    Diagnostic Endoscopic Ultrasound in Pancreatology: Focus on Normal Variants and Pancreatic Masses

    Background: Endoscopic ultrasound (EUS) is a main tool in gastroenterology for both diagnosis and exclusion of pancreatic pathology. It allows minimally invasive assessment of various diseases or anatomic variations affecting the pancreas also with the help of new Doppler technologies, elastography, contrast-enhanced imaging including post hoc image processing with quantification analyses, three-dimensional reconstruction, and artificial intelligence. EUS also allows interventional direct access to the pancreatic parenchyma and the retroperitoneal space, to the pancreatic duct, pancreatic masses, cysts, and vascular structures. Summary: This review aimed to summarize new developments of EUS in the field of pancreatology. We highlight the role of EUS in evaluating pancreatic pathology by describing normal anatomic variants like pancreas divisum, pancreatic lipomatosis, pancreatic fibrosis in the elderly and characterizing pancreatic masses, both in the context of chronic pancreatitis and within healthy pancreatic parenchyma. EUS is considered the optimal imaging modality for pancreatic masses of uncertain dignity and allows both cytological diagnosis and histology, which is essential not only for neoplastic conditions but also for tailoring therapy for benign inflammatory conditions. Key Messages: EUS plays an indispensable role in pancreatology and the development of new diagnostic and interventional approaches to the retroperitoneal space and the pancreas exponentially increased over the last years. The development of computer-aided diagnosis and artificial intelligence algorithms hold the potential to overcome the obstacles associated with interobserver variability and will most likely support decision-making in the management of pancreatic disease

    The Gut-Brain Axis in Inflammatory Bowel Disease—Current and Future Perspectives

    The gut–brain axis is a bidirectional communication system driven by neural, hormonal, metabolic, immunological, and microbial signals. Signaling events from the gut can modulate brain function and recent evidence suggests that the gut–brain axis may play a pivotal role in linking gastrointestinal and neurological diseases. Accordingly, accumulating evidence has suggested a link between inflammatory bowel diseases (IBDs) and neurodegenerative, as well as neuroinflammatory diseases. In this context, clinical, epidemiological and experimental data have demonstrated that IBD predisposes a person to pathologies of the central nervous system (CNS). Likewise, a number of neurological disorders are associated with changes in the intestinal environment, which are indicative for disease-mediated gut–brain inter-organ communication. Although this axis was identified more than 20 years ago, the sequence of events and underlying molecular mechanisms are poorly defined. The emergence of precision medicine has uncovered the need to take into account non-intestinal symptoms in the context of IBD that could offer the opportunity to tailor therapies to individual patients. The aim of this review is to highlight recent findings supporting the clinical and biological link between the gut and brain, as well as its clinical significance for IBD as well as neurodegeneration and neuroinflammation. Finally, we focus on novel human-specific preclinical models that will help uncover disease mechanisms to better understand and modulate the function of this complex system

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

It allows minimally invasive differentiation of various diseases, with a minimal degree of inflammation or anatomic variations. EUS also enables interventional direct access to the pancreatic parenchyma and the retroperitoneal space, the pancreatic duct, the pancreatic masses, cysts, vascular structures for diagnostic and therapeutic purposes. Summary: This review aimed to summarize the new developments of EUS in the field of pancreatology, with special interest on inflammation and interventions. EUS enables way to perform pseudocyst drainage, necrosectomy, transenteral drainage and transenteric access of the main pancreatic duct, or the direct visualization or therapy of vascular structures adjacent to the pancreas. Key messages: EUS has a deep impact on pancreatology, and the development of new diagnostic and interventional approaches to the retroperitoneal space and the pancreas has increased in the last years exponentially, allowing minimal invasive diagnostics and therapy and avoiding surgery and percutaneous therapy

    T-Zell-Wanderung bei chronischer Entzündung und akuter Infektion des Gastrointestinaltrakts

    Cell trafficking is an essential prerequisite for the function of the immune system. In particular, surveillance of the body by T cells and their differentiation require complex and coordinated trafficking processes. These build on the basic elements homing, retention and recirculation, which are governed by cell- and tissue-specific surface markers such as integrins, cell adhesion molecules and chemokine receptors as wells as secreted mediators. In the gastrointestinal tract, immune cell trafficking secures the fight against invading pathogens and the tolerance towards commensal organisms. However, the complexity of those mechanisms predisposes for dysregulation and associated disease development. Consistently, a key role of immune cell trafficking in the context of inflammatory bowel diseases (IBD) has been shown and the blockade of gut homing with the anti-α4β7 integrin antibody vedolizumab is an established therapeutic concept today. The aim of this work was to examine translationally relevant aspects of intestinal immune cell trafficking. Three publications are included in this cumulative dissertation, which each focus on one of the mentioned elements homing, retention and recirculation: In a cohort study, the use of vedolizumab (homing) in IBD patients in the Department of Medicine 1 of the University Hospital Erlangen was analyzed. The data confirm the efficacy described in randomized trials in a “real world” collective with a high portion of complex and refractory cases. Moreover, it could be shown that lower disease activity is associated with a higher probability of response to vedolizumab therapy. In a translational study, the pathogenetic impact of tissue resident memory T cells (TRM cells, retention) on IBD was assessed. It turned out that these cells are increased in IBD and associated with early flares. In experimental colitis models, mice deficient for TRM cell transcription factors or depleted of TRM cells developed less severe colitis. Mechanistically, a crosstalk mechanism between TRM cells and innate immune cells could be identified, which leads to TRM-dependent recruitment of further effector cells upon antigen re-exposure. Together, TRM cells seem to play a central role for the development of flares and emerge as a promising future therapeutic target. Finally, in the light of the SARS-CoV-2 pandemic, lymphocytes from the peripheral blood of patients with COVID-19 were analyzed for their expression of gut homing markers to draw conclusions on whether their antigen contact had taken place in the gut-associated lymphatic tissue (recirculation). The number of α4β7-expressing T and B cells was substantially reduced, whether gastrointestinal disease symptoms were present or not. Together, these works mirror the diversity of intestinal immune cell trafficking and show, that cell trafficking in the context of acute intestinal infection and, in particular, chronic intestinal inflammation contributes to pathogenesis and is relevant for future therapeutic strategies, whether by optimizing existing or developing novel treatment options.Für die Funktion des Immunsystems sind Wanderungsvorgänge von Zellen entscheidend. Insbesondere im Rahmen der Surveillance des Körpers durch und der Differenzierung von T-Zellen finden komplexe und koordinierte Wanderungsprozesse statt. Diese bauen auf den Grundmotiven Homing, Retention und Rezirkulation auf, die je nach Zelle und Gewebe durch spezifische Oberflächenmoleküle wie Integrine, Zelladhäsionsmoleküle und Chemokinrezep-toren sowie sezernierte Botenstoffe kontrolliert werden. Im Bereich des Magen-Darm-Traktes wird durch Immunzellwanderung die Bekämpfung von Krankheitserregern und die Toleranz kommensaler Organismen sichergestellt. Die Komplexi-tät dieser Vorgänge prädisponiert zugleich aber für Fehlregulation und Krankheitsentstehung. Insbesondere bei chronisch entzündlichen Darmerkrankungen (CED) spielt Immunzellwande-rung eine wichtige Rolle und die Blockade von Einwanderungsprozessen durch den anti-α4β7 Integrin-Antikörper Vedolizumab stellt heutzutage ein etabliertes Therapieprinzip dar. Ziel der vorliegenden Arbeit war es daher translational relevante Aspekte der Immunzellwan-derung im Gastrointestinaltrakt zu untersuchen. Drei Publikationen gehen in diese Arbeit ein, sie widmen sich jeweils schwerpunktmäßig einem der genannten Grundmotive Homing, Re-tention und Rezirkulation: So wurde in einer Kohortenstudie der Einsatz des Antikörpers Vedolizumab (Homing) bei an der Medizinischen Klinik 1 des Universitätsklinikums Erlangen behandelten CED-Patienten analysiert. Die Daten bestätigen die in randomisierten Studien beschriebene Effektivität in einem ‚real world‘-Kollektiv mit einem hohen Anteil komplexer und refraktärer Krankheitsver-läufe. Zudem konnte gezeigt werden, dass eine geringere Erkrankungsaktivität ein Anspre-chen auf eine Vedolizumab-Therapie wahrscheinlicher macht. In einer translationalen Studie wurde die pathogenetische Bedeutung von gewebsansässigen Gedächtnis-T-Zellen (TRM-Zellen, Retention) bei CED untersucht. Es zeigte sich, dass diese Zellen bei CED vermehrt sind und zudem mit dem frühen Auftreten erneuter Schübe assozi-iert sind. In experimentellen Colitismodellen waren Mäuse mit einer Defizienz für TRM-Zell-Transkriptionsfaktoren oder nach Depletion von TRM-Zellen vor der Entwicklung von Colitis geschützt. Mechanistisch konnte ein Crosstalk-Mechanismus zwischen TRM-Zellen und Zel-len des angeborenen Immunsystems identifiziert werden, der zur TRM-abhängigen schnellen Rekrutierung weiterer Effektoren bei erneutem Antigenkontakt führt. Insgesamt scheinen TRM-Zellen somit insbesondere eine zentrale Rolle für die Auslösung von Schüben zu spielen und stellen ein vielversprechendes zukünftiges Therapieziel dar. Schließlich wurden vor dem Hintergrund der SARS-CoV-2-Pandemie Lymphozyten aus dem peripheren Blut von COVID-19-Patienten auf ihre Expression von Darmhomingmarkern ana-lysiert, um so Rückschlüsse darauf zu erhalten, ob ihr Antigenkontakt im darmassoziierten lymphatischen Gewebe stattfand (Rezirkulation). Die Zahl α4β7-exprimierender T- und B-Zellen war dabei unabhängig vom Vorliegen gastrointestinaler Symptome substantiell ernied-rigt. Zusammen spiegeln diese drei Arbeiten also die Vielfalt der intestinalen Zellwanderung wider und zeigen auf, wie diese im Kontext akuter Infektionen und insbesondere chronischer Ent-zündung von pathogenetischer Relevanz ist und Implikationen für zukünftige Therapiestrate-gien haben kann, sei es durch den optimierten Einsatz bestehender oder die Entwicklung neuer Therapieoptionen

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Effekte der Integration von Kollagen in einem Poly-ϵ-caprolacton-basierten Nanofasermaterial zur Herstellung eines bioartifiziellen Gefäßersatzes

    Die Erforschung und Herstellung von bioartifiziellen Gefäßprothesen, so genannten „tissue engineered vascular grafts“ (TEVG) stellt ein zentrales und wachsendes Feld im Bereich des Tissue Engineerings dar. Forscher rund um den Globus befassen sich mit der Suche nach einer Lösung für eines der elementaren Probleme der kardiovaskulären und regenerativen Medizin - der Versorgung verschiedener Gewebe mit Sauerstoff und Nährstoffen, sowie den Abtransport anfallender Stoffwechselendprodukte. Bisweilen werden verschiedene Gefäßprothesen aus Polyurethan (PU), Dacron® oder Teflon® verwendet, welche jedoch langfristig keine zufriedenstellenden Ergebnisse aufweisen. Seit geraumer Zeit steht das Elektrospinnen von Biopolymeren und nativen Proteinen im Fokus der Wissenschaft. Hierbei werden Nanofasern erstellt, welche die extrazelluläre Matrix (ECM) verschiedener Gewebe, in diesem Fall von Gefäßen, imitieren und somit die Beschichtung mit Gewebe-spezifischen Zellen fördern sollen. Diese bioartifiziell hergestellten Gefäßprothesen könnten möglicherweise in Zukunft überlegene Flusseigenschaften zeigen und somit die insbesondere bei niedrigen Flussraten häufig thrombosierenden Prothesen heutiger Tage ablösen. Folglich stellen TEVGs eine potenzielle Lösung für die oben genannte Problematik dar. Unter den stark erforschten Materialzusammensetzungen zeigen Poly-ε-caprolacton (PCL)/Kollagen TEVGs bereits gute Ergebnisse hinsichtlich der Biomechanik und den supportiven Eigenschaften für verschiedene Zelltypen. Unter der Annahme, dass es einen positiven Zusammenhang zwischen der beigefügten Rate an Kollagen Typ I und den Zell-supportiven Eigenschaften des Materialgemischs gibt, stellten wir plane, elektrogesponnene PCL-Scaffolds mit unterschiedlichen Beimengungen an Kollagen Typ I (zwischen 0% und 75%) her. Es wurden in vitro Biokompatibilitäts- und Morphologiestudien mit einer murinen endothelialen Progenitorzelllinie (T17b EPCs) durchgeführt, um die ideale Zusammensetzung von PCL und Kollagen Typ I zu bestimmen. Hierbei wurden T17b EPCs auf den verschiedenen Scaffolds ausgesät und über 72 Stunden mit einer Kontrollgruppe verglichen. Als Kontrollgruppe dienten T17b EPCs, die auf Gelatine ausgesät wurden. Im Fokus unserer Untersuchung standen die Proliferation, Apoptose, Zellmorphologie und Funktionalität hinsichtlich der Produktion endotheltypischer Botenstoffe. Hierbei erfolgten standardisierte Tests mittels WST-8, BrdU, Cell Death Detection ELISA und Vascular Endothelial Growth Factor (VEGF)-ELISA. Darüber hinaus erfolgten elektronenmikroskopische und Immunfluoreszenz-mikroskopische Analysen der Zell-beladenen Scaffolds. Entgegen unserer Annahmen zeigten sich nicht nur positive, sondern auch negative Effekte durch die Kollagenbeimischung. So ließen sich beispielsweise positive Effekte durch Kollagenbeimischungen auf die Zellviabilität (WST-8 Assay) und insbesondere Funktion nachweisen. Letzteres erfolgte durch den Nachweis der Produktion gewebespezifischer Effektormoleküle (hier: VEGF). In diesem Zusammenhang zeigte sich nach 72h eine statistisch signifikante Zunahme metabolisch aktiver Zellen bis zu einem Kollagenanteil von 25%. Des Weiteren ermittelten wir eine signifikant erhöhte VEGF Produktion ab 25% Kollagenanteil des Biomaterials. Dem gegenüber steht jedoch ein Verlust der Endothel – typischen Pflasterstein- artigen Monozellschicht bei Kollagenbeimengungen >25%. Darüber hinaus zeigte bereits reines PCL erstaunliche Eigenschaften hinsichtlich der Zellproliferation und Morphologie. Wir konnten in der reinen PCL Gruppe nach 72h eine gegenüber allen Kollagengruppen überlegene Proliferationsrate bei vergleichbarer Apoptoserate beobachten. Die auf reinem PCL Nanofaserscaffolds ausgesäten T17b EPCs nahmen bereits eine Endothel-typische Morphologie an. Zusammenfassend ließ sich durch unseren Versuchsaufbau demonstrieren, dass eine Poly-ε-caprolacton : Kollagen I – ratio von 75 : 25% (3:1) hinsichtlich der Herstellung von so genannten Tissue Engineered Vascular Grafts zu präferieren ist

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
