Given the long follow-up periods that are often required for treatment or intervention studies, the potential to use surrogate markers to decrease the required follow-up time is a very attractive goal. However, previous studies have shown that using inadequate markers or making inappropriate assumptions about the relationship between the primary outcome and surrogate marker can lead to inaccurate conclusions regarding the treatment effect. Currently available methods for identifying and validating surrogate markers tend to rely on restrictive model assumptions and/or focus on uncensored outcomes. The ability to use such methods in practice when the primary outcome of interest is a time-to-event outcome is difficult because of censoring and missing surrogate information among those who experience the primary outcome before surrogate marker measurement. In this paper, we propose a novel definition of the proportion of treatment effect explained by surrogate information collected up to a specified time in the setting of a time-to-event primary outcome. Our proposed approach accommodates a setting where individuals may experience the primary outcome before the surrogate marker is measured. We propose a robust non-parametric procedure to estimate the defined quantity using censored data and use a perturbation-resampling procedure for variance estimation. Simulation studies demonstrate that the proposed procedures perform well in finite samples. We illustrate the proposed procedures by investigating two potential surrogate markers for diabetes using data from the Diabetes Prevention Program. Landmark Prediction of Survival
The importance of developing personalized risk prediction estimates has become increasingly evident in recent years. In general, patient populations may be heterogenous and represent a mixture of different unknown subtypes of disease. When the source of this heterogeneity and resulting subtypes of disease are unknown, accurate prediction of survival may be difficult. However, in certain disease settings the onset time of an observable intermediate event may be highly associated with these unknown subtypes of disease and thus may be useful in predicting long term survival. Throughout this dissertation, we examine an approach to incorporate intermediate event information for the prediction of long term survival: the landmark model. In Chapter 1, we use the landmark modeling framework to develop procedures to assess how a patient’s long term survival trajectory may change over time given good intermediate outcome indications along with prognosis based on baseline markers. We propose time-varying accuracy measures to quantify the predictive performance of landmark prediction rules for residual life and provide resampling-based procedures to make inference about such accuracy measures. We illustrate our proposed procedures using a breast cancer dataset. In Chapter 2, we aim to incorporate intermediate event time information for the prediction of survival. We propose a fully non-parametric procedure to incorporate intermediate event information when only a single baseline discrete covariate is available for prediction. When a continuous covariate or multiple covariates are available, we propose to incorporate intermediate event time information using a flexible varying coefficient model. To evaluate the performance of the resulting landmark prediction rule and quantify the information gained by using the intermediate event, we use robust non-parametric procedures. We illustrate these procedures using a dataset of post-dialysis patients with end-stage renal disease. In Chapter 3, we consider improving efficiency by incorporating intermediate event information in a randomized clinical trial setting. We propose a semi-nonparametric two-stage procedure to estimate survival by incorporating intermediate event information observed before the landmark time. In addition, we present a testing procedure using these resulting estimates to test for a difference in survival between two treatment groups. We illustrate these proposed procedures using an AIDS dataset
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Group Sequential Testing of a Treatment Effect Using a Surrogate Marker
The identification of surrogate markers is motivated by their potential to make decisions sooner about a treatment effect. However, few methods have been developed to actually use a surrogate marker to test for a treatment effect in a future study. Most existing methods consider combining surrogate marker and primary outcome information to test for a treatment effect, rely on fully parametric methods where strict parametric assumptions are made about the relationship between the surrogate and the outcome, and/or assume the surrogate marker is measured at only a single time point. Recent work has proposed a nonparametric test for a treatment effect using only surrogate marker information measured at a single time point by borrowing information learned from a prior study where both the surrogate and primary outcome were measured. In this paper, we utilize this nonparametric test and propose group sequential procedures that allow for early stopping of treatment effect testing in a setting where the surrogate marker is measured repeatedly over time. We derive the properties of the correlated surrogate-based nonparametric test statistics at multiple time points and compute stopping boundaries that allow for early stopping for a significant treatment effect, or for futility. We examine the performance of our testing procedure using a simulation study and illustrate the method using data from two distinct AIDS clinical trials
Robust Evaluation of Longitudinal Surrogate Markers with Censored Data
The development of statistical methods to evaluate surrogate markers is an
active area of research. In many clinical settings, the surrogate marker is not
simply a single measurement but is instead a longitudinal trajectory of
measurements over time, e.g., fasting plasma glucose measured every 6 months
for 3 years. In general, available methods developed for the single-surrogate
setting cannot accommodate a longitudinal surrogate marker. Furthermore, many
of the methods have not been developed for use with primary outcomes that are
time-to-event outcomes and/or subject to censoring. In this paper, we propose
robust methods to evaluate a longitudinal surrogate marker in a censored
time-to-event outcome setting. Specifically, we propose a method to define and
estimate the proportion of the treatment effect on a censored primary outcome
that is explained by the treatment effect on a longitudinal surrogate marker
measured up to time . We accommodate both potential censoring of the
primary outcome and of the surrogate marker. A simulation study demonstrates
good finite-sample performance of our proposed methods. We illustrate our
procedures by examining repeated measures of fasting plasma glucose, a
surrogate marker for diabetes diagnosis, using data from the Diabetes
Prevention Program (DPP)
Leveraging patient experience measures as surrogate outcomes to evaluate health care interventions
Patient experience quality measure scores are widely accepted as outcomes in health services research. For some patients and in some settings, such as hospice care, they can be the most important outcomes. While these measures are widely used, the potential to use them as surrogate outcomes in a clinical trial sense has gone under-recognized. The purpose of this commentary is to discuss the use of patient experience measures as potential surrogate outcomes in evaluating the effect of a health care intervention.
