    Radial head fractures: review of current evidence about assessment, classification and management

    The radial head fractures are frequent elbow injuries with several fracture patterns and associated lesions. The literature highlights the importance of correct patient management right from the earliest stages to achieve the best results and to avoid complications. The purpose of this article was to provide an overview of current concepts of the management of radial head fractures. The main goal of each treatment should be to restore elbow biomechanics, kinematics and stability to achieve a complete range of motion and to early begin rehabilitation programs. For Mason type 1 fractures the nonoperative management is the best choice providing good or excellent results in most patients. There is less agreement regarding Mason type 2 fractures, in term of nonoperative or surgical treatment. When surgical treatment is chosen, open reduction and internal fixation seems to guarantee the best clinical outcomes with excellent results reported. The Mason type 3 or 4 fractures are managed with surgical treatment. However, the best method of treatment remains controversial in literature between open reduction and internal fixation, radial head arthroplasty and radial head excision. In the last years, arthroscopy is going to assume an important role in the management of elbow diseases, not only as diagnostic tool but also as surgical solution and probably in the future its role will become more and more relevant. Further studies with long-term follow-up are needed to determine the best form of treatment of these complex injuries.     The role of education in psychological response to adverse health shocks

    The prevalence of common mental disorders is on the rise: in the last decade mental disorders have become one of the major contributors to the global burden of disease and the leading cause of disability worldwide. While the association between depressive symptoms and physical health has been the subject of many studies, little is known about the potential pathways through which physical health affects mental health and how this relationship varies across different socioeconomic groups. This study aims at investigating on the role that a higher educational level may have not only in protecting people from depressive and anxiety symptoms but also on its role in mediating the relationship between mental and physical health shocks. For the scope of our analysis, we relied on hospital administrative records collected from Liguria, a north-western Italian region. We evaluate the impact of education in protecting individuals from mental disorders when they experienced an adverse health event, such as severe hospitalization or an onset of a chronic condition. Our results suggest that among those who suffer from severe physical health issues, highly educated individuals are less likely to experience depressive and anxiety symptoms compared to those with a lower level of education, even though, in presence of an adverse health shock, the protecting role of education slightly decreases

    Le analisi archeobotaniche integrate permettono di ricostruire il paesaggio vegetale naturale e culturale sia a breve che ad ampio raggio intorno al sito in esame, dando informazioni su flora e biodiversità pregressa, sul quadro ecologico e sull’impatto antropico nell’area investigata. In questo contributo vengono presentati i dati ottenuti per il sito di Novi Sad da una campionatura di saggio che si sviluppa nell’arco cronologico che va dal IV–V a.C. al XI-XII sec. d.C., ma che fa perno soprattutto sul periodo romano fino al tardo antico. I risultati raggiunti incoraggiano l’allargamento della ricerca all’intera campionatura, anche alla luce della complessità del sito

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Older patients and geographic barriers to pharmacy access: When nonadherence translates to an increased use of other components of health care

    This paper studies whether geographic barriers can influence nonadherence to prescription drugs and its effect on patients' health. We used a multivariate probit model estimated by maximum simulated likelihood that considers individual unobserved heterogeneity, which may characterize the relationship between adherence, medical care utilization, and health outcome. We used administrative data from Liguria, Italy, the region with the highest rate of individuals over the age of 65 in Europe. Our sample included older individuals affected by cardiovascular diseases, which remain one of the leading causes of death in most OECD countries. Our results showed that geographic barriers to pharmacies negatively influence patients' adherence. According to our results, patients' nonadherence to pharmacological therapy is responsible for an increased probability of patients' mortality and the overuse of other medical services, namely, hospitalizations and emergency department visits. Nonadherence may thus represent a potential source of waste for the health care system
