractured using different methods, such as knapping, mechanical crushing, and handheld tools. Analysis and comparison of the attributes of fractured quartz from natural, archaeological and experimental assemblages will generate information about the ‘signature’ of quartz pieces from these different contexts. The resulting datasets, excluding those derived from Aboriginal places, will be made freely available online. This will facilitate development of integrated and collaborative approaches to the identification and characterisation of quartz artefacts in archaeology and cultural heritage management.
