Histopathologic features of erythematous papulopustular eruption to epidermal growth factor receptor inhibitors in cancer patients
BACKGROUND: Erythematous papulopustular eruption (EPPE) is the most frequent skin adverse event to epidermal growth factor receptor (EGFR) inhibitors but its histopathologic features have been poorly studied. As EPPE is a strong predictor of patient's treatment response, the EPPE histopathologic features and their correlations with skin eruption severity and involved drug were investigated.
METHOD: An involved skin biopsy was carried out in 39 informed patients treated with EGFR inhibitors (mainly cetuximab and erlotinib). The cutaneous changes in hematoxylin-eosin stained sections were evaluated.
RESULTS: The EPPE to EGFR inhibitors is histopathologically characterized by neutrophilic subcorneal or intraepidermal pustules and polymorphous infiltrate of the superficial dermis during the earliest phases and by lymphocytic perifolliculitis and/or suppurative folliculitis at a later phase. The widespread dermis inflammation was more frequent in severe EPPEs and in patients treated with cetuximab, while the hair follicle inflammation was observed only in mild/moderate EPPEs.
CONCLUSION: Our study shows the histopathologic signs of EPPE and their correlation with clinical severity and the offending drug

Drug- and Virus- or Bacteria-induced Exanthems
BACKGROUND: The differential clinical diagnosis between drug-induced exanthema (DIE) and virus- or bacteria-induced exanthema (VBIE) is frequently not easy because the serologic analysis for virus and bacteria and skin tests are not always exhaustive. In these cases, only the oral challenge test is nullifying.
OBJECTIVES: This study wants to identify 1 or more structural changes and/or cytokine markers that might be helpful in discriminating the etiology and the possible correlation with the clinical features, type of the involved drug, blood and skin eosinophilia, and time of skin biopsy.
METHODS: Involved non-sun-exposed skin biopsy specimens were obtained from 36 patients with DIE and 30 patients with VBIE. Blood investigations, skin tests, and oral rechallenge tests were carried out in all subjects. The histopathologic features and the immunohistochemical expression of a cytokine panel [fatty acid synthase-ligand, granzyme B, interleukin (IL) 2, IL-4, IL-5, IL-10, IL-13, interferon γ, perforin, tumor necrosis factor α] were analyzed.
CONCLUSIONS: Finally, DIE and VBIE have distinct skin cytokine profile (IL-5 alone or in combination with granzyme B and perforin in DIEs was statistically more frequent than in VBIEs, mainly when skin biopsy was carried out within 2 days from clinical onset), which might be helpful in discriminating the etiology

Background: Erythematous and papulo-pustular eruption (EPPE) is the most common cutaneous adverse reaction induced by Epidermal Growth Factor Receptor inhibitors (EGFRI). This rash greatly worsens the quality of life of patients. Tolllike receptors (TLRs) are a family of receptors involved in immune responses which may present genetic Single Nucleotide Polymorphisms (SNPs) able to alter the gene functionality. A role of some TLRs in the pathogenesis of inflammatory diseases and cancers was recently suggested. It has been reported that TLR9 presents a mutation in its C-1237T SNP which seems to be correlated with several autoimmune diseases, such as atopic dermatitis and asthma. Furthermore, it is possible that some synthetic agonists of TLR9, called CpG-oligodeoxynucleotides, are able to stimulate the antitumoral innate immune response. Objectives: to assess the incidence of TLR9 C-1237T SNP in patients with EPPE induced by EGFRI and the correlations of genotypic variations of TLR9 C-1237T SNP with gender, EPPE and tumor site. Materials and methods: blood sampling for DNA extraction and sequencing to search the TLR9 C-1237T SNP mutation were carried out in 26 patients with EPPE induced by erlotinib in 14, cetuximab in 11, and tovok in 1. The eruption severity was quantified using EGFRISTI (Epidermal Growth Factor Receptor Inhibitor-related Skin Toxicity Index). The rash was considered mild, moderate, or severe if EGFRISTI value was 40, respectively. Results: TLR9 C-1237T SNP mutation was found in 30.8% of patients. This percentage is higher than that reported in the literature for Caucasian population (12%). The presence of mutation was correlated with the eruption severity (0% in mild, 26.7% in moderate, 57.1% in severe) and the cancer site (7/15 lung cancers, 1/9 colon/rectum cancers, 0/2 head/neck cancers). Conclusions: the data, if confirmed in a wider number of cases, could allow the screening of patients to be treated with EGFRI because the most serious EPPEs are indicative of better response to cancer treatmen
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
