Severity of Chronic Idiopathic Urticaria in RSUP Dr. Moh. Hoesin Palembang
There were differences in clinical finding among authors about autologous serum skin test/ASST result in chronic idiopathic urtikaria/CIU. Some research reported there was no significant differences in clinical features of CIU, whereas other research found significant differences in duration and itch of CIU patients. The prevalence of urticaria in General Hospital Dr. Moh. Hoesin Palembang showed increased tendency but there was no data about CIU and the severity of clinical features. The objectives of this study were to find out the significant correlation between ASST and severity of clinical features of CIU, and compared the difference of clinical features of CIU with and without positive ASST. This study was an observational analytic study, with cross sectional design. Fifty four subjects included in this study after following selection. Ethical approval was obtained from Bioethics and Humanities Unit, Faculty of Medicine Sriwijaya University and Dr. Moh. Hoesin Hospital Palembang. The clinical feature of 54 patients with CIU were evaluated. The data of physical examination and the anamnesis about clinical scores collected and analyzed using SPSS software and Spierman correlation was used to analyze the correlation between clinical severity of CIU and ASST result. Student's t-test was used to analyze the differences in clinical severity of CIU with and without positive ASST. There was significant correlation between the severity clinical feature of CIU and ASST result (p=0,00; R=0,598). Patients with positif ASST results (46,3%) had more duration of illness (0,00), higher pruritus score (0,00), and angiodema score (0,024). The CIU patients with severity clinical score of 27 (cut-off point), 11 times more likely to develop autoimmune urticaria than the patients with cut-off below 27 (p=0,00)
Terapi Laser untuk Skar Hipertrofik dan Keloid
This study aim to evaluate the success rate of laser therapy in hypertorphic, and keloid scar patients based on scar characteristics. The research method used is systematic review using data from Pubmed, Cochrane Library, Clinicaltrials.gov, and Mendeley from 1994 to July 2022. The results found a total of 28 studies, consisting of 3 retrospective studies, 5 randomized controlled trials (RCT), and 20 prospective studies included in the review. This research evaluated 11 585nm pulse dye laser (PDL) studies, 2 595nm PDL studies, 3 1064 nm Nd:Yag studies, 3 1550nm Er:Yag studies, 3 2940nm Er:Yag studies, and 6 10,600nm fractional CO2 laser studies. In conclusion, the 585 nm pulse dye laser showed the best overall therapeutic success rate. Fractional CO2 10,600nm, Er:YAG 2940nm, and Nd:YAG 1064nm lasers showed the best therapeutic response for reducing erythema, pigmentation, height, and scar consistency. Further research is needed to assess the success rate of therapy.

The efficacy of autologous serum therapy in chronic spontaneous urticaria: A systematic literature review and meta-analysis
Chronic spontaneous urticaria (CSU) is an autoimmune disease characterised by urtica lesions and/or angioedema accompanying an itching sensation, recurring for at least six weeks without any specific trigger. Autologous serum therapy (AST) is an adjuvant therapy for CSU that is resistant to H1 antihistamines. This therapy is an economical option in developing countries. There were a few studies discussing the role of AST in CSU. This systematic review and meta-analysis were conducted to evaluate the efficacy of AST based on urticaria activity scores (UAS or UAS7) and urticaria total severity scores (TSS) so that clinicians can consider them. Data were searched systematically in Cochrane, PubMed, Google Scholar, Willey, and EMBASE from 2000 to March 2023. Data analysis using Excel 2010 (Microsoft Corp) and MedCalc version 20.218. There were 14 studies: 4 randomised controlled trials (RCT), 9 prospective, and 1 cross-sectional. The average improvement in UAS and TSS scores at the end of therapy was 42.24% and 41.24%. Results of subgroup analysis of AST administration in the group autologous serum skin test (ASST) positive and ASST negative based on the end of therapy UAS score (p=0.18). Results of subgroup analysis of AST administration in the positive ASST and negative ASST groups based on the TSS score at the end of therapy (p=<0.001). Results of subgroup analysis of AST administration versus placebo based on TSS score (p=0.861). Based on subgroup analysis, autologous serum therapy improves TSS scores in CSU patients (ASST positive). However, AST is not significantly different from placebo
Extensive Actinomycetoma Clinical Manifestations and Histopathology: A Case Report
Background: Actinomycetoma is a chronic infection of cutis, subcutaneous, bone, and viscera tissue. It is characterized by sinuses containing masses of causative organisms often referred to as "grains." Actinomycetoma is caused by aerobic bacteria, and they have been misclassified as a fungal species. Diagnosis of actinomycetoma was based on clinical features and confirmation by direct examination. This study aimed to describe the clinical and histopathological features of actinomycetoma.
Case presentation: A 34-year-old woman complained of a mass on her left leg that had increased in number and pain 3 months before admission to the hospital with a history of trauma. Regio pedis sinistra has trias mycetoma, such as edema (swelling), multiple sinuses, and grain with hypertrophic scars. Histopathological with fibro collagenous connective tissue, moderate to dense diffuse PMN inflammatory cells, plasma cells, lymphocytes, hemosiderophages, multinucleated giant cells, epithelioid cells, and microorganisms. In periodic acid Schiff (PAS) there is the Splendore-Hoeppli phenomenon.
Conclusion: Diagnosis of actynomycetoma was confirmed by the mycetoma triad and the Splendore-Hoeppli phenomenon. Actinomycetoma is more invasive and can cause bone destruction more quickly than eumycetoma
Adverse Skin Reaction to Hand Hygiene and Personal Protective Equipment among Health-Care Workers during COVID-19 Pandemic in Dr. Mohammad Hoesin General Hospital Palembang
Background: Indonesian health-care workers (HCWs) are at the front liners battling against the Coronavirus-2019 disease (COVID-19) pandemic since March 2020 and must protect themselves by hand hygiene and using personal protective equipment (PPE). Hand hygiene and the use of PPE are associated with an adverse skin reactions. This study aimed to find out the prevalence, characteristics, and risk factors for adverse skin reactions to hand hygiene and PPE among HCWs in Indonesia during the COVID-19 pandemic.
Methods: A cross-sectional online questionnaire-based survey among HCWs in Dr. Mohammad Hoesin General Hospital Palembang on April 1-30, 2022. The data were collected, including demographic characteristics, occupational-related characteristics, and adverse skin reactions.
Results: The study included 134 respondents, the majority of the respondents were female (57,5%), with a mean age was 31,18 ± 4,89 years old. Adverse skin reactions were reported only on 16,4% of respondents. Hands (40,9%) were the most common site affected. The highest symptom and lesion were itch (54,5%), erythema (59,1%) and scales (59,1%). There was a significant association between adverse skin reactions to hand hygiene (p <0,001) and PPE (p = 0,001) before and during the pandemic. However, adverse skin reactions were not associated with increased frequency of hand hygiene and duration of PPE worn. History of pre-existing inflammatory skin condition is the risk factor for adverse skin reactions to hand hygiene and PPE.
Conclusion: Adverse skin reactions to hand hygiene and PPE are increasing among Indonesian HCWs during the COVID-19 pandemic
Anatomy and Histologic of Intrinsic Aging Skin
Aging is an inevitable and dynamic biological process that is characterized by the progressive deterioration of body systems and declines in physiological reserve capacity. Aging skin has distinct two types: intrinsic and extrinsic. Intrinsic changes reduce collagen production, blood flow, amount of skin lipid, and loss of rete ridges. Intrinsic aging or chronological aging is cannot be restored to the skin with characterized by sagging skin and some expression of excess wrinkling lines. Intrinsic aging changes in thickness and characteristics of the epidermis, dermis, and hypodermis. Histologically, epidermis thinner by leveling off the dermo-epidermal junction. In the dermis, collagen fibers become thicker and irregular than younger skin, reducing the elasticity of the skin, while hypodermis reduces lipid volume

Ichthyosisvulgaris is a disease caused by disruption of epidermal differentiation process.Disrupted of profilaggrin conversion to filaggrin caused by mutations from thefilaggrin gene (FLG) located on chromosome 1q21. Recently, caused of ichthyosisvulgaris is mutation of the CASP14 gene on chromosome 19p13.12 which producescaspase-14, is involved in the proteolytic degradation of filaggrin. Clinicalmanifestations of ichthyosis vulgaris are hyperlinear palmar and plantar, keratosispilaris, xerosis, and localized or generalized scaling of the skin. Application ofemollients, humectants and keratolytic agents are the main treatment of ichthyosisvulgaris. Further research on caspase-14 as a therapeutic target is needed in thetreatment of ichthyosis vulgaris
Role of Androgen on Physiological Function of Pilosebaceous Unit
The pilosebaceous unit is a sebaceous gland with hair follicles. One of the hormones that affects the pilosebaceous unit is androgen, a steroid hormone that plays an important role in the expression of the phenotype of men and women. Androgen consist of testosterone and dihydrotestosterone which are activated by androgen receptors. Androgen in the pilosebaceous unit affect sebum production and hair growth.
