    Late-Onset Capsular Block Syndrome with Pupillary Block Angle Closure after Cataract Surgery with Posterior Chamber Intraocular Lens Implantation: A Case Report

    Introduction: Pseudophakic pupillary block angle-closure glaucoma is an uncommon complication following uneventful cataract surgery with posterior chamber intraocular lens (IOL) implantation. Interestingly, capsular block syndrome (CBS) has been reported as another plausible cause of pseudophakic pupillary block angle-closure glaucoma, especially in the early postoperative period. Unlike early postoperative CBS, late postoperative CBS is not associated with a shallow anterior chamber, myopic shift, or elevated intraocular pressure. We report a case of late postoperative CBS presenting with an acute-onset pupillary block angle-closure attack occurring 13 years after uneventful cataract surgery with posterior chamber IOL implantation, which has not been reported in the literature. Case Presentation: An 87-year-old male diagnosed with pseudoexfoliation syndrome developed pseudophakic pupillary block following uneventful cataract surgery with posterior chamber IOL implantation. Late-onset CBS has been identified as the underlying cause of the pupillary block. The combination of zonular laxity observed in pseudoexfoliation syndrome and the presence of a Soemmering ring are potential predisposing factors for this condition. After performing laser peripheral iridotomy (LPI) followed by Nd: YAG capsulotomy, the pupillary block was resolved and vision was improved. Conclusion: CBS should be considered as a potential cause of pseudophakic pupillary block, even in the late postoperative period. The management of late-onset CBS accompanied by pupillary block angle-closure glaucoma typically includes LPI to eliminate the pupillary block, followed by Nd: YAG capsulotomy

    Comparison of Swept-Source Anterior Segment Ocular Coherence Tomography and Gonioscopy in Detecting Anterior Chamber Angle Closure

AS-OCT was undergone before any contact procedure. Intraocular pressure (IOP) measurements using applanation tonometry and gonioscopy were performed, respectively. AS-OCT images were blindly interpreted by 3 glaucoma specialists, involving both qualitative assessments at 0, 90, 180, and 270 degrees and quantitative analysis using iris-trabecular contact (ITC) index. The sensitivity and specificity of AS-OCT, compared with gonioscopy—the gold standard for identifying anterior chamber angle closure—were calculated.Results: The mean age was 61± 12.9 years, and females constituted 67.6%. Eyes classified by Van Herick’s technique as grade 0, 1 and 2 were 16%, 58.40%, and 25.60%, respectively. Closed-angle identified by gonioscopy and AS-OCT were 100 eyes (80%) and 102 eyes (81.60%), respectively. The sensitivity and specificity of AS-OCT in detecting angle-closure were 92.16% (95% CI, 87.44– 96.87) and 73.91% (66.22– 81.61), respectively. For angle-closure identified by ITC index ≥ 55%, sensitivity was 86.36% (77.53– 95.20) and specificity was 92.86 (86.23– 99.49). The inter-observer agreement of gonioscopy was moderate (Kappa = 0.55), while the intra-observer and inter-observer agreement of AS-OCT was substantial (Kappa = 0.71– 0.80 and Kappa = 0.69, respectively).Conclusion: AS-OCT enables a contactless qualitative and quantitative assessment of angle-closure in narrow-angle suspected eyes, demonstrating high sensitivity, acceptable specificity, and good inter-observer and intra-observer reliability.Keywords: angle-closure, anterior segment ocular coherence tomography, gonioscop

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    Prevalence, reasons, and factors associated with loss to follow-up in newly diagnosed glaucoma suspects and glaucoma patients in Thailand

    Abstract This study aimed to investigate the prevalence and reasons for discontinuation of follow-up among newly diagnosed glaucoma suspects and glaucoma patients. Patients newly diagnosed with glaucoma suspect, primary glaucoma, and secondary glaucoma within a 2-year period who had visited the ophthalmic outpatient department and had missed an appointment by at least 6 months were identified. These patients were traced via phone call, and reasons for discontinuing follow-up and knowledge about glaucoma were retrieved. We also identified factors associated with discontinuing treatment using univariable and multivariable regression analyses. During this period, 150 out of 2,742 patients were identified as lost to follow-up, representing a prevalence rate of 5.5%. The primary reason for participants discontinuing follow-up care was a lack of knowledge and understanding about glaucoma, reported by 61.7% of participants. The second most common reason was transportation issues, reported by 18.3%. Multivariable logistic regression analysis, higher income; OR = 0.06 (0.01;0.54), awareness of glaucoma; OR = 0.18 (0.08;0.42), higher knowledge of glaucoma; OR = 0.01 (0.00;0.25) were protective factors of discontinuation of treatment

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
