Asiakasohjaajien käsityksiä etäteknologian avulla tehtävästä ikääntyneiden kotiarvioinnista
No full textPäijät-Hämeen hyvinvointialueen ikääntyneiden asiakasohjauksessa on käytössä kotiarviointi eli asiakkaan kotona etäteknologian avulla tehtävä toimintakyvyn arviointi. Kotiarviointia käytetään harkinnanvaraisesti asiakkaan palvelutarpeen arvioinnissa pääasiassa asiakkaan ympärivuorokautisen palveluasumisen hakuprosessissa. Kotiarvioinnissa käytettävät etäseurantalaitteet on otettu käyttöön alun perin vuonna 2021, mutta niitä on päivitetty ja täydennetty vuonna 2025.
Tutkimuksen tarkoituksena oli kuvata asiakasohjaajien käsityksiä etäteknologian avulla tehtävästä kotiarvioinnista ja tiedon avulla tuottaa kehittämisehdotuksia kotiarviointiin. Tutkimus tehtiin soveltavana laadullisena tutkimuksena. Tutkimusaineisto kerättiin teemahaastattelun (n=8) sekä kyselytutkimuksen (n=32) avulla. Kerätty aineisto analysoitiin aineisto- ja teorialähtöisellä sisällönanalyysillä. Tutkittavat teemat käsittivät kotiarvioinnissa tarvittavan osaamisen, kotiarvioinnilla saadun tiedon käytön päätöksenteossa, sekä kotiarvioinnin arvioidut hyödyt ja koetut haasteet.
Tutkimustulosten mukaan asiakasohjaajat kokivat tarvitsevansa moniulotteista ammatillista osaamista voidakseen hyödyntää kotiarvioinnilla saatua tietoa päätöksenteossa. Kotiarvioinnin koetaan olevan päätöksenteon luotettavuutta lisäävä arviointimenetelmä, jolla voidaan vahvistaa asiakaslähtöisyyttä ja tukea hyvinvointialueen toimintaa. Kotiarvioinnin käytössä ilmenee kuitenkin osaamiseen, prosessiin, teknologiaan sekä suhtautumiseen liittyviä haasteita.
Tulosten perusteella asiakasohjaajat kokevat kotiarvioinnilla voitavan saavuttaa merkittäviä hyötyjä. Se edellyttää, että etäseurantalaitteilla kerättävä tieto on merkityksellistä ja menetelmässä tarvittavaa osaamista tuetaan organisaatiossa

The role of educative thought in the life and work of Antonio Gramsci
No full textMany philosophers have propounded a vision of an improved society, what distinguishes Antonio Gramsci is his continuous effort to make it happen by understanding the process in order to put into practice. Gramsci's conviction about the importance of educative development came from both theory and experience. While there has been considerable examination of Gramsci's work in relation to the Prison Notebooks, this study will seek to address a lacuna in Gramsci scholarship. Using Gramsci's philological method, I analyse Gramsci's pre-prison activity; his pre-prison articles and letters, which, together with his letters from prison, formed part of his educative mission. This educative process was necessary, in order to construct a new party which would develop a collective will, collaboratively, with the masses.In this study therefore, I explore the contexts and formative experiences of the first part of his life together with the intellectual sources from which Gramsci developed his later theories, making central hitherto underemphasised connections between them which informed his writing and ideas. I intend to illustrate that Gramsci's underlying purpose in his writing, and political activity, was not only practical, on how to create a new socialist ruling class, but also educative in forming the mindset and values of his comrades. So that in addition to outlining his vision of a new order, he implicitly guided or explicitly explained the processes by which the necessary changes in social relations and moral climate could be made in order to achieve it. Each person had to engage with the values of the new order so that each could contribute to the construction of a new robust state. It was essential to build a hegemony at the most profound level, one which was dependent on collective understandings and a collective will

No full textIl processo di digitalizzazione e lo sviluppo dei media, stravolgendo il paradigma tradizionale del copyright/diritto d’autore conducono a reazioni opposte. Da un lato, estendendo in vario modo l’ampiezza dell’esclusiva autorale favoriscono l’adozione di regole restrittive di accesso e uso dei contenuti; dall’altro, alimentano le logiche di condivisione, specie in alcune aree di produzione del sapere.
Il contratto, pur mutata la propria natura nella dimensione digitale, rappresenta la prima leva per l’affermazione di tali divergenti dinamiche, che, in entrambe le direzioni, riguardano anche la circolazione della conoscenza scientifica. Nel senso dell’apertura, lo strumento negoziale consente di perseguire i principi affermati dal movimento dell’Open Access (OA), abbattendo le barriere economiche e giuridiche all’accesso e utilizzo dei contenuti.
Dal deposito e pubblicazione su archivi istituzionali e disciplinari di opere transitate già attraverso i canali editoriali convenzionali, comunemente definita green road, alla pubblicazione su riviste ad accesso aperto, gold road, il fenomeno si sviluppa dal basso verso l’alto grazie alle dichiarazioni di principio e alle norme informali che hanno sin ora guidato le comunità accademiche nell’affermazione dell’OA. Di recente, tuttavia, i principi dell’OA sono oggetto di attenzione da parte del decisore pubblico che, pur timidamente, ne “impone” l’attuazione a tutte le comunità accademiche. Eppure, il diritto formale non sembra da solo sufficiente: è soltanto il primo tassello di una disciplina organica tesa a definire regole e incentivi per la produzione e la disseminazione della conoscenza scientifica, allo scopo di bilanciare la libertà “accademica” con il diritto di accesso alla conoscenza. = ENGLISH VERSION = Along with a comparative perspective that takes account of the U.S. and Italian law, this work aims to explore the interface between copyright and contract lae in publishing process.
In the current publishing environment, contracts and technology play a dominant role in the exploitation of copyrighted works. Publishers are granted by assignment of all copyright rights to reproduce and publish the work, but also to exercise control over its contents through technological protection measures. At the same time, mass digitization allows libraries and other organizations to make contents available online, which it entails a redefinition of the traditional publishing process and introduces new players to the scene (e.g., Google Books).
Hence, technology proves to be a powerful instrument for the spread of knowledge and it is on this pattern that Open Access (OA) is rapidly gaining ground.
Mostly based on a bottom-up approach that is on soft law, institutional policies and contracts, OA designs a new legal environment targeting the objectives of free accessibility, further distribution, and proper archiving of publications. These aims can be achieved through the creation of new open access business models to publish on OA journals (gold road) or to self-archive in institutional or disciplinary repositories works that have been originally published in conventional journals (green road).
However, in order for OA to be fully developed it is necessary to devise a principled and feasible approach to the dissemination of scholarly works against the current social, economic and legal background.
Indeed, the importance of OA is steadily recognized by legislators who integrate OA provisions into their legal system. This is an innovation of great significance, which was first fostered in the USA, and then extended in some European countries such as Italy and Germany in the European framework. Nevertheless, considering the different law systems, the formal law need to be combined with national strategies and institutional policies providing adequate incentives to the authors, while also promoting academic freedom and the right to knowledge access
Nenä- ja suumahaletkun laiton indikaatiot ja enteraalisen ravitsemuksen suositukset
No full textOpinnäytetyö tehtiin Laurea -ammattikorkeakoulun ja Helsingin ja Uudenmaan sairaanhoitopiirin (HUS) yhteisen hoitotyön laadunkehittämishankkeen osana vuosille 2013 – 2017. Hanke toteutetaan HYKS Pää- ja Kaulakeskuksen Neurokirurgian linjan kanssa.
Opinnäytetyön tarkoituksena oli selvittää neurokirurgisen potilaan nenä- ja suumahaletkun laiton indikaatiot ja enteraalisen ravitsemuksen suositukset. Tavoitteena oli tuottaa nenä- ja suumahaletkun käyttöä ja enteraalista ravitsemusta koskevat suositeltavat hoitokäytänteet tukemaan sairaanhoitajan ammattitaitoa, osaamista ja tietämystä neurokirurgisen potilaan hoidossa. Tarve hoitokäytänteille muodostuu nenä- ja suumahaletkun asentamiseen ja enteraalisen ravitsemukseen liittyvästä haastavuudesta. Komplikaatioiden mahdollisuus on olemassa ja niistä vakavimmat ovat johtaneet jopa kuolemaan.
Opinnäytetyössä tutkittiin kirjallisuuskatsauksen avulla, miten 2000-luvun tutkimustieto tukee nenä- ja suumahaletkun käyttöä ja enteraalisen ravitsemuksen toteutusta. Tutkimusaineisto etsittiin ennalta asetettujen hakukriteerien avulla myös Euroopan ulkopuolelta. Saadut tulokset teemoiteltiin ja niiden pohjalta vastattiin opinnäytetyön tutkimuskysymyksiin. Tulosten valossa luotiin lopuksi suositeltavat hoitokäytänteet nenä- ja suumahaletkun laittoon sekä turvallisen enteraalisen ravitsemuksen toteutukseen.
Löydetyistä tutkimuksista paljastui, että aiheesta löytyy heikosti tuoretta tutkimustietoa. 2000-luvulla on kiinnostuttu enemmän hoitohenkilökunnan mielipiteistä ja tavoista enteraalisen ravitsemuksen toteutuksessa sen sijaan, että tutkittaisiin oikeita, turvallisimpia tai hoidon kannalta tehokkaimpia hoitokäytänteitä. Alan kirjallisuudessa esitetyt ohjeet perustuvatkin vanhempiin tutkimuksiin. Saaduista suosituksista sairaanhoitajat saavat näyttöön perustuvaa tukea enteraalisen ravitsemuksen toteutuksessa. Jatkotutkimusaiheena suositeltavia hoitokäytänteitä voisi vielä jatkaa kattavimmiksi työelämän tarpeiden mukaisesti.This thesis was made as part of a nursing quality development project between Laurea University of Applied Sciences and the Hospital District of Helsinki and Uusimaa (HUS) for the years 2013-2017. The project is carried out with the Department of Neurosurgery at Helsinki University Central Hospital (HUCH).
The purpose of this thesis was to examine the indications of placing a nasogastric and an orogastric tube and the guidelines on enteral nutrition for neurosurgical patients. The aim of the thesis was to develop recommended treatment methods in order to support the nurses' professional skills, know-how and knowledge of treating a neurosurgical patient. The need for the guidelines is created by the challenges concerning inserting a nasogastric and an orogastric tube and enteral nutrition. The possibility of complications exists and the most severe ones have even been fatal.
This thesis is a literature review and it examines how the research papers written in the 21st century discuss the use of a nasogastric and an orogastric tube as well as enteral nutrition practices. The study material was searched also from outside of Europe with the criteria set in advance. The obtained results were themed and the research questions of the thesis were answered based on them. Recommended practices for inserting a nasogastric and an orogastric tube and giving enteral nutrition safely were developed in the light of the results.
The data revealed that there are only few research papers concerning the topic. In the 21st century there has been more interest in nursing staff’s opinions and customs as regards giving enteral nutrition rather than studying the correct, safest and most effective nursing practices. Guidelines presented in the literature are based on older research papers. The guidelines provide evidence-based support on giving enteral nutrition for the nurses. Further studies could discuss the guidelines even more comprehensively taking into account working life needs

No full textAnalysing the Ragusan medieval practice of designating a potential witness by pulling his ear, the author traces the same custom in the legal codes from South East Adriatic (from the islands of Mljet and Lastovo to Shkodër). Finding striking similarities with the antestatio rite in the Twelve Tables code of Roman law, and in a number of testimonies from the Roman literature, the author follows the emergence of a ritual of similar features in the early Germanic law collections (5th-8th c.), in the documents of the Austro- Bavarian region (8th-12th c.), and in the Old Slavic legal terminology. According to the author, the link between the existence of a virtually identical legal ritual in different areas and periods might be accounted by the Roman law tradition, yet basically nourished by the shared understanding of the ear as the seat of memory
