Effekte einer Nozizeptionsmonitor-gesteuerten Analgesie während Allgemeinanästhesie mit Sufentanil und Sevofluran
ZIELSETZUNG: Die Dosierung intraoperativer Opioide beruht aktuell maßgeblich auf der Bewertung klinischer Parameter. Mit dem Ziel einer bedarfsgerechteren Opioiddosierung wurden verschiedene Messverfahren zur Erfassung nozizeptiver Aktivität mit unterschiedlichen Messgrundlagen entwickelt. Die vorliegende Pilotstudie untersuchte die Effekte einer nozizeptionsmonitorbasierter Sufentaniltitration gegenüber einer klinisch orientierten Dosierung.
METHODEN: In der vorliegenden Studie wurden 48 Patienten im Zuge einer radikalen retropubischen Prostatektomie in Allgemeinanästhesie mit Sevofluran und Sufentanil in vier Gruppen randomisiert. Die nozizeptive Aktivität wurde mithilfe des Surgical Pleth Index (SPI), des Pupillary Pain Index (PPI) und des Nociception Level Index (NOL) erfasst. Die intraoperative Sufentanilgabe orientierte sich an einem der drei Monitorsysteme bzw. der Beurteilung klinischer Parameter (Kontrolle). Primärer Endpunkt war die intraoperative Sufentanildosis. Zusätzlich wurden das postoperative Schmerzniveau und die Piritramiddosierungen im Aufwachraum, die Aufwach- und Erholungszeiten sowie die Hormonspiegel von ACTH und Cortisol im Verlauf als sekundäre Endpunkte erfasst. Die Daten wurden mittels Kruskal-Wallis- und Mann-Whitney-U-Tests ausgewertet. Die Analyse der Hormonspiegel erfolgte mittels Regressionsanalyse mit einem gemischten Modell.
ERGEBNISSE: Die Sufentanilgesamtdosis (μg/min/kg x 103) variierte zwischen den Gruppen (Median [Quartile]: Kontrolle: 5,6 [4,4–6,4], SPI: 7,2 [4,8–8,4], PPI: 2,0 [1,8–2,9], NOL: 3,8 [3,3–5,1]; Hodges–Lehmann-Schätzer [99%-KI], p-Wert: SPI vs. Kontrolle: −1,6 [−3,7–1,7], p = 0,128; PPI vs. Kontrolle: 3,4 [2,0–4,6], p < 0,001; NOL vs. Kontrolle: 1,6 [−0,2–3,3], p = 0,017; NOL vs. PPI: 1,7 [0,6–3,4], p < 0,001; NOL vs. SPI: −3,0 [−5,2–0,2], p = 0,024; PPI vs. SPI: −5,1 [−6,6—1,3], p < 0,001). Die kumulativen Hormonspiegel als Flächenintegral (AUC) zeigten Unterschiede zwischen den Studiengruppen. ACTH-Konzentrationen (ng/l x min, log10, Mittelwert [99%-KI], p-Wert): NOL vs. PPI (−1,079 [−1,950––0,208], p = 0,001) und SPI vs. PPI (1,192 [0,317–2,068], p = 0,001); Cortisolkonzentrationen (μg/l x min, Mittelwert [99%-KI], p-Wert): SPI vs. PPI (46,710 [21,145–72,274], p < 0,001), SPI vs. NOL (27,645 [3163–52,126], p = 0,003) und SPI vs. Kontrolle (31,824 [6974–56,675], p = 0,001). Die weiteren sekundären Endpunkte zeigten keine Unterschiede.
SCHLUSSFOLGERUNGEN: Die Anwendung der Nozizeptionsmonitore resultierte in unterschiedlichen Sufentanilgesamtdosierungen. Geringere Dosierungen waren mit einer gesteigerten neuroendokrinen Stressreaktion assoziiert und umgekehrt. Diese Ergebnisse zeigen eine eingeschränkte Validität der Monitorsysteme zur indexbasierten Opioidtitration. Für den klinischen Routinegebrauch benötigen die untersuchten Nozizeptionsmonitore und die intraoperativen Zielbereiche der Nozizeptionsindizes noch weitere Entwicklungsarbeit
Prospective study of three analgesia monitors to observe the unconscious pain sensation during general anesthesia
Einleitung: Für die Steuerung der Opioiddosierung während der Allgemeinanästhesie sind seit einigen Jahren Analgesiemonitore erhältlich, die die Analgesie-Nozizeptions-Balance eines anästhesierten Patienten abschätzen sollen. In dieser Studie verglichen wir die Testqualitäten der Analgesieparameter Analgesia Nociception Index (ANI), den Surgical Pleth Index (SPI) und den Pupillendurchmesser (PD) mit den bisher üblichen, unspezifischen, klinischen Parametern Herzfrequenz (HR) und mittleren arteriellen Blutdruck (MAP) sowie mit dem Bispektral Index (BIS). Primäre Endpunkte waren die Fähigkeit Schmerzreize zu detektieren, die Vorhersagefähigkeit (PK) der Ausgangswerte für eine hämodynamische Reaktion und Abwehrreaktionen, und der Zusammenhang der Änderung der Parameter mit dem infundierten Opioid-Analgetikum.
Material und Methoden: Wir untersuchten 37 Patienten, die eine Allgemeinanästhesie mit Propofol und ansteigenden Remifentanil-Dosierungen erhielten und applizierten jeweils einen tetanischen sowie intrakutanen Schmerzreiz ohne und unter jeder Remifentanil-Dosierung. Die Ausgangswerte und die Änderungen der Parameter nach einem Schmerzreiz und deren Bezug zur Remifentanil-Dosierung werteten wir mithilfe von Receiver-Operating-Characteristic Kurven, Vorhersagewahrscheinlichkeiten (PK) und mixed-model Analyse aus.
Ergebnisse: In der Erkennung beider Schmerzreize unter alleiniger Propofolgabe waren der ANI (PK=0,98), der PD (PK=0,98) und der SPI (PK=0,87) den bisher verwendeten, klinischen Parametern, dem MAP (PK=0,75) und der HR (PK=0,74), und dem BIS (PK=0,55) überlegen. Anhand der Ausgangswerte konnten nur eingeschränkt durch beide Schmerzreize hervorgerufene hämodynamische Reaktionen oder Abwehrbewegungen vorhergesagt werden (ANI (PK=0,70), SPI (PK=0,63), PD (PK=0,67), BIS (PK=0,67)). Die Änderung des ANI, SPI, PD, MAP und der HR nach einem Schmerzreiz zeigten einen signifikanten Zusammenhang (p<0,001) zur Remifentanil-Dosierung, wohingegen die Änderungen des BIS keinerlei Zusammenhang (p=0,216) erkennen ließen.
Diskussion: Die drei untersuchten Analgesieparameter ANI, SPI und PD ermöglichten eine zuverlässige Detektion beider standardisierten Schmerzreize und waren den klinischen Parametern, HR und MAP, überlegen. Die Vorhersagekraft für Reaktionen aus den Ausgangswerten war jedoch stark limitiert. Aus einer guten Korrelation zwischen der Änderung von ANI, SPI sowie PD und der Remifentanildosierung schlussfolgern wir, dass tatsächlich nozizeptive Reize mit den Analgesiemonitoren abgebildet wurden. Unsere Daten zeigen, dass der BIS nicht geeignet ist, Schmerzen abzubilden.Introduction: The aim of this study was the comparison of the three analgesic indices Analgesia Nociception Index (ANI; heart rate variability), Surgical Pleth Index (SPI; photoplethysmography) and pupillary dilatation (PD), to heart rate, mean arterial pressure, and bispectral index. Primary endpoints were the diagnostic accuracy and prediction probability for nociceptive response as well as the correlation between Δ values and the remifentanil dose administered.
Methods: We included 37 patients anesthetized with propofol and increasing doses of remifentanil. Standardized tetanic and intracutaneous electrical painful stimulations were applied on each analgesic level. The baseline and the Δ values of ANI, SPI, PD, heart rate, mean arterial pressure and bispectral index were collected and their relations to remifentanil doses were calculated. Receiver operating characteristic curves, prediction probability (PK), and mixed-model analysis were used for evaluation.
Results: Analgesia Nociception Index (PK=0,98), Surgical Pleth Index (PK=0,87) and pupil dilatation (PK=0,98) were superior in detecting both painful stimulations compared to previously used clinical signs, mean arterial pressure (PK=0,75) and heart rate (PK=0,74), and bispectral index (PK=0,55). The baseline values had limited prediction probabilities towards hemodynamic changes and nocifensive movements (ANI (PK=0,70), SPI (PK=0,63), PD (PK=0,67), BIS (PK=0,67)). The delta values of ANI, SPI, PD, mean arterial pressure and heart rate showed a significant correlation (p<0,001) to remifentanil doses. Furthermore, remifentanil doses had no effect on ΔBIS (p=0,216).
Conclusions: The three analgesic indices ANI, SPI and PD enabled a reliable detection of both standardized pain stimulations and were superior to the clinical signs, heart rate and mean arterial pressure. However, the prediction probabilities of the baseline values were highly limited. Consequently, it seems reasonable that intermittent stimulation to test the patient’s responsiveness could increase the diagnostic value of the analgesic monitoring methods. On basis of the correlation between the delta values of ANI, SPI, PD and the remifentanil doses we conclude that nociceptive stimulations are reflected by the analgesic monitors. Our data confirm that the bispectral index is incapable to measure pain. Further studies are needed to prove the clinical benefit of the three analgesic indices
Vorhersagekraft von drei verschiedenen Nozizeptionsmonitoren für moderate bis starke postoperative Schmerzen
Die intraoperative Dosierung von Opioiden ist eine Herausforderung im anästhesiologischen Alltag und erfolgt meist anhand von Herzfrequenz, Blutdruck, Spontanbewegung und der Erfahrung der Anästhesist:innen. Eine effektive postoperative Schmerztherapie ist entscheidend für die Erholung. Es fehlen jedoch zuverlässige Methoden zur Abschätzung des Analgesiebedarfs vor dem Erwachen aus der Allgemeinanästhesie. Diese Studie testete das Nozizeptionsmonitoring, das als objektive Methode zur Steuerung der intraoperativen Analgesie entwickelt wurde, auf seine Fähigkeit, moderate bis starke postoperative Schmerzen vorherzusagen. Die Nozizeptionsindizes Surgical Pleth Index (SPI), Pupillary Pain Index (PPI) und Nociception Level Index (NOL) wurden hierfür untersucht.
Es wurden 60 Patient:innen eingeschlossen, die unter Allgemeinanästhesie einen orthopädisch-unfallchirurgischen Eingriff durchliefen. Die Herzfrequenz als klassischer Nozizeptionsparameter sowie SPI, PPI und NOL wurden in den fünf Minuten vor dem Erwachen gleichzeitig gemessen, um ihre diagnostische Genauigkeit direkt miteinander zu vergleichen. Ihre Vorhersagekraft wurde mittels ROC-Kurven, Vorhersagewahrscheinlichkeiten und multivariater logistischer Regressionsanalyse evaluiert.
Der NOL zeigte die beste Vorhersagekraft für postoperative Schmerzen (ROC-AUC = 0,661; P = 0,038), jedoch wies keiner der Indizes allein eine ausreichende diagnostische Genauigkeit auf. In Kombination mit anderen Variablen erreichte der NOL in der multivariaten Regressionsanalyse jedoch eine gute Vorhersagekraft (ROC-AUC = 0,83; P < 0,001). HF, SPI und PPI zeigten nur schwache Zusammenhänge mit dem postoperativen Schmerzniveau und Opioidbedarf, während der NOL moderate Korrelationen aufwies. Diese Ergebnisse sprechen gegen den Einsatz eines einzelnen Nozizeptionsmonitors zur Vorhersage postoperativer Schmerzen. Die multivariate logistische Regressionsanalyse deutet jedoch darauf hin, dass Nozizeptionsmonitore in Kombination mit weiteren Variablen verlässliche Aussagen über das Auftreten moderater bis starker postoperativer Schmerzen ermöglichen könnten. Dies könnte eine individuell angepasste perioperative Schmerztherapie begünstigen. Weitere Studien sind erforderlich, um zu prüfen, ob das Nozizeptionsmonitoring die postoperative Schmerztherapie und damit das Patientenoutcome verbessern kann.Intraoperative opioid dosing is challenging in routine anaesthesiology and typically relies on heart rate, blood pressure, spontaneous movement, and anaesthetists' experience. Effective postoperative pain management is essential for recovery, yet no reliable methods exist to assess analgesic needs before awakening from general anaesthesia. Nociception monitoring has been developed as an objective measurement method for controlling intraoperative analgesia. In this study its ability to predict moderate to severe postoperative pain was tested. We examined the nociception indices Surgical Pleth Index (SPI), Pupillary Pain Index (PPI), and Nociception Level Index (NOL).
A total of 60 patients undergoing orthopaedic-trauma surgery under general anaesthesia were included. The monitors were used simultaneously to directly compare their diagnostic accuracy. In the five minutes prior to awakening, the heart rate as a classical parameter for assessing intraoperative nociception as well as SPI, PPI and NOL were measured. The predictive power of the heart rate and the indices was analysed using ROC curves, predictive probabilities, and multivariate logistic regression analysis.
When comparing the nociception indices, the NOL achieved the best predictive power for postoperative pain levels (ROC-AUC = 0,661; P = 0,038). However, none of the indices demonstrated sufficient diagnostic accuracy to serve as a sole predictor for postoperative pain. In the multivariate logistic regression analysis, the NOL, in combination with other variables, showed good predictive accuracy (ROC- AUC = 0,83; P < 0,001). Heart rate, SPI and PPI exhibited only weak correlations with postoperative pain levels and opioid requirements, while the NOL showed moderate correlations.
Our results argue against the use of a single nociception monitor for predicting postoperative pain, as we found no clear correlation with postoperative pain levels or opioid requirements. However, the multivariate logistic regression analysis suggests that nociception monitors, in combination with other variables, could provide reliable predictions about the occurrence of moderate to severe postoperative pain, potentially supporting an individually tailored perioperative pain management strategy. Further studies are needed to determine whether nociception monitoring can improve postoperative pain management and thus enhance patient outcomes
Nociception level-guided opioid administration in general anesthesia
Einleitung: Die Nozizeption in Allgemeinanästhesie wird bisher zumeist anhand von Surrogatparametern wie Herzfrequenz und Blutdruck gemessen. Seit einigen Jahren sind nun Nozizeptionsmonitore erhältlich, welche eine gezielte Steuerung der Analgesie versprechen. In der vorliegenden randomisierten, kontrollierten Studie verglichen wir die Opioiddosierung während Allgemeinanästhesie in vier Gruppen: Surgical Pleth Index (SPI), Pupillary Pain Index (PPI), Nociception Level Index (NOL)) und einer Kontrollgruppe (Steuerung der Analgesie durch den/die Anästhesist:In). Primärer Endpunkt war die mittlere intraoperativ verabreichte Dosis Remifentanil. Sekundäre Endpunkte waren unter anderem der Verlauf der erhobenen Spiegel der Stresshormone Cortisol und Adrenocorticotropes Hormon (ACTH) sowie postoperative Schmerzen und Analgetikabedarf.
Material und Methoden: Eingeschlossen wurden 96 Patienten zur Radikalen Retropubischen Prostatektomie mit einer total-intravenösen Allgemeinanästhesie aus Propofol und Remifentanil. Es erfolgte die Randomisierung in eine von vier Gruppen, wobei die Analgesie mit Hilfe eines der drei Monitore oder nach klinischem Standard gesteuert wurde. Die definierten primären und sekundären Endpunkte wurden mittels Varianzanalysen (ANOVA, Analysis of Variance) und post-hoc Gruppenvergleichen analysiert, für die Berechnung der Hormonkonzentrationen wurde ein allgemeines lineares gemischtes Modell erstellt.
Ergebnisse: Die mittlere Remifentanil-Dosis zeigte einen Gruppenunterschied (p < 0,001), wobei die Patienten der PPI-Gruppe die niedrigste Menge (0,072 μg/kg/min) und die Patienten der SPI-Gruppe die größte Menge Remifentanil erhielten (0,501 μg/kg/min). In der NOL-Gruppe wurden im Mittel 0,189 und in der Kontrollgruppe 0,349 μg/kg/min Remifentanil verabreicht. Die Area Under the Curve (AUC) des Serum-Cortisolspiegels (μg/kg·min) zeigte eine inverse Proportionalität zur Opioiddosis: PPI 72,1; NOL 54,4; SPI 38,6; Kontrollgruppe 37,9 (p < 0.001). Der ACTH-Plasmaspiegel zeigte einen ähnlichen Verlauf, in den anderen sekundären Endpunkte ließ sich kein Gruppenunterschied nachweisen.
Diskussion: Die Verwendung der Nozizeptionsmonitore führte zu teilweise stark differierenden Opioiddosierungen, wobei niedrige Dosierungen mit einem starken Anstieg der Stresshormone ACTH und Cortisol assoziiert waren und vice versa. Dies lässt darauf schließen, dass die Nozizeptionsmonitore zum jetzigen Zeitpunkt noch nicht ausreichend validiert sind. Der Nutzen der Monitore für die intraoperative Anleitung der Opioiddosierung wird in weiteren Studien untersucht werden müssen

Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
