    The city and landscapes beyond Harold Pinter's rooms

    Pinter's dramas have been labelled as 'absurd', 'mysterious', 'enigmatic', 'taciturn'. There has been a constant tendency to reduce the idea of the 'Pinteresque' to language when Pinter is preoccupied with the tensions between reality and the world of the imagination. He has, actually and accurately, used theatre as a 'critical act' to denote the abstracted realities, and he has applied his language to embody his world-view - his concerns in the contemporary capitalist world. Pinter has journeyed from the room to the outside world, from the private to the public social space, and has identified an inescapable sense of pessimism and alienation, and investigated an alarming world of atrocities. There are cities and landscapes beyond Pinter's rooms, cities peopled by wandering, displaced figures surveying the self-estranged city that is modern consciousness, and landscapes where his people retreat into the private realms of memory and fantasy. This thesis explores the virtual geographies beyond Pinter's rooms through the vocabulary of some modernist theoreticians and social scientists, as there are significant parallels between their analytical observations and the poetic perceptions of Pinter, a practising artist, and the phantom images of his characters. Pinter's plays and film adaptations tend to portray the city as a colonial present, and the country as a mythological past. The 1970s' plays portray a community of isolation, urban decay, dispossession and suffering, through the figure of the 'flâneur' - his characters' subjective experiences, memories and fantasies in the metropolis. In these memory plays, men and women have different mental landscapes and desires. To some extent the city is both a male-constructed world and an image of the twentieth century; in both senses it is anti-human and in decline. In his 1980s mature plays, Pinter's lyrical interiors and serene landscapes are colonised by the metropolis. Here Pinter investigates a universally oppressive space filled with misery and social dislocation. The city destroys humanity in a decaying modem world. These plays identify the global city as the locus of existential alienation and as the centre of political power and oppression - a world of brute masculine power. The last two plays, in this study explore other wastelands of human isolation and suffering, and criticise the British suspicion of the 'intelligentsia'. Using scenes that are ingrained in the contemporary audience's physical memory, Pinter makes the distinction between being an active participant and being a witness, a 'spectator' in this alarming world. And thus, he criticises the tradition of mockery of the artistic and the intellectually curious in Britain, and urges a need for a 'politically curious', at politically questioning theatre-going society

    Characterization of water and wildlife strains as a subgroup of Campylobacter jejuni using DNA microarrays.

    Campylobacter jejuni is the leading cause of human bacterial gastroenteritis worldwide, but source attribution of the organism is difficult. Previously, DNA microarrays were used to investigate isolate source, which suggested a non-livestock source of infection. In this study we analysed the genome content of 162 clinical, livestock and water and wildlife (WW) associated isolates combined with the previous study. Isolates were grouped by genotypes into nine clusters (C1 to C9). Multilocus sequence typing (MLST) data demonstrated that livestock associated clonal complexes dominated clusters C1-C6. The majority of WW isolates were present in the C9 cluster. Analysis of previously reported genomic variable regions demonstrated that these regions were linked to specific clusters. Two novel variable regions were identified. A six gene multiplex PCR (mPCR) assay, designed to effectively differentiated strains into clusters, was validated with 30 isolates. A further five WW isolates were tested by mPCR and were assigned to the C7-C9 group of clusters. The predictive mPCR test could be used to indicate if a clinical case has come from domesticated or WW sources. Our findings provide further evidence that WW C. jejuni subtypes show niche adaptation and may be important in causing human infection

    Advances in ITP - therapy and quality of life - a patient survey.

    Current guidelines recommend glucocorticoids and splenectomy as standard 1(st) and 2(nd) line treatments for chronic immune thrombocytopenia (ITP). We sought to find out how German ITP-patients are treated with respect to these guidelines. Members of a patient support association ≥18 years with a self-reported history of chronic ITP>12 months were surveyed with a web-based questionnaire. 122 questionnaires were evaluated. 70% of patients had chronic ITP for more than 5 years and 20% an average platelet count of ≤30·10(9)/L. 41% of the patients reported haematomas or petechiae more than once or twice and up to 12 times or more per year and 17% oropharyngeal and nasal bleeds. 11% had been admitted to hospital during the last 12 months. 88% had received or currently receive glucocorticoids, 27% were splenectomised. IVIG had been given to 55%, rituximab to 22%, anti-D to 12%, ciclosporin to 7%, while complementary and alternative medical treatments had been used by 36%. 50 women responded to questions concerning pregnancy. 14 (28%) had been advised not to become pregnant. 23 reported pregnancies and 10 (44%) required treatment for their ITP during pregnancy. Glucocorticoids are the most common therapy for chronic ITP but complementary and alternative treatments already come second and less than ⅓ of patients are splenectomised. This and the frequent use of complementary medicines suggests patients' dissatisfaction with conventional approaches. Many patients receive off-label therapies. There is a major need for adequate counselling and care for pregnant ITP-patients

    Self assessment in religious education

    This research investigates the nature of pupil self-assessment in religious education. It considers the implications of theories of self-assessment as assessment for leaming for self-reflection in pedagogies of pluralistic religious education, and vice versa. Assessment for learning: Research on assessment has claimed that selfassessment is essential in formative assessment, to combat the negative effects of summative assessment. Other recent research has considered the situated nature of classroom practice. How would these classroom factors affect selfassessment in RE? Policy and pedagogy In religious education: The history of the current policy documents is analysed using policy scholarship, and the tension is revealed between measurable intellectual skills and a wider understanding of the place of religious education in developing tolerance and respect, both in the England and Wales, and internationally. Are policy and assessment properly aligned? Practitioner research: Virtue theory is developed as a research paradigm for practitioner research for professional development. Rigour is established through a reflexive use of qualitative, largely ethnographic methods, especially group interviews. Analysis includes consideration of pupils' assessment careers. Reflexive self-assessment: As a result of analyzing the data on assessment and religious education an original form of self-assessment is proposed. Reflexive self-assessment is a subject-specific model of self-assessment, linked to interpretive approaches. This harmonizes classroom self-assessment of both intellectual skills and intercultural values. The classroom conditions necessary to allow it to develop are examined. The implications of this for theories of self-assessment, learning autonomy and current policies of religious education are considered. Finally, the research is reviewed, notably the implications for researching and teaching, and future developments.     Phenotypic and Functional Characterization of Human Memory T Cell Responses to Burkholderia pseudomallei

In animal models, production of interferon-gamma (IFN-γ) is critical for resistance, but in humans the characteristics of IFN-γ production and the bacterial antigens that are recognized by the cell-mediated immune response have not been defined. Methods: Peripheral blood from 133 healthy individuals who lived in the endemic area and had no history of melioidosis, 60 patients who had recovered from melioidosis, and 31 other patient control subjects were stimulated by whole bacteria or purified bacterial proteins in vitro, and IFN-γ responses were analyzed by ELISPOT and flow cytometry. Findings: B. pseudomallei was a potent activator of human peripheral blood NK cells for innate production of IFN-γ. In addition, healthy individuals with serological evidence of exposure to B. pseudomallei and patients recovered from active melioidosis developed CD4+ (and CD8+) T cells that recognized whole bacteria and purified proteins LolC, OppA, and PotF, members of the B. pseudomallei ABC transporter family. This response was primarily mediated by terminally differentiated T cells of the effector–memory (TEMRA) phenotype and correlated with the titer of anti-B. pseudomallei antibodies in the serum. Conclusions: Individuals living in a melioidosis-endemic region show clear evidence of T cell priming for the ability to make IFN-γ that correlates with their serological status. The ability to detect T cell responses to defined B. pseudomallei proteins in large numbers of individuals now provides the opportunity to screen candidate antigens for inclusion in protein or polysaccharide–conjugate subunit vaccines against this important but neglected disease

    Derivation and validation of a simple, accurate and robust prediction rule for risk of mortality in patients with Clostridium difficile infection.

However, the derivation of a simple, evidence based prediction rule to assist patient management has not yet been described. METHOD: Univariate, multivariate and decision tree procedures were used to deduce a prediction rule from over 186 variables; retrospectively collated from clinical data for 213 patients. The resulting prediction rule was validated on independent data from a cohort of 158 patients described by Bhangu et al. (Colorectal Disease, 12(3):241-246, 2010). RESULTS: Serum albumin levels (g/L) (P = 0.001), respiratory rate (resps /min) (P = 0.002), C-reactive protein (mg/L) (P = 0.034) and white cell count (mcL) (P = 0.049) were predictors of all-cause mortality. Threshold levels of serum albumin ≤ 24.5 g/L, C- reactive protein >228 mg/L, respiratory rate >17 resps/min and white cell count >12 × 10(3) mcL were associated with an increased risk of all-cause mortality. A simple four variable prediction rule was devised based on these threshold levels and when tested on the initial data, yield an area under the curve score of 0.754 (P < 0.001) using receiver operating characteristics. The prediction rule was then evaluated using independent data, and yield an area under the curve score of 0.653 (P = 0.001).     The N-glycan Glycoprotein Deglycosylation Complex (Gpd) from Capnocytophaga canimorsus Deglycosylates Human IgG

    Author Summary Capnocytophaga canimorsus are Gram-negative bacteria from the normal oral flora of dogs and cats. They cause rare but severe infections in humans that have been bitten or simply licked by a dog or cat. Fulminant septicemia and peripheral gangrene with a high mortality are the most common symptoms. A surprising feature of these bacteria is their capacity to feed by foraging the glycan moieties of glycoproteins from animal cells, including phagocytes. Here we show that C. canimorsus can also deglycosylate human IgGs reinforcing the idea that this property of harvesting host glycoproteins may contribute to pathogenesis. We also unravel the complete deglycosylation system which belongs to a large family of systems devoted to foraging complex glycans, found exclusively in the Capnocytophaga-Flavobacteria-Bacteroides group, and whose archetype is the starch harvesting system Sus. It is the first system devoted to deglycosylation of glycoproteins to be characterized
