    Serological profiles as prognostic clues for progressive systemic scleroderma: the Italian experience

    Ninety-one patients with progressive systemic sclerosis have been examined both clinically and serologically in order to have a better prognostic insight. Three main serological profiles have been isolated. The patients with anticentromere antibodies (ACA) represented one third of the cases, developed skin sclerosis rather later and rarely exhibited ankyloses and ulcerations. The esophagus was commonly involved while the lung, heart and kidneys were not. ACA-positive patients were not identified with the CREST syndrome, as the latter disclosed other profiles with the same frequency. Patients with anti-Scl-70 antibody represented one fourth of the cases and had the fastest progression, developing sclerosis in less than 5 years after the onset of Raynaud's phenomenon. Ankyloses and lung fibrosis, as well as joint, heart and kidney involvement, were found in most of them. Patients with anti-SSA/Ro antibodies were uncommon, but corresponded to a severe subset, having a fast progression and a constant involvement of the lung. Probably due to the rougher definition of their serology, patients with antinuclear, antispeckle-patterned and anti-Ku antibodies or without any detectable antibody could be defined less easily and corresponded to an intermediate position between ACA- and anti-Scl-70-positive patients.     Phytomenadione (vitamin K) is essential for liver biosynthesis of prothrombin and clotting factors VII, IX and X. Few cutaneous adverse reactions to vitamin K1 intramuscular administration have been reported in literature and almost all the cases were related to fat-soluble preparations. Two main clinical patterns have been described: a) Eczematous-urticarial pruritic plaques of each onset at the site(s) of injection or generalized, even after a single dose of vitamin K1 administration. These usually disappear in few weeks (2-4), leaving no signs, b) Erythematous-pigmented sclerodermatous plaques of late onset, that usually come out after high doses of vitamin K1 and take months or years to disappear, leaving pigmented and/or atrophic areas. Vitamin K1 is more commonly employed to treat hypothrombine- mic conditions resulting from chronic liver disease. This is the reason why, at first vitamin K1 adverse reactions were described only in patients with liver disease (alcoholic cirrhosis or chronic active hepatitis), while, subsequently), these reactions have also been reported in patients without liver compromission. Local vitamin K1 hypersensitivity probably has an immunological basis (type IV or cell-mediated immune response) that can be tested with patch and/or prick test. These tests often give rise to positive results for pure vitamin K1 or its oleous preparation, while they almost always yield negative results for individual components of vehicles or water soluble preparation of vitamin K3. We describe three new cases of skin hypersensitivity to vitamin K1 that developed in patients with liver disease, long time treated with fat-soluble vitamin K1: two cases had aczematous- urticarial plaques, only at the injection sites or generalized, the other had a sclerodermatous evolution

    We describe a woman who had elephantiasis of the vulva dating from one year. In addition multiple ulcerations and fistulae on vulva, perineal and perianal region were also present. Direct microscopic examination of superficial smears from ulcers was negative for Donovanan's bodies. Histological examination of biopsy specimens made the identification of clusters of blue staining organisms within the cytoplasm of large histiocytes possible. Vulvar elephantiasis is a rare sequela of chronic donovanosis. Its pathogenesis is related to the granulomatous inflammatory process and concurrent fibroproliferative reaction inducing occlusion of the lymphatic vessels and chronic lymphedema. Problems of clinical and histological differential diagnosis with other elephantiatic condition involving the genital region are discussed

    Dermoid cyst of the penis

    A dermoid cyst of the penis has not been documented previously in the literature. We report a young Caucasian who had an important swelling of the penis dating back several months. On examination a subcutaneous mass on the dorsal aspect of the shaft and prepuce was present. A cutaneous sinus discharging purulent material was also noted above the mass. Laboratory tests including serology for syphilis and bacteriologic examination were negative. An excisional biopsy revealed a nodular lesion in the subcutaneous tissue with tufts of hair emerging from the surface. Histopathologic examination disclosed granulation tissue containing fragments of hair surrounded by granulomatous infiltrate. Serial sections allowed identification of a subcutaneous cyst. The wall was epidermoid and connected with several pilosebaceous units. A sinus tract lined by stratified squamous epithelium contiguous with the epidermis was also noted. Clinical and histopathologic differential diagnoses are discussed. Evidence for a congenital origin of this condition is given

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
