    Effects of tramadol on synovial fluid concentrations of substance P and interleukin-6 in patients with knee ostearthritis<. comparison with paracetamol

    Both the analgesic drugs tramadol and paracetamol are widely used for the symptomatic therapy of osteoarthritis (OA). The aim of this double-blind, randomised study in patients with knee OA was to compare their effects on synovial fluid concentrations of interleukin (IL)-6 and substance P (SP). Moreover, we evaluated plasma and synovial fluid concentrations of tramadol and its active metabolite (O-desmethyl-tramadol, M1) after oral treatment with this drug. Twenty patients were enrolled. A group of 10 patients received tramadol (50 mg three times a day), and another group of 10 patients were treated with paracetamol (500 mg three times a day) for 7 days. Both drugs significantly reduced the intensity of joint pain. The synovial fluid concentrations of SP were significantly reduced only by the treatment with tramadol. In this group of patients, IL-6 synovial fluid concentrations were slightly, but not significantly, decreased. Paracetamol did not significantly change the synovial fluid concentrations of SP and IL-6. After oral administration, a considerable amount of tramadol was measurable in synovial fluid. Both in plasma and synovial fluid the concentrations of M1 were markedly lower than those of tramadol, with a T/M1 ratio of 14.7±4.6 and 9.3±3.9, respectively. These data demonstrate that the activity of tramadol may involve the modulation of inflammatory mediators. Moreover, they indicate that after oral treatment with tramadol, both the parent drug and its active metabolite can penetrate into synovial fluid

    Plasma and synovial fluid concentrations of nimesulide and its main metabolite after a single or repeated oral administration in patients with knee osteoarthritis

    The aim of this study was to evaluate plasma and synovial fluid concentrations of the non-steroidal anti-inflammatory drug nimesulide and its major metabolite (hydroxynimesulide, M1), after a single 100 mg dose of nimesulide and a repeated (14 day) administration, 100 mg twice a day, in patients with osteoarthritis of the knee and joint effusion. Nimesulide was rapidly absorbed in plasma and distributed in synovial fluid. On day 1, effective concentrations were present 30 min after the first dose and on day 14, the synovial fluid concentration of nimesulide was significantly higher than that measured on day 1; no accumulation was observed in plasma. After 14 days of treatment, both the plasma and synovial fluid concentrations of M1 were significantly higher than those measured on day 1. These data may help to explain the rapid onset of the analgesic effect of nimesulide demonstrated in several clinical conditions, including painful osteoarthritis

Estos modelos para datos correlacionados han sido exitosamente utilizados ya que permiten contemplar información de la estructura poblacional y parentesco subyacente entre las unidades de análisis. El mapeo asociativo en especies vegetales pretende reconocer QTLs (de su nombre en inglés Quantitative Trait Loci) que codifican para variables de interés. Las pruebas de hipótesis realizadas gen-por-gen, o marcador-por-marcador, son múltiples y tienden a estar altamente correlacionadas cuando existe estructura genética de población, por lo que es necesario identificar una corrección apropiada para los valores p usados para declarar la significancia de la asociación. La corrección por multiplicidad propuesta por Bonferroni, la tasa de descubrimiento de falsos positivos y la estimación del número efectivo de pruebas independientes propuesto por Li y Ji (2005) son herramientas usadas para la corrección de los valores-p en el contexto del análisis de QTL clásico, donde los individuos se suponen igualmente emparentados. El objetivo de este trabajo es evaluar una nueva propuesta de corrección de valores p para el contexto de MA, que toma la idea del número efectivo de pruebas independientes pero éste es deducido luego de ajustar la estructura genética subyacente en las líneas de mapeo bajo diferentes modelos lineales mixtos para datos genéticamente correlaconados.Fil: Peña Malavera, Andrea Natalia.     Hantaviruses in agricultural and forestry workers: Knowledge, attitudes and practices in Italian physicians

    Hantaviruses are viral pathogens usually endemic in rodent populations. Human exposure follows inhalation of dusts contaminated with rodent excreta, and most individuals have been in-fected in occupational settings heavily contaminated with rodent droppings, such as agricultural and forestry. To date, knowledge, attitudes and practices of medical professionals, especially occupational physicians (OP), regarding hantavirus disease in at-risk workers have been scarcely investigated. We investigated these topics through a structured questionnaire administered through an online survey of 223 medical professionals (42.2% of them working as OP). Adequate general knowledge of hantavirus disease was found in 48.9% of respondents, with OP exhibiting a better understanding of clinical features of human hantavirus infections. OP aware of the endemic status of hantavirus in North-Eastern Italy exhibited higher risk perception for agricultural workers (odds ratio 21,193, 95% confidence interval 3.666–122.505). On the contrary, a better knowledge of hantaviruses was association with acknowledging an increased risk of hantavirus infection in forestry workers (odds ratio 5.880, 95% confidence interval 1.620–21.343). Hantavirus in Italy represent an often-overlooked biological risk in occupational settings. The lack of preventive immunization, the inappropriate risk perception and the unsatisfying awareness of hantavirus issues collectively stress the importance of appropriate information campaigns among health care providers
