Abstract: INTRODUCTION: End-stage renal disease patients on hemodialysis (HD) often present osteoarticular and musculoskeletal complications related both to chronic renal disease as well as to chronic dialysis. The aim of the present study was to assess the degree of upper-limb disability, and its determinants, in a group of patients on chronic HD to determine functional capacity and the need for rehabilitative treatment.
MATERIALS AND METHODS: We studied 97 HD patients (46 male, 51 female) with a mean age of 64.4 years, and 51 healthy controls. The Disabilities of the Arm, Shoulder, and Hand (DASH) Outcome Measure was used to clinically assess degree of disability as well as finger function and movement.
RESULTS: For study purposes, patients were divided into 3 groups according to the duration of dialysis: 10 years. The median DASH score was 9.5 (interquartile range [IQR]: 3.4-24.1) in the entire population. However, it differed significantly in the 3 dialysis groups with scores of 5.2 (IQR: 2.6-18.1), 14.7 (3.4-23.7), and 37.5 (9.5-73.3) in the 10 year groups, respectively (p < .001). Additionally, disability correlated with shoulder and hand pain and with finger stiffness. Moreover, patients on dialysis for more than 10 years were affected by pain involving several upper limb joints. All patients who worked had a DASH score lower than 20. The median DASH score was 2.6 (IQR: 0.9-5.2) in the control group.
CONCLUSION: Duration of dialysis is associated with progressive disability. Thus, early rehabilitation programs are recommended in patients who are expected to undergo long-term dialysi

Il Project Management Interculturale offre modelli e strumenti operativi che permettono alle aziende di strutturare e migliorare la gestione di progetti in cui operano individui o gruppi appartenenti a culture diverse.
Studi recenti mostrano che modalità di agire e comportamenti manageriali sono fortemente influenzati dalla cultura nazionale dei singoli manager. Gli autori ritengono che un approccio flessibile alla gestione dei progetti debba essere capace di adattare e plasmare strumenti, processi e “practice” in quella che chiamano “Modulazione culturale nel Project Management”: la dimensione interculturale può quindi dimostrarsi capace di integrare e potenziare anche gli strumenti e i processi più avanzati di project ma- nagement.
Il risultato è un nuovo approccio, realmente in grado di adattarsi in modo da massimizzare la capacità di raggiungere i risultati voluti.

Harrington et al reported on the updated results of the phase III KEYNOTE-048 trial assessing pembrolizumab, an anti-PD-1 monoclonal antibody, with or without chemotherapy in recurrent/metastatic (RM) head and neck squamous cell carcinoma (HNSCC). A total of 882 recurrent/metastatic HNSCC patients were randomized 1:1:1 to pembrolizumab monotherapy (PM) (n = 301), pembrolizumab-chemotherapy (PC) (n = 281) or cetuximab-chemotherapy (n = 300).

The subgroup analysis data suggests that there could be a population of patients achieving greater benefit from ICI in RM-HNSCC, thus giving a boost to clinical and translational research

Surface plasmon resonance based on molecularly imprinted nanoparticles for the picomolar detection of the iron regulating hormone Hepcidin-25
Objective: the study aimed at developing molecularly imprinted polymer nanoparticles (MIP NPs) for the recognition of Hepcidin-25 and at their further integration into an optical sensor based on surface plasmon resonance (SPR) suitable in the real time monitoring of the hormone. Materials and methods: MIP technique is a powerful mean to produce tailor made synthetic recognition sites. Here precipitation polymerization was exploited to produce a library of MIP NPs targeting the N terminus of the hormone Hepcidin-25. Biotinylated MIP NPs were immobilized onto a NeutrAvidinTM SPR sensor chip. The response of the MIP NP sensor to Hepcidin-25 was studied. Results: morphological analysis showed MIP NPs of 20-50 nm; MIP NPs exhibited high affinity and selectivity for the target analyte: low nanomolar Kds for the interaction with Hepcidin-25, but no interaction with the non-regulative Hepcidin-20. MIP NPs were integrated as recognition element in the SPR allowing the detection of Hepcidin-25 in 3 minutes. Linearity was observed with the logarithm of Hepcidin-25 concentration in the range 7.2-720 pM. The LOD was 5 pM. The response for Hepcidin-20 was limited. Hepcidin-25 determination in real serum samples spiked with known analyte concentrations was also attempted. Conclusion: the integration of MIP NPs to SPR allowed the determination of Hepcidin-25 at picomolar concentrations in short times outperforming the actual state of art. Optimization is still needed for real sample measurements in view of future clinical applications
Activities on PNS neural interfaces for the control of hand prostheses
The development of interfaces linking the human nervous system with artificial devices is an important area of research. Several groups are working on the development of devices able to restore sensory-motor function in subjects affected by neurological disorders, injuries or amputations. Neural electrodes implanted in peripheral nervous system, and in particular intrafascicular electrodes, seem to be a promising approach for the control of hand prosthesis thanks to the possibility to selectively access motor and sensory fibers for decoding motor commands and delivering sensory feedback. In this paper, activities on the use of PNS interfaces for the control of hand prosthesis are presented. In particular, the design and feasibility study of a self-opening neural interface is presented together with the decoding of ENG signals in one amputee to control a dexterous hand prosthesis
