Transient stenosing ureteritis in childhood. A rare complication of rheumatoid purpura
A case of transient stenosing ureteritis in a boy presenting with the clinical features of Henoch-Schoenlein purpura (diffuse articular swelling, petechiae, abdominal pain) complicated by macroscopic haematuria with blood clots is reported. On urography the right collecting system and the right ureter up to the distal lumbar portion were dilated owing to tight ureteral stenosis at that level. Above the stricture, numerous ring-like functional ureteral wall defects were detectable. On the left a stenosis was evident at the lumbo-sacral level with mild dilatation of the ureter. A small filling defect due to subephitelial haemorrhage was evident in the upper part of this ureter. Three months later, after prednisone therapy, the urography was normal on the right: residual mild stenosis was still evident on the left. Radiological findings in Henoch-Schoenlein purpura and the possible differential diagnosis of this condition are discussed
Atraumatic patellofemoral joint disorders. Long-term results of surgery.
Clinical and radiographic follow-up was done on a group of patients with atraumatic patellofemoral joint disorders who had been treated surgically by realignment of the extensor apparatus at the authors' institution from 1985 to 1989. The preoperative radiographic examination in the 65 patients treated (for a total of 74 realignments) evaluated the following parameters: sulcus angle, Insall-Salvati index, and congruence angle. The 46 patients who returned for follow-up (after an average of 2 years and 3 months) were evaluated clinically according to a classification system proposed by the authors which is based on objective clinical data and a multi-factorial assessment of pain. Moreover, radiographic follow-up of the same 46 patients made it possible to quantify the degree of realignment achieved with each type of surgical technique. The clinical and radiographic data made it possible to determine more precise indications for each type of surgical procedure in relation to the disease involved, using a correct diagnostic approach and making a thorough evaluation of the clinical, anatomical and pathological features
[Shoulder instability and pain. Computerized arthro-tomography assessment].
The growing interest in shoulder joint imaging comes from the ever-increasing demand for such an examination in orthopedics. Since more complete and detailed imaging of bone, capsuloligamentous and musculotendinous compartments is always needed, CT arthrography has necessarily become a widely used method. In this study, 282 patients were investigated with CT arthrography. Seventy per cent of them had anatomical instability, 17.3\% had functional instability and the extant 12.7\% had shoulder pain. In traumatic instability, the most often injured structures were the glenoid labra (91\%) and the capsule (82\%). Lesions in the two structures were nearly always associated. Moreover, high incidence (65.8\%) of Hill-Sachs lesions of the humeral head was observed. In atraumatic instability, abnormal anteversion of the scapular glenoid was always detected. In the patients with shoulder pain, the most common causes were the impingement syndrome (30.5\%), superior labrum lesions (16.7\%), adhesive capsulitis (16.7\%) and synovial osteochondromatosis (13.7\%). With CT arthrography, labial abnormalities (detachments, tears, amputations, eversions and degeneration) can be identified, as well as capsular lesions (insertional detachment and laxity), rotator cuff conditions (bursitis, tendinitis and partial/complete tears), biceps tendon abnormalities and glenoid rim and humeral head fractures. Moreover, CT arthrography is minimally invasive and well tolerated. It exhibits 97.7\% specificity, 91\% sensitivity and 96\% accuracy. Furthermore, it has been proven to be extremely useful in treatment and surgical planning

Extensive endobronchial calcification contributing to hypoxia in a quadriplegic patient
We report a rare case of a quadriplegic patient in whom the entire bronchial tree of the lower right lobe was completely occluded by calcium deposit.
A 30-yr-old patient presented with hyperthermia, dyspnea, and severe hypoxia. He had been quadriplegic following spinal trauma to C4–6 nine years previously, experienced recurrent pulmonary infections and episodes of hemoptysis, and still needed a tracheostomy to remove bronchial secretions. Mechanical ventilation and antibiotics were started (levofloxacin at first empirically, later substituted for with linezolid after isolation of methicillin-resistant Staphylococcus aureus). Thoracic CT scan showed widespread consolidation in the lower right lobe (Fig. 1), and an extensive, occlusive calcification of the bronchial tree (Figs. 1 and 2), whereas arterial perfusion of the lobe was maintained (Fig. 2), resulting in severe but localized perfusion/ventilation mismatch contributing to hypoxia. In fact, despite resolution of pneumonia, gas exchanges were severely impaired (Pao2/Fio2 150). Resection of the right lower lobe resulted in an improvement of oxygenation (Pao2/Fio2 250–310) and separation from the ventilator. Three years later the patient is still breathing autonomously in a stable general condition
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
