Electronic health record and produce information for health care in the SUS

Rio de Janeiro, RJ, Brasil.O presente trabalho tem como objetivo formular uma revisão crítica, sob perspectivas históricas, sobre a produção de informações voltadas à atenção à saúde no país ao longo dos últimos 40 anos. Acredita-se que tais perspectivas, sob tais enfoques, se justificam pela necessidade de constituição de um Registro Eletrônico de Saúde (RES) no âmbito do Sistema Único de Saúde que considere os obstáculos que se formaram no contexto do desenvolvimento histórico social do país nas últimas décadas. Para tanto, apresenta-se uma revisão bibliográfica crítica fundamentada em uma digressão histórica que se estende da década de 70, pela descrição do surgimento dos primeiros sistemas de informação em saúde e que se estende até os dias atuais, quando se inicia um processo de incorporação de padrões técnicos e tecnológicos internacionais visando à construção de um RES alinhado às resoluções da OMS e ao tema da eSaúde. O processo de construção de sua proposta suscitou uma série de questões que alteraram profundamente seu modelo. Coloca-se em evidência a evolução da forma de produzir informações, concernentes à atenção à saúde dos anos 70 até os dias atuais e a constatação de incompatibilidades de diversas ordens com um modelo de RES que atenda às necessidades atuais da sociedade brasileira. Em síntese, acreditamos que o sucesso do projeto RES-SUS Nacional depende da existência de sistemas de informação em saúde orientados a processos clínicos e de atenção à saúde, tais como os Prontuários Eletrônicos de Pacientes. A necessidade de um RES Nacional nunca foi tão evidente no país e percebe-se que os sistemas de informação recentemente desenvolvidos pelo Ministério da Saúde, tais como o e-SUS AB, e-SUS Hospitalar, SISPRENATAL e SISCAN tem sido desenvolvidos nesta perspectiva, o que viabilizaria a plena implantação de um Registro Eletrônico de Saúde no país. Conclui-se que há necessidade de definir caminhos e recursos que considerem elementos técnicos e políticos no seu desenvolvimento, admitindo o RES como elemento fundamental, a ser abordado na totalidade de uma estratégia de e-Saúde.The present work has the objective to make a critical review, in historical perspectives concerning the production of information aimed at health care in the country over the last 40 years. It is believed that such perspectives under these approaches are justified by the need to establish an Electronic Health Record (RES) under the Health System that considers the obstacles that formed in the context of the historical social development over the past decades. To this end, it is presented a critical literature review based on a historical tour that stretches from the 70s, by the description of emergence of the first systems of health information and that extends to the present day, when starting a process of incorporation international technical standards and technology aiming to build a RES aligned with WHO resolutions and the topic of eHealth. The process of building a proposal raised a number of issues that profoundly altered its model. It is put in evidence the evolution of the way to produce information concerning the health care from the 70s to the present day and the finding of incompatibilities of various orders with a RES model that meets the current needs of Brazilian society. In summary, it is believed that the success of RES-SUS National project depends on the existence of information systems in health oriented clinical processes and health care, such as Electronic Medical records of patients. The need for a National RES has never been so evident in this country and its noticed that the systems recently developed by the Ministry of Health, such as e-SUS AB, e-SUS Hospitalar, SISPRENATAL and SISCAN has been developed in this perspective, which would make viable the full implementation of an Electronic Health Record in this country. It is concluded that there is need to define pathways and resources to consider technical and political elements in its development, acknowledging the RES as a key element to be discussed in the totality of a strategy of e-Health
Sistema de informações hospitalares do SUS: potencialidades e limitações, análises de dados e propostas para o aprimoramento da informação hospitalar no Brasil
This doctoral thesis investigates the Hospital Information System (SIH) of Brazil’s
Unified Health System (SUS) through an integrated approach involving multiple
studies and diverse methodologies. It aims to identify the system’s strengths and
weaknesses, assess the applicability and consistency of its data, and explore
innovative solutions that can enhance its business model and informational
architecture in light of recent developments in digital health in Brazil and globally.
The study is based on the premise that, although the SIH plays a central role in
hospital management and the allocation of public resources, its technological
structure and business logic remain outdated, inherited from the former social
security system, which limits its ability to accurately represent the current reality of
hospital care in Brazil. The research is developed through five scientific articles and
one technical product. The first article conducts an integrative literature review,
identifying strengths and limitations that characterize the system and provide a
foundation for subsequent studies. The second article analyzes the applicability of
SIH data through a performance assessment of public and private hospitals
accredited by SUS. The third evaluates the consistency of SIH records by
comparing them with SIVEP-Gripe data on hospitalizations and deaths caused by
COVID-19. The fourth article presents the Minimum Health Data Set (CMD) as an
innovative solution under development by the Ministry of Health to reorganize
clinical data recording. The fifth article proposes a new architectural framework for
the SIH, encompassing data collection, information models, interoperability
standards, technological architecture, terminologies and classifications, and new
hospital reimbursement models. The thesis is complemented by a technical product
that outlines an informational modernization proposal for the SIH, aligned with the
National Health Data Network (RNDS). The original contributions of this thesis lie
particularly in the construction of a structured and feasible path to overcome the
identified limitations, represented by the proposition of practical solutions that can
improve the production of hospital care data in BrazilA tese investiga o Sistema de Informação Hospitalar (SIH) do SUS por meio de uma
abordagem integrada envolvendo múltiplos estudos e com diferentes métodos para
identificar suas potencialidades e limitações, analisar a aplicabilidade e
consistência de seus dados, bem como explorar soluções inovadoras que possam
aprimorar o seu modelo de negócio e as suas informações à luz dos avanços da
saúde digital no Brasil e no mundo. O estudo parte da premissa de que, embora o
SIH desempenhe papel central na gestão hospitalar e na alocação de recursos
públicos, sua estrutura tecnológica e lógica negocial permanecem anacrônicas,
herdadas do sistema previdenciário, o que compromete sua capacidade de refletir
adequadamente a realidade assistencial brasileira. A pesquisa se desenvolve a
partir de cinco artigos científicos e um produto técnico. O primeiro artigo realiza uma
revisão integrativa da literatura, identificando potencialidades e limitações que
caracterizem o sistema e embasam os estudos posteriores. O segundo artigo
analisa a aplicabilidade de suas informações por meu de um estudo sobre o
desempenho da rede hospitalar pública e privada credenciada ao SUS. O terceiro
verifica a consistência de seus registos frente ao SIVEP-Gripe ao comparar
registros de internações é óbitos por COVID-19. O quarto apresenta o Conjunto
Mínimo de Dados (CMD) como solução inovadora em desenvolvimento pelo
Ministério da Saúde para reorganizar o registro assistencial. O quinto artigo propõe
um novo framework arquitetural para o SIH, incluindo as dimensões de coleta de
dados, modelos de informação, padrões de interoperabilidade, arquitetura
tecnológica, classificações e terminologias, e novos modelos de reembolso
hospitalar. Complementa-se a tese com um produto técnico, o qual estrutura uma
proposta de modernização informacional do SIH, alinhada à Rede Nacional de
Dados em Saúde (RNDS). As contribuições inéditas da tese residem
especialmente na construção de um caminho estruturado e viável para superar as
limitações identificadas, representado pela proposição de soluções práticas que
possam qualificar a produção de informações da assistência hospitalar no Brasil
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
