Anticorps à domaine unique pour le traitement de l'hémophilie A
No full textL'hémophilie A est une maladie hémorragique rare liée au chromosome X, consécutive à l'absence de facteur VIII (FVIII) pro-coagulant. La prévention ou le traitement des hémorragies chez les patients atteints d'hémophilie A sont obtenus par l'injection de molécules procoagulantes dérivées du FVIII ou d'agents de dérivation. Cependant, les médicaments disponibles ont une demi-vie courte, sont immunogènes et/ou ne compensent pas entièrement le FVIII manquant. L'objectif principal de mon projet de thèse est de développer une nouvelle molécule de FVIII qui restaure entièrement l'activité procoagulante du FVIII, possède une longue demi-vie et est dépourvue d'immunogénicité. À cette fin, j'ai conçu un FVIII recombinant fusionné à des nanocorps (VHH) spécifiques du récepteur x des plaquettes : FVIII-VHH. Mon hypothèse est que le FVIII lié aux plaquettes devrait bénéficier de la demi-vie de 7 à 9 jours des plaquettes dans le sang. Au cours de ma thèse, j'ai sélectionné et caractérisé le VHH spécifique du récepteur plaquettaire x. J'ai également produit et caractérisé des protéines FVIII fusionnées au VHH, validé leur activité procoagulante spécifique et leur liaison aux plaquettes. Cependant, de manière inattendue, les molécules FVIII-VHH se sont avérées activer les plaquettes. Après une étude rétrospective, nous avons documenté une liaison de faible affinité des domaines C1 et C2 du FVIII au récepteur x des plaquettes. Cette interaction pourrait éventuellement induire une activation des plaquettes par réticulation des récepteurs. Suite à cette découverte, j'ai optimisé les fragments FVIII et VHH du FVIII-VHH pour réduire l'activation plaquettaire en introduisant des mutations dans les résidus C1 et C2 du FVIII, et augmenter l'affinité de liaison au récepteur plaquettaire x, respectivement. Après l'introduction des mutations C1 et C2, les constructions FVIII-VHH ont montré une activation plaquettaire plus faible par rapport au FVIII-VHH sans la mutation C1C2, mais l'activation plaquettaire est restée au-dessus du seuil in vitro. L'activation plaquettaire déclenchée par les protéines de fusion FVIII-VHH optimisées doit encore être validée dans un cadre plus physiologique in vivo. À cette fin, nous générons un nouveau modèle préclinique de souris déficientes pour le FVIII et transgéniques pour le récepteur plaquettaire humain x. Les travaux futurs confirmeront la demi-vie accrue du FVIII-VHH optimisé, valideront son efficacité hémostatique dans des modèles de saignement aigu et détermineront son immunogénicité.Hemophilia A is a rare X-linked hemorrhagic disorder consecutive to the lack of pro-coagulant factor VIII (FVIII). Prevention or treatment of bleeds in hemophilia A patients are achieved by the injection of FVIII-derived procoagulant molecules or bypassing agents. However, the available drugs have a short half-life, are immunogenic and/or do not fully compensate for the missing FVIII. The main objective of my PhD project is to develop novel FVIII molecule that fully restores the FVIII procoagulant activity, possess a long half-life and devoid of immunogenicity. To this end, I have designed recombinant FVIII fused to nanobodies (VHH) specific for the platelet receptor x: FVIII-VHH. My hypothesis is that platelet-bound FVIII should benefit from the 7-9-day long half-life of platelets in the blood. During my PhD, I have selected and characterized VHH specific for platelet receptor x. I have also produced and characterized VHH-fused FVIII proteins, validated their specific procoagulant activity and their binding to platelets. However, unexpectedly the FVIII-VHH molecules were found to activate platelets. Upon retrospective study, we documented a low affinity binding of the FVIII C1 and C2 domains to platelet receptor x. This interaction could possibly induce platelet activation by cross-linking of the receptors. Following this discovery, I optimized the FVIII and VHH moieties of FVIII-VHH to reduce platelet activation by introducing mutations in the FVIII C1 and C2 residues, and increase binding affinity to platelet receptor x, respectively. Following the introduction of C1 and C2 mutations, the FVIII-VHH constructs demonstrated lower platelet activation compared to FVIII-VHH without the C1C2 mutation, yet the platelet activation remained above threshold in vitro. The platelet activation triggered by the optimized FVIII-VHH fusion proteins remains to be validated in more physiological in vivo setting. To this end, we are generating a new preclinical model of mice deficient for FVIII and transgenic for human platelet receptor x. Future work will confirm the increased half-life of optimized FVIII-VHH, validate its hemostatic efficiency in acute bleeding models and determine its immunogenicity

Evaluation de l'analyseur Multiplate® dans le diagnostic et la caractérisation de la maladie de Willebrand et de certaines thrombopathies
No full textL objectif de notre travail a été d étudier les performances du MultiplateÒ, agrégomètre par impédance de dernière génération, dans la détection des anomalies constitutionnelles de l hémostase primaire chez 30 patients atteints d une maladie de Willebrand définie par une activité cofacteur de la ristocétine < 40%. Nous avons comparé les résultats obtenus par le MultiplateÒ avec ceux obtenus par agrégométrie optique classique. L'étude a également inclus 30 volontaires bien portants. D autre part, nous avons également étudié 4 patients atteints d une thrombopathie constitutionnelle (2 thrombasthénies de Glanzmann et 2 syndromes de Bernard et Soulier). En conclusion, nos résultats suggèrent que le MultiplateÒ a une sensibilité équivalente à celle de l agrégométrie optique pour le diagnostic de la maladie de Willebrand, met en évidence les maladies de Willebrand de type 2B ainsi que les thrombopathies constitutionnelles. Ces résultats préliminaires encourageants doivent être confirmés sur une plus large population de patients atteints de maladie de Willebrand et de thrombopathies.CHATENAY M.-PARIS 11-BU Pharma. China and the world economy: a European perspective

No full textLe texte lauréat du Prix RFI Théâtre (2019) Victoria K, Delphine Seyrig et moi ou La petite chaise jaune (2021) de Valérie Cachard s’inscrit dans la mouvance de la « littérature hors du livre » (Rosenthal et Ruffel, 2010 : 4), où l’auteur·trice légitime la réception de son texte à travers une performance narrative. Après avoir interrogé la nature et le bien-fondé des archives intimes de Victoria K, lettres extraites de son journal intime abandonné dans sa maison, l’article s’intéresse aux allers-retours entre la fiction et le réel fictionnel qui systématisent une mémoire solidaire de la guerre civile en apparence fragmentaire. La performance du texte à Beyrouth par la dramaturge et son acolyte artistique Hadi Deaibes tend à éroder les dimensions sociopolitiques de l’archivage au profit d’une hybridation des identités scéniques. Cette contribution, coécrite par Noha Nemer et son amie de longue date Valérie Cachard, propose une réflexion sur l’expropriation discursive, esthétique et scénique d’une littérature intimiste.The award-winning text of the RFI Theater Prize (2019), Victoria K, Delphine Seyrig et moi ou La petite chaise jaune (2021) by Valérie Cachard, falls within the trend of “literature beyond the book” (“littérature hors du livre”; Rosenthal and Ruffel, 2010: 4), where the author legitimizes the reception of their text through a narrative performance. After questioning the nature and validity of the intimate archives of Victoria K – letters taken from her diary left behind in her house – the article focuses on the back-and-forth between fiction and fictional reality, which creates a cohesive memory of the seemingly fragmented civil war. The text’s performance in Beirut by the playwright and her artistic collaborator Hadi Deaibes tends to erode the sociopolitical dimensions of archiving in favor of a hybridization of stage identities. This contribution, co-written by Noha Nemer and her long-time friend Valérie Cachard, offers a reflection on the discursive, aesthetic, and scenic expropriation of intimate literature
