    Diseased, replaced or repaired mitral valve can lead to restricted blood flow to left ventricle and inadequate flow in left ventricular assist device (LVAD). A middle age woman with 'burnt out' hypertrophic cardiomyopathy had mitral valve repair for mitral regurgitation. She needed LVAD to support severe decompensating heart failure. Repaired mitral valve posed a risk of restricted flow through the device. Mitral commissurotomy was performed on beating heart through the left ventricular apical hole created for insertion of inflow cannula of LVAD

    Medical student competence in ophthalmology assessed using the Objective Standardized Clinical Examination

    Purpose: To assess pre-clerkship and clerkship medical student performance in an ophthalmology Objective Standardized Clinical Examination (OSCE) station. Methods: One hundred pre-clerkship medical students and 98 clerkship medical students were included in this study. The OSCE station consisted of a common ocular complaint – blurry vision with decreased visual acuity – and students were asked to take an appropriate history, provide two or three differential diagnoses to explain the symptoms, and perform a basic ophthalmic examination. Results: Generally, clerks performed better than pre-clerks in the history taking (P < 0.01) and ophthalmic examination (P < 0.05) sections, with few specific exceptions. In the history-taking section, more pre-clerkship students asked about patient age and past medical history (P < 0.00001) and for the ophthalmic examination, more pre-clerkship students performed the anterior segment examination (P < 0.01). Interestingly, more pre-clerkship students were also able to provide two or three differential diagnoses (P < 0.05), specifically diabetic retinopathy (P < 0.00001) and hypertensive retinopathy (P < 0.00001). Conclusion: The performance of both groups was generally satisfactory; however, many students in both groups had scores that were unsatisfactory. Notably, pre-clerks also outperformed clerks in certain areas, which emphasizes the importance of revisiting ophthalmology content through clerkship. Awareness of such knowledge can allow medical educators to incorporate focused programs into the curriculum

    Thesis (M.S.) University of Alaska Fairbanks, 2017Crude oil is used extensively around the world as a source of energy and as a means of producing various petroleum products. However, apart from being an excellent energy source, crude oil can also be a major pollutant in the form of oil spills. Crude oil spills can occur on land and in the ocean during the drilling, production, transportation or storage stages. While it is possible to reduce the damage caused to the environment by an oil spill, it is almost impossible to completely remove the adverse effects. New techniques need to be developed to clean-up oil spills at higher rates and with increased efficiency. The use of nanoparticles (NPs) for oil spill clean-up has gained popularity in recent years. This research focuses on the use of alginate gel as an immobilizing agent for nanoparticles that are then used for the removal of heating oil from an oil-water mixture. Iron Oxide and Graphene Oxide Nanoparticles were immobilized using sodium alginate in calcium chloride. The concentration of the nanoparticles was varied from 1 g/L to 5 g/L. The immobilized nanoparticles were then added to an oil-water mixture which was prepared by spiking heating oil in methanol and adding the solution to deionized water to achieve uniform distribution. 10 mL samples containing residual oil were extracted from the heating oil and water mixture at regular intervals and were analyzed for the residual oil. Measurements were carried out for residual hydrocarbons using Gas Chromatography/Mass Spectroscopy (GC/MS), Fluorescence Spectroscopy and Ultraviolet--visible spectroscopy (UV-Vis). Results from GC/MS show the highest percentage of oil being removed from the mixture by 2 g/L iron oxide (77%) and 2 g/L graphene oxide (81%) NPs. Analysis for residual hydrocarbons based on the time of contact showed promising results, with 75% of oil removed in 70 minutes. Further, based on the data obtained it was observed that the nanoparticles reached saturation after 70 minutes and were unable to remove additional quantities of oil from the mixture. Changing the nanoparticles from Iron Oxide to Graphene Oxide increased the amount of oil removed by 4%. This research will assist future development of oil spill clean-up and water treatment techniques that make use of nanoparticles as sorbents

    EXTERNAL PHOTOEVAPORATION OF THE SOLAR NEBULA: JUPITER's NOBLE GAS ENRICHMENTS

    abstract: We present a model explaining the elemental enrichments in Jupiter's atmosphere, particularly the noble gases Ar, Kr, and Xe. While He, Ne, and O are depleted, seven other elements show similar enrichments (~3 times solar, relative to H). Being volatile, Ar is difficult to fractionate from H[subscript 2]. We argue that external photoevaporation by far-ultraviolet (FUV) radiation from nearby massive stars removed H[subscript 2], He, and Ne from the solar nebula, but Ar and other species were retained because photoevaporation occurred at large heliocentric distances where temperatures were cold enough (lesssim 30 K) to trap them in amorphous water ice. As the solar nebula lost H, it became relatively and uniformly enriched in other species. Our model improves on the similar model of Guillot & Hueso. We recognize that cold temperatures alone do not trap volatiles; continuous water vapor production is also necessary. We demonstrate that FUV fluxes that photoevaporated the disk generated sufficient water vapor in regions [< over ~]30 K to trap gas-phase species in amorphous water ice in solar proportions. We find more efficient chemical fractionation in the outer disk: whereas the model of Guillot & Hueso predicts a factor of three enrichment when only <2% of the disk mass remains, we find the same enrichments when 30% of the disk mass remains. Finally, we predict the presence of ~0.1 M [subscript ⊕] of water vapor in the outer solar nebula and protoplanetary disks in H II regions.Copyright IOP Publishing.     Does CircuLite Synergy assist device as partial ventricular support have a place in modern management of advanced heart failure?

    The discrepancy between the number of patients on the waiting list and available donor hearts has led to the successful development of left ventricular assist devices (LVAD) as a bridge to transplantation. The conventional LVADs are designed to provide full hemodynamic support for the end-stage failing heart. However, full-support LVAD implantation requires major surgery, sternotomy and cardiopulmonary bypass in majority of cases. The Synergy Micro-pump is the smallest implantable LVAD and provides partial flow support up to 3 l/min. It was shown that early intervention with this device can provide substantial benefits to patients with severe heart failure not yet sick enough for a full-support LVAD. Due the small dimensions it can be implanted without cardiopulmonary bypass or a sternotomy. The purpose of this article is to review the clinical use of the Synergy Micro-pump as partial hemodynamic support.CircuLit
